1.Clinical Analysis of Hashimoto′s Thyroiditis Combined with Hyperthyroidism in 56 Children

Objective To explore the clinical feature,diagnosis,treatment and prognosis of Hashimoto′s thyroiditis(HT) combined with hyperthyroidism in children.Methods The clinical features of 56 children with HT combined with hyperthyroidism,including clinical features,complications,thyroid hormone,thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb),the detection of ultrasonic imaging and fine needle aspiration biopsies,diagnosis,treatment and prognosis were analyzed.Results The proportion of men to women was 16 in all 56 children,and the mean age at diagnosis was(9.95?2.09) years.The percentage of positive TGAb and TPOAb were 93% and 98%,respectively in all the 56 children.One child was diagnosed as HT combined with hyperthyroidism coexistent and myasthenia gravis type Ⅰ.The duration of hyperthyroidism ranged from 3 to 24 months.There were 14 children who had hypothyrodism and 8 children were euthyroid in all the 56 children.Conclusions Children with HT hyperthyroidism are more frequent in young females.TGAb and TPOAb are important markers for the diagnosis of HT.Pharmacotherapy is the first choice to treat HT hyperthyroidism.Hyperthyroidism is a clinical process of HT,and the percentage of children with hypothyrodism is increasing along with the development of disease.

Objective To investigate the relationship between the imbalance of suppressors of cytokine signaling 1(SOCS1)/SOCS3 and abnormal activation of monocytes/ macrophages in children with acute-phase asthma, and explore the molecular mechanism of chronic inflammatory process on airway.Methods The present study enrolled 20 asthmatic children and 20 age-matched normal children. 4.Clinical application of the compound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels

Objective To explore the method of clinical application and the final result of the com- pound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels.Methods Basing on the anatomic basis that the nutrient vessels of saphenous nerve-great saphenous vein have the same deriva- tion as the branches of the muscle,bone and skin.Eight cases of the tissue defects of ankles and feet were re- paired by the flap.Results Seven cases were survived completely except the distal partly necrosis in one case.The calcaneal defects of one case were repaired by the iliac graft after two weeks of the operation.Follow up from 3 monthes to one year,the texture,appearance and function of the multiflap were good.Conclusion The compound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels has the ad- vantages as follow:the reliable blood supply,avoidance of sacrificing the major arteries,reconstruction of sen- sation and simply procedure.Besides,the defects of soft tissue and bone can be repaired on one operation.

6.Analysis of monitoring results of Kaschin-Beck disease in Yan'an city Shaanxi province from 2006 to 2008

Objective To find out the situation of Kashin-Beck disease and it's dynamic changes in Yan'an city, and provide basis for decision-making on prevention and control of the disease. Methods In accordance with the national monitoring program on Kaschin-Beck disease, 21 villages were selected from 6counties in Yan'an, clinical and X-ray inspection of 7 to 13 year-old children were made according to historical illness information from 2006 to 2008. Clinical monitoring on Kashin-Beck disease was made in 2008 for people over 16 years old from 5 villages in 5 counties. Results One thousand and one hundred eighty children were found positive with the illness from 2006 to 2008, with 7 cases of grade Ⅰ , the detection rate was 0.59%(7/1180).One thousand and one hundred sixty-two people were taken X-ray photo on right hand, with no positive case found.In 2008, 1444 adults were taken clinical examination, the detection of grade Ⅰ and over were 160 cases, the detection rate was 11.08%(160/1444), mainly in the age of 36 years[93.75%(150/160)]. Conclusion KashinBeck disease in Yan'an city is in stable condition, but surveillance and preventing measures are stilled needed.
7.Effect of kebimin decoction on serum levels of adhesion molecule and Th1/Th2 cytokines in patients with allergic rhinitis.

OBJECTIVETo investigate the mechanisms of Kebimin decoction (KD) in treating allergic rhinitis (AR).
METHODSSixty patients with AR were randomly divided into two groups, patients in the treated group (TG) were treated with KD, those in the control group (CG) were given Xinfang rhinitis capsule. The serum levels of adhesion molecule (sICAM-1, P-selectin), Th1 cytokines, including interferon-gamma (IFN-gamma) and interleukin-2 (IL-2), and Th2 cytokines (IL-4 and IL-5) were determined by enzyme-linked immunosorbent assay (ELISA) before and after treatment, and compared with those in 30 healthy subjects as control.
RESULTSThe serum levels of adhesion molecule and Th2 cytokines were higher and Th1 cytokines were lower in AR patients than those in healthy control (P < 0.01). After treatment, these indexes were restored significantly (P < 0.01) in the treated group, but no significant change was found in the control group.
CONCLUSIONKD has definite effect in treating allergic rhinitis by inhibiting production of adhesion molecule and regulating the levels of Th1/Th2 cytokines to correct the imbalanced Th1/Th2 cytokines network.

8.Expandable implants in immediate breast reconstruction.

