1.Relationship between nutritional status, psychosocial stimulation, and cognitive development in preschool children in Indonesia.

The purposes of the study were to analyze nutritional status, psychosocial stimulation, and factors affecting the cognitive development of preschool-age children. This study was conducted in the Village of Babakan, Sub-District of Dramaga, Bogor Regency, West Java. This cross-sectionally designed study was conducted with mothers who had preschool children aged 3-5 years as respondents. Fifty-eight children were included. The distribution of mother's educational level was quite diverse, and the largest percentage (44.8%) had senior high school education. Approximately 78% of the family income per capita was classified into the non-poor category and 22.4% into the poor category. The average mother's nutritional knowledge score was 76.7 +/- 2.5 (moderate category). Most of the preschool children (84.4%) had psychosocial stimulation scores in the moderate category (30-45). The nutritional status of children showed that 15.5% were underweight, 5.2% were wasted, 3.4% were severely wasted, and 19% of the children were in the short and very short categories (stunted). The stepwise regression results showed that psychosocial stimulation (P < 0.001), participation in early childhood education (P = 0.002) and nutritional status based on the height index for age (P = 0.028) had a positive and significant effect on cognitive development of the preschool children (adjusted R2, 0.434; P = 0.028).

2.Effects of Galohgor Nutraceutical Lactation Cookies on Breast Milk Volume and Lactose Concentration

Background:
Galohgor nutraceutical lactation cookies (NLC) contain polyherbal which is believed and consumed by postpartum mothers of Sundanese Ethnic in Indonesia to increase breast milk production. This study was aimed to analyze the effects of NLC cookies on breast milk volume and lactose concentrations of postpartum mothers.
Methods:
The experimental study with post-controlled design was conducted. Twenty eligible mother-infant pairs were enrolled and divided into two groups. They were randomly assigned to receive NLC (n=9) or original cookies (ORC, n=11). Each subjects consumed 4 pieces/d of NLC which containing 1 g galohgor nutraceutical powder/ pieces or ORC (without galohgor nutraceutical powder), for 14 days intervention period since the first day of giving birth. The measurement of breast milk production was carried out by baby weighing method using SECA 334 digital baby scale (SECA, Hamburg, Germany). A high-performance liquid chromatography method was used to determine lactose concentrations in breast milk. Data were analyzed by parametric Independent T-test or the nonparametric Mann-Whitney U-test and adjusted analysis of covariance.
Results:
NLC significantly increased (P<0.05) breast milk production, as shown by average breast milk volume of NLC compared to ORC, i.e., 557.0 (95% confidence interval [CI], 497.3–616.6) mL/d vs. 435.9 (95% CI, 382.3–489.6) mL/d. This result was supported by lactose concentration in breast milk of mothers who consumed NLC significantly higher (P<0.05), compared to ORC, i.e., 6.03% (95% CI, 5.66%–6.40%) vs. 5.18% (95% CI, 4.85%–5.51%).
Conclusion
NLC which containing galohgor nutraceutical can be recommended to increase breast milk volume and lactose concentration in the early postpartum period.
3.High participation in the Posyandu nutrition program improved children nutritional status.

The objective of this research was to analyze nutritional status and food consumption of children participating in the Posyandu nutrition program. A total sample of 300 children under five years had been drawn. Sample was divided into two categories namely high participation and low participation in the Posyandu nutrition program. The sample was selected from two sub districts of Cianjur District, West Java. The districts were areas with a high proportion of poor people and many of them take the benefits of Posyandu nutrition program conducted by the government. The participation of children (under five years old) in visiting Posyandus was relatively good, namely, 92.4% (for the high participation group). However, for the low participation group, the number of participating children was relatively low (28.3%). The average consumption of energy for children under five years old was still below the recommended dietary allowance < 80% of RDA, whereas the protein consumption was already above the RDA. The prevalence of underweight, stunting, and wasting among children were respectively 30.0%, 43.7%, and 12.3%. The activities at Posyandus had a positive impact on the nutritional status of children under five years olds, measured in terms of weight for age (W/A) and weight for height (W/H). The more frequent the visit to Posyandus, the better the nutritional statuses would be.

4.Acute Gastrointestinal Bleeding in COVID-19 Patients: A Systematic Review and Meta-Analysis

Background/Aims:
More than 100 million people to date have been affected by the coronavirus disease-2019 (COVID-19) pandemic. Patients with COVID-19 have a higher risk of bleeding complications. We performed a systematic review and meta-analysis to evaluate the outcomes of COVID-19 patients with signs and symptoms of acute gastrointestinal bleeding (GIB).
Methods:
A systematic literature search was carried out for articles published until until November 11, 2020, in the Embase, MEDLINE, Web of Science, and Cochrane Library databases. We included studies on COVID-19 patients with signs and symptoms of GIB.
Results:
Our search yielded 49 studies, of which eight with a collective 127 patients (86 males and 41 females) met our inclusion criteria. Conservative management alone was performed in 59% of the patients, endoscopic evaluation in 31.5%, and interventional radiology (IR) embolization in 11%. Peptic ulcer disease was the most common endoscopic finding, diagnosed in 47.5% of the patients. Pooled overall mortality was 19.1% (95% confidence interval [CI]; 12.7%-27.6%) and pooled mortality secondary to GIB was 3.5% (95% CI; 1.3%–9.1%). The pooled risk of rebleeding was 11.3% (95% CI; 6.8%–18.4%).
Conclusion
The majority of COVID-19 patients with GIB responded to conservative management, with a low mortality rate associated with GIB and the risk of rebleeding. Thus, we suggest limiting endoscopic and IR interventions to those with hemodynamic instability and those for whom conservative management was unsuccessful.

6.Dermatomyositis Following BNT162b2 mRNA COVID-19 Vaccination

Dermatomyositis (DM) is one of the uncommon multi-organ idiopathic inflammatory myopathies that has been reported following the hepatitis B, Influenza, tetanus toxoid, H1N1, and BCG vaccines. However, an association with the coronavirus disease 2019 (COVID-19) vaccine is yet to be reported. In this case, we present the case of a 43-year-old Asian Indian female who was diagnosed with DM 10 days after receiving the second dosage of BNT162b2 mRNA COVID-19 vaccination, in the absence of any additional triggering factors. The diagnosis was established based on physical examination, serological antibodies, magnetic resonance imaging of the muscles, skin biopsy, and electromyography. She received standard treatment for DM, including oral high doses of prednisolone, hydroxychloroquine, mycophenolate, and physiotherapy. The treatment successfully reversed skin changes and muscle weakness. This is the first reported case of classic DM complicated by interstitial lung disease following COVID-19 vaccination. More clinical and functional studies are needed to elucidate this association. Clinicians should be aware of this unexpected adverse event following COVID-19 vaccination and arrange for appropriate management.
