2.Embolization of cerebral dural arteriovenous fistula of the cavernous sinus through the superior ophthalmic vein approach

Objective To evaluate the clinical effect of embolization of cerebral dural atreriovenous fistulas (cDAVF) of the eavemous sinus through the superior ophthalmic vein approach. Methods Twnety-seven patients with eDAVF of the cavernous sinus were embolized through the superior ophthalmic vein approach. Cerebral angiography and follow-up examination of the patients were performed to evaluate the effect ofernbolization. Results The fistulae showed complete angiographic disappearance in 15 patients, and 12 patients had blood velocity flow reduction at the fistula orifice. Ocular proptosis and chemosis deteriorated transiently in 11 patients after the procedure. The patients were followed-op for 3 to 48 months, and clinical cure was achieved in 17 patients, and 10 showed significant symptom relief. Conclusion cDAVF of the cavernous sinus can be effectively embolized through the superior ophthalmic vein approach.
3.Pituitary prolactinoma with severe erectile dysfunction as the initial symptom: diagnosis and treatment of 4 cases.

OBJECTIVEPituitary prolactinoma with severe erectile dysfunction (ED) as the initial symptom is often misdiagnosed. This article explores the diagnosis and treatment of severe ED caused by pituitary prolactinoma.
METHODSWe retrospectively analyzed the diagnosis and treatment of 4 cases of pituitary prolactinoma with severe ED (IIEF-5 score 5 - 7) as the initial clinical symptom confirmed by MRI.
RESULTSThe 4 cases of pituitary prolactinoma-induced severe ED, with serum prolactin 10 times above the maximum normal level, were misdiagnosed for 2 years. All failed to respond to the PDE5 inhibitor therapy, and then 3 of them underwent transnasal hypophysectomy. Twenty-four months of follow-up found the level of prolactin restored to normal in 1 case (IIEF-5 = 19), and reduced to 600 and 768 IU/L respectively (IIEF-5 = 15) in the other 2. Then administration of the PDE5 inhibitor was followed, which produced satisfactory efficacy. One case was treated with oral bromocriptine, which restored the prolactin level to normal at 12 months (IIEF-5 > 21).
CONCLUSIONProlactin detection and brain MRI can help to confirm pituitary prolactinoma with severe ED at the onset. As for its treatment, in case of an extremely high level of prolactin, simple administration of the PDE5 inhibitor is ineffective. When the prolactin level is reduced after surgery or medication, the symptom of ED can be improved and, in case of no obvious relief, administration of the PDE5 inhibitor can be followed, which may achieve satisfactory results.

4.The gene expression patterns of genetic P77PMC epilepsy-prone rats' hippocampus.

OBJECTIVETo find out the differentially expressed genes in the hippocampus of the rats with genetic epilepsy so as to lay a foundation for exploring the pathogenesis of epilepsy by means of cDNA array technology.
METHODSGene expression patterns in the hippocampus of the genetic epilepsy-prone P77PMC rats and normal Wistar rats were established using the alpha-32P-labeled cDNA probes hybridized with the Atlas Rat cDNA Expression Array, and then were analyzed by an image analysis instrument to get the differentially expressed genes.
RESULTSFifteen genes were found having differential expression patterns in hippocampus between the P77PMC rats and the Wistar rats, while there may be many other differentially expressed genes left undiscovered due to having no appropriate image analysis software. And among the fifteen genes, the expression levels of twelve genes in the P77PMC rats were higher than those in the Wistar rats, while the expression levels of the other three genes were lower. The results of reverse transcription-polymerase chain reaction(RT-PCR) have demonstrated the reliability of cDNA arrays method.
CONCLUSIONcDNA array is a powerful tool for identifying differential expression genes of epilepsy on large scales. There are several differentially expressed genes in hippocampus of the P77PMC rats and the Wistar rats. All these identified genes could play potentially important roles in the pathogenesis of epilepsy.

5.Study on the injurious effect of a self designed micro-skin machine on the epithelia.

OBJECTIVETo observe the injury on micro-skin induced by a self designed micro-skin machine.
METHODSMicro-skin was produced either with the machine or by hand. Cells at the edge of micro-skin were observed by transmission electron microscope. succinic dehydrogenase activity in supernatant of cultivated cells was analyzed, and the cell proliferation of micro-skin was assessed by (3)H-TdR. Twenty patients were enrolled in the study for the observation of the wound healing time between the two groups of micro-skin after being grafted.
RESULTSTransmission electron microscope examination revealed that the cellular injury at the edge of the micro-skin in machine-made group was mild compared with that in man-made group. (3)H-TdR rate was elevated but the activity of succinic dehydrogenase in the supernatant of cultured cells decreased in supernatant of cultured cells of machine produced micro-skin. Wound healing time was shortened in machine made group. (P < 0.05).
CONCLUSIONThe cellular injury at the edge of micro-skin in the machine made group was mild when compared with that in the man-made group with cell proliferation accelerated and wound healing time shortened.