OBJECTIVETo investigate the indications, surgical methods and esthetic results in patients undergoing immediate breast reconstruction with expandable implants.
METHODSFrom January 2002 to March 2006, immediate breast reconstructions were carried out with expandable implants following mastectomy in 30 breast cancer patients. Nipple-areola were preserved in 5 patients.
RESULTSVery satisfactory results were achieved. Minor complications were recorded in 3 patients, two with local skin necrosis, and another with partial necrosis of nipple-areola complex.
CONCLUSIONSThe techniques provide better results for the patient who is not a candidate for simple implant reconstruction. These techniques also eliminate the need for a second major surgery to replace a temporary expander with a permanent implant in those patients needing soft tissue expansion. It is a simple procedure with advantages of no additional scarring, short time for convalescence.

9.Influence of Radix Astragali on nitric oxide and endothelin-1 in pulmonary tissue in hypoxemic pulmonary hypertension in rats.

OBJECTIVETo study the influence of Radix Astragali (RA) on pulmonary tissue endothelin-1 (ET-1) and nitric oxide (NO) in hypoxic pulmonary hypertension rats.
METHODSTwenty one healthy male Wistar rats weighing 210-310 g were divided into three group at random with 7 in each. The rats in control group were raised in ordinary room condition; those in hypoxic group were raised in ordinary pressure hypoxic box [concentration of O(2) was (10.0 +/- 0.5)%] for 8 hours a day, for 30 days; those in RA group were raised in the same condition as hypoxic group and treated with an intraperitoneal injection of RA 8 g/kg per day. The rats in the control group and hypoxic group were given the same volume of intraperitoneal injection of normal saline. Mean pulmonary arterial pressure (mPAP), mean carotid artery pressure (mCAP) were measured via right cardiac catheterization, concentration of NO in pulmonary tissue was measured by radioimmunoassay.
RESULTS(1) The mPAP (mm Hg) (21.9 +/- 1.6) and ET-1 (pg/ml) (309.1 +/- 58.1) in hypoxemic group were significantly higher than those in RA group (16.2 +/- 0.8, 287.7 +/- 57.5) and control group (15.3 +/- 0.8, 241.1 +/- 52.5) (P < 0.01, < 0.05), but the difference between RA group and control group was not significant. (2) NO (micromol/L) in pulmonary tissue in hypoxemic group (6.5 +/- 0.3) was lower than that in RA group and control group (9.2 +/- 0.9), NO in RA group was higher than that in hypoxic group but lower than that in control group (P < 0.05). (3) There was no significant difference in mCAP among the three groups (P > 0.05). (4) Under electron microscope, the endothelial cells of arterioles of the lung tissue of control group were flat and had normal morphology. However, in the lung tissue of hypoxic group, there were proliferation, hypertrophy and swelling of endothelial cells of pulmonary medium and small arteries and plenty of mitochondria and endoplasmic reticula in cytoplasm.
CONCLUSIONChronic hypoxia can result in reconstruction and endothelial lesion in pulmonary arterioles of rats, elevation of mPAP and ET-1 in pulmonary tissue, and decrease of NO. Injection of Radix Astraglai can reverse the reconstruction of pulmonary vessels partially, regulate the concentration of ET-1 and NO in pulmonary tissue, which may have certain therapeutic effects on pulmonary arteriolar changes induced by hypoxia.

10.Roles of CD8+ CD28- T regulatory cells in acute infectious mononucleosis in children.

OBJECTIVETo explore the role of CD(8)(+)CD(28)(-) T regulatory cells (Tr) in the immunological pathogenesis of acute infection with Epstein-Barr virus in children.
METHODSThe present study enrolled 25 children with infectious mononucleosis (IM) and 25 age-matched healthy children. Flow cytometric analysis was performed to detect the percentage of CD(3)(+), CD(3)(+)CD(4)(+), CD(3)(+)CD(8)(+), CD(8)(+)CD(28)(+) by determining the ratio of positive cells in lymphocytes. Reverse transcriptase-polymerase chain reaction (RT-PCR) and real-time PCR were used to analyze IL-6, IL-10, IFN-gamma expression in CD(8)(+)CD(28)(-) Tr cells and ILT-3, ILT-4 expression in monocytes/macrophages.
RESULTSThe proportions of CD(8)(+)CD(28)(-)T cells in children with acute-phase IM was significantly higher than those in the controls (P < 0.01). The expression level of IL-6, IL-10, IFN-gamma, ILT-3, ILT-4 mRNA significantly increased compared to those of the controls (P < 0.01).
CONCLUSIONThe CD(28) expressed on CD(8)(+) T cells in vivo is gradually lost with age and CD(8)(+)CD(28)(-) cells increase up 50% to adult. EBV can directly infect B cells, trigger CD(8)(+) CTL response and destroy the target cells to cause serious immunopathological lesion. Therefore we speculate that the expansion of CD(8)(+)CD(28)(-) Tr cells in children with IM may be an adaptive immune response to avoid serious inflammation and autoimmune reactions.
