Objective To investigate the role of RhoA/Rho associated kinase-2 (RhoA/ROCK-2) in the development of pulmonary hypertension (PH) in patients with obstructive sleep apnea-hypopnea syndrome (OS-AHS). Methods Thirty patients diagnosed as OSAHS by polysomnograshy(PSG) test in our sleep laberatoty were recruited as the observation group, and fifteen healthy subjects matched in gender, age and body mass index (BMI) were recruited as the controls. Pulmonary arterial pressure was measured by echocardiography. Serum RhoA/ROCK-2 levels were measured. Results The level of PAP was (47.30±12.85)mm Hg in OSAHA patients complicated with PH, (22.31±3.07)mm Hg in OSAHA patients without PH, which were significantly higher than that in the controls (19.47±1.92) mm Hg (W=175.50, P < 0.05). The serum RhoA and ROCK-2 in OSAHA patients with-out PH (10.43±3.10 and 22.31±16.10 μ/L, respectively) were significantly higher those in the controls (2.94±1.20)μg/L and (6.04±0.28)μg/L, respectively) (W=120.00, W= 121.00, respectively, P<0.05), whereas significantly lower than that in OSAHA patients complicated with PH(14.85±8.49)μg/L, (36.81±12.69) μg/L, respectively) (H =29.172, H =30.242, respectively, P <0.05). There was a positive correlation between PAP and AHI in patients with OSAHS, whether complicated with PH or not(r_s=0.793, r_s=0.887,P <0.05), and there was a negative correlation between PAP and LSaO2 in patients with OSAHS (r_s=-0. 562,r_s = -0.751, P <0.05). There were positive correlations between the level of RboA/ROCK-2 and PAP in patients with OSAHS(r_s = 0.793,r_s = 0.887,P < 0.05). Finally, there was a positive correlation between the level of RhoA and ROCK-2 in patients with OSAHS (r_s = 1.000,r_s = 1.000,P < 0.05). Conclusions OSAHS is an inde-pendent risk factor for pulmonary hypertension. The levels of serum RhoA/ROCK-2 in OSAHS patients with PH were increased in the development of the disease. It may play an important role in the process of pulmonary hyper-tension in patients with OSAHS.

OBJECTIVE:To study the clinical efficacy and adverse drug reactions (ADR) of paclitaxel combined with oxaliplatin and 5-fluorouracil (5-Fu) in the treatment of advanced gastric cancer.METHODS:31 patients with advanced gastric cancer were treated with paclitaxel 175 mg?m-2 (i.v.) for 3 hours followed by oxaliplatin 85 mg?m-2 on the first day and 24 hour infusion of 5-Fu (500 mg?m-2,first to fifth day) every 3 weeks.Total effective rate, survival duration and ADR were observed.RESULTS:The total effective rate was 38.7% (12/31),including complete remission of 9.7% (3/31),partial remission of 29.0%(9/31),steady progress (SD) of 41.9% (13/31) and promotion progress (PD) of 19.4% (6/31).The median time to progression (TTP) was 5.9 months and median survival time (MST) was 10.7 months. The total response rates were 70% for newly diagnosis patients and 23.8% for recurrence patients. The main ADR were neurotoxicity (89.1%),leukopenia (82.6%),nausea/vomiting (72.8%) and alopecia (66.3%).None of chemotherapy-related death occurred.CONCLUSION:Paclitaxel combined with oxaliplatin and 5-Fu POF regimen appears to be effective and tolerated in the treatment of advanced gastric cancer.

By analyzing repeated judicial expertise cases of illegal practice,we find it a serious threat to patients' health and safety, and more deep-seated reasons for illegal medical practice of medicine are discovered as follows.Investigators and expertise staff have to face legal, ethical, social and other factors when judging the relationship between illegal medical practice and adverse consequences(patient injury or death),which are quite worth exploration in depth to clear up the complicated relationships.

The application of Pseudomonas aeruginosa-mannose-sensitive hemagglutinin(PA-MSHA)in the field of an-titumors has been increasing.PA-MSHA has been found to promote tumor cell apoptosis, inhibit tumor cell invasion and migration, differentiation, and change drug resistance and epithelial-mesenchymal transformation(EMT)by inhibiting the EGFR pathway.Meanwhile,PA-MSHA also enhances the immune killing and inhibition of macrophages and T cells to tumor cells through toll-like receptors(TLRs).In this paper,we reviewed the reported anti-tumor mechanism and clinical application of PA-MSHA,suggesting that PA-MSHA may alter the glycosylation of EGFR and TLR proteins by acting on the regulatory process of the cellular mannosy-lation process.PA-MSHA can act on cell membrane proteins, including more receptors with high-mannosylation of signaling path-ways.Elucidating the deep relationship between PA-MSHA and mannosylation is of great significance for the mechanism research and clinical application of PA-MSHA.

ObjectiveTo analyze the community structure and dynamics of parasitic fleas on the body surface of host animals and nested fleas in different seasons in the natural foci of wild rat plague in Yulong County,Yunnan Province, to explore the relationship between seasonal fluctuation of fleas and the prevalence of plague among animals, so as to provide evidence for plague prevention and control in the natural foci. MethodsNanxi Village, Huangshan Town, the core area of plague epidemic in Yulong County, was selected as the monitoring sample area in December 2019 (winter), August 2020(summer), October 2020(autumn) and March 2021(spring). Host animals were captured by rattrap at night and rat nests were excavated for collecting parasitic fleas on host animals and rat nest fleas in different seasons. Excel 2010 and SPSS 26.0 software were used to analyze the data, and Chi square test was used to compare the rate. Community ecological indicators were used to analyze the community structure and species diversity of the host animals and their parasitic fleas. ResultsA total of 355 vector fleas were captured, belonging to 7 species of 5 genera in 2 families. 441 small animals were captured and 138 rat body fleas were detected with the flea infection rate of 14.51% and the flea index 0.31. 96 effective rat holes were excavated and 217 fleas were detected with the flea infection rate of 35.42% and the flea index 2.26. Among the four seasons, the flea infection rates of rat body and rat nests were higher in summer and winter, showing a significant difference in general (χ2=15.851, P<0.01; χ2=16.398, P<0.01). The dominant species of flea community were Ctenophthalmus quadratus, Stenischia humilis, Neopsylla specialis and Frontopsylla spadix, with a dominance index of 0.434, 0.254, 0.180 and 0.110, respectively. The diversity and evenness of rat body fleas showed a distribution characteristic of decreasing, increasing and then decreasing again with season changes, and both were the highest in spring, while the ecological dominance showed an opposite trend. The diversity and evenness of rat nest fleas showed a distribution characteristic of increasing first and then decreasing in summer, autumn and winter, with the highest in autumn, while the ecological dominance was diametrically opposite. ConclusionThe fleas community structure is relatively stable in Yulong County, but the number of species in the community is unevenly distributed by seasons, and the status of dominant species is prominent. Local authorities should carry out timely preventive deratization and depulization measures according to the results of daily monitoring, so as to effectively avoid the prevalence and spread of plague among animals in plague foci.