6.Clinical analysis of a family with familial hemangioblastoma

Objective To analyze the clinical and imaging features of a family(3 patients)with familial hemangioblastoma,and their diagnosis and prognosis.Methods The detailed data about clinical and imaging features of all patients diagnosed as familial hemangioblastoma,admitted to our hospital from October 2004 to May 2010,were analyzed,and the lesions of other regions,besides the tumor lesion,were observed.Results No lesions of other regions were noted in these 3 patients.Cranial MRI showed that 2 had cystic and solid tumor and 1 had solid tumor;,total removal was performed on these patients under microscope; regular follow-up was given and no recurrence was noted.Conclusion Familiar hemangioblastoma is serious hereditary disease; and MRI is the most important detective method; microsurgical operation is the most important therapy.Early diagnosis and treatment should be given to the patients with familiar hemangioblastoma due to its high recunence rate,having difficulty in operation and its trend to combining with other lesions of the other parts.
7.Efficacy analysis of coagulation disorders of patients with traumatic brain injury after plasma adjuvant therapy

Objective To analyze the efficacy of plasma adjuvant therapy on coagulation disorders in patients with traumatic brain injury and its relation with the prognosis.Methods Prospective study was performed on patients with traumatic brain injury and coagulation disorders,admitted to our hospitals from January 2010 to June 2012; these patients were divided into conventional treatment group and plasma adjuvant therapy group.Cranial CT was performed and blood coagulation function was checked at admission,and then,coagulation function was re-checked again 3 days after therapy and cranial CT within 3 days.The Glasgow Outcome Scale (GOS) was marked 21 days after treatment.The improvement of coagulation disorders,secondary bleeding and prognosis were compared between the two groups.Results As compared with those in the conventional treatment group,the prothrombin time,partial thromboplastin time,secondary hemorrhage rate in the plasma adjuvant therapy group were significantly reduced (P<0.05).The differences of D-dimer content and GOS scores between the two groups were not statistically significant (P>0.05).Conclusion Plasma adjuvant therapy effectively improves the coagulation function,reduces the incidence of intracranial secondary bleeding,but can not obviously improve the prognosis of patients with traumatic brain injury.
8.Immune response of dendritic cells capturing antigens from apoptotic U937 cells induced by artesunate.

The objective of study was to investigate whether U937 cells-loaded dendritic cells (DCs) could induce anti-leukemic immune activity. The apoptosis of U937 cells was induced by artesunate (ART). DCs derived from peripheral blood mononuclear cells of health donors were loaded with apoptotic U937 cells, and induced to maturation in the presence of TNF-alpha. Matured DCs were cocultured with autologous T-lymphocytes, and combined with IL-2 in order to induce the leukemia-specific CTL. The phenotypes of DCs and T lymphocytes were tested by flow cytometry. The ability of DC capturing antigens was measured by Dextran-FITC endocytosis. The IL-12p70 level was assayed by ELISA kit. The proliferation of CTL and CTL activity were measured by MTT assay. The results showed that the apoptotic rate of the U937 cells was 51.2% when U937 cells were induced by 1 microg/ml ART for 48 hours in vitro. DCs had the most powerful ability of endocytosis in its immature phase. Apoptotic U937 cells could not induce the features of DC maturation, and apoptotic U937 cell-pulsed immature DCs could be matured with TNF-alpha. The IL-12p70 level secreded by apoptotic U937 cell-loaded mature DCs (mDC-(Apo)U937) was higher than that of non-loaded mDC. The proliferation of autologous T lymphocytes co-cultured with mDC-(Apo)U937 was significantly remarkable and the content of CD8(+) CTL was significantly higher in comparison with any other groups. CTL induced by mDC-(Apo)U937 had stronger killing effect on U937 cells than NB4 (p < 0.01). It is concluded that the mDC-(Apo)U937 can effectively generate T cell-mediated dendritic antileukemic responses in vitro.

9.Experimental study of preventing postoperative stenosis by modifying artificial esophagus in dogs.

OBJECTIVETo observe the effect of the modified artificial esophagus on postoperative stenosis in dogs.
METHODSThe models of defected esophagus were established in dogs. The double-layered membrane tube (modifying type) was implanted in the test group (n = 10) and the esophageal stent was further inserted when the stenosis occurred. The single pattern tube (original type) was transplanted to the control group (n = 30). The dilation treatment was performed to relieve the postoperative stenosis; alternatively, the esophageal stent was implanted in the unsuccessful dogs.
RESULTSThe average artificial esophagus removal time was 19.10 days in the test group, which was significantly lower than 39.07 days in the control group (t = 15.6, P = 0.000). No obstruction after removal was observed in the experimental group. The incidence of postoperative stenosis had no significant difference between these two groups.
CONCLUSIONThe double-layered membrane tube can make the tube removal safer by shortening the removal time.

10. Effect of Active Components of Traditional Chinese Medicine in Resisting Gallbladder Carcinoma

Allbladder cancer is highly malignant and has few effective therapeutic drugs. Traditional Chinese medicine (TCM) has attracted the attention from researchers because of its multi-target, multi-link, multi-channel and less toxic side effect. In recent years, the basic studies of gallbladder cancer have made certain achievements in active components of TCM. Relevant experimental studies have extended to the lever of cells, molecules and genes. The main mechanism of experimental research include inhibiting tumor cell proliferation, inducing tumor cell apoptosis, inhibiting cell metastasis, inhibiting cell migration, influencing signal transduction pathway, enhancing sensitivity of chemotherapeutic drugs, and inhibiting tumor angiogenesis. The existing experimental studies have showed that active components of TCM have certain curative effect on gallbladder cancer, but with many problems. For example, although the extract of active components of TCM shows an anti-tumor activity, the specific composition and chemical molecular structure are still unclear. Most studies focus on the level of in vitro cell experiments, but only a few in vivo experimental studies have been carried out in animals. Therefore, more in-depth studies need to be carried out in the future. Currently, most of the mechanisms of anti-gallbladder cancer of active components of TCM are classical signaling pathways. The next step is to find new signaling pathways and new targets. In addition, under the guidance of TCM theory, the future studies of TCM compound can bring advantages of TCM against tumor into full play. The experimental study on the mechanism of action of active components of TCM in the treatment of gallbladder cancer is summarized as follows, which is helpful for researchers to understand the current experimental studies on active components of TCM in gallbladder cancer, in order to conduct more in-depth studies. Basic experimental studies on gallbladder cancer can provide theoretical basis for clinical treatment.
