Thyroid cancer is the most common malignancy in the endocrine system .In recent years ,the incidence of thyroid cancer has increased year by year ,which sparked people′s attention.With the development of thyroid cancer research ,it is found that cytokines play a key role in the occurrence and development of tumor .At present ,the cause of thyroid carcinoma and mechanism is not yet entirely clear ,however ,the occurrence of thyroid cancer is possiblely associated with the body regulating cellular immune dysfunction .Cytokines play an important role in the whole process ,in which the relationship between cytokines IFN -γ,IL-6 and thyroid carcinoma is closely related .
4.A study of the treatment of delayed emergency percutaneous coronary intervention on the prognosis of patients with ST elevation acute myocardial infarction

Objective To compare the prognosis of ST elevation acute myocardial infarction(STEMI) of delayed emergency per‐cutaneous coronary intervention(PCI) and elective PCI following the thrombolysis with reteplase ,and to investigate the clinic value of the former solution .Methods One hundred and twenty STEMI patients were reviewed retrospectively and were divided into 2 groups according to PCI .Eighty two cases were divided into emergency group :thrombolysis with reteplase offered within 6h was followed by elective PCI;38 cases were divided into delayed group:PCI was done after 12-24 h after ATEMI′occurring .The clini‐cal features and CTG changes were recorded ,TIMI class of IRA was conducted before and after PCI ,and their prognosis were com‐pared .Results There was no statistical differences in the class of thrombolysis of myocardial infarction(Class 0 -1 ,2 -3) of in‐farction relative artery (IRA)(4 .88% ,95 .12% vs .5 .26% ,94 .74% ) after PCI(P>0 .05) .There was no statistical difference in LVEF ,LVEDVI and LVESVI 3 months after attack between two groups(P>0 .05) .There was statistical difference in the severe cardiac failure and malignancy arrhythmia between two groups(P<0 .05) ,while no statistical difference exist in angina after infarc‐tion as well as cardiac mortality after attack between two groups(P>0 .05) .Conclusion Delayed emergency PCI to remove the ob‐struction in the coronary artery has no significant difference with elective PCI following thrombolysis in the incidence of composite end point events in STEMI .
5.Pathogenic Fungi of Blood Culture:Their Distribution and Resistance

OBJECTIVE To study the constituent ratio of the pathogenic fungi of blood culture in recent 24 months and their resistance in our hospital.METHODS Blood culture of patients in our hospital was performed by BacT/AlerT120 and the isolated pathogenic fungi were identified by API identified tests(API Inc,France).In(addition) antibiotics sensitivity test was by K-B.RESULTS Of the specimens in 4135 cases,there were 110 strains((2.7%)) with Candida albicans(29%).C.tropicalis(21%) and C.portugal(9%).The(specimens) come from(hepatobilliary)(25%),neurosurgery(24%) and emergency(10%) departments.CONCLUSIONS It is important and necessary to monitor the circumstance of fungal(infection) and resistance of the pathogenic fungi due to its(morbidity) increased.
6.Trans-supraorbital keyhole appraoch microsurgery with an eyebrow incision for sellar tumors: Report of 24 cases

Objective To study the feasibility of trans-supraorbital keyhole approach microsurgery for sellar tumors.Methods The operation was conducted under combined intravenous general anesthesia and the patient was maintained at a dorsal position.A skin incision was made along the lateral eyebrow at 3.5~4.0 cm in length.The frontal muscle periosteous flap was draged upwards and the orbicularis muscle periosteous flap was dragged downwards.Then the tempotal muscle was dragged laterally.The position of the keyhole was located behind the temporal line.By using a milling cutter,a bone window 2 cm ? 3 cm in size was made medially.The inner margin of the supraorbital skull was shaped to expand the visual field of microsurgery.The cerebral dura mate was opened curvily,with the base directing to the orbital margin.The frontal lobe was elevated gently and the cerebrospinal fluid was drained.The tumor was fully exposed and resected.Results Total resection of tumor was achieved in 22 cases,and part of posterior tumor membrane remained in 2 cases of craniopharyngioma.Hypoadrenocorticism occurred in 1 case of ACTH adenoma after operation and a hormone replacement therapy was required. Temporary postoperative diabetes insipidus occurred in 8 cases and was relieved after 1 week.The postoperative urinary volume decreased significantly in 9 cases of preoperative diabetes insipidus.Visual disorder on the operational side was aggravated after operation in 1 case of tuberculum sellae meningioma,and was gradually improved after 1 week.No recurrence was observed under MRI examination in 18 cases at 3 months after operation.Conclusions Trans-supraorbital keyhole approach microsurgery is feasible,and it gives minimal operational trauma,thorough resection of tumor,and good cosmetic outcomes.
7.Burdens of Spouse Caregivers of Patients with Dementia at Home

Objective: To explore the burden of spouse caregivers of patients with dementia at home and factors having influence.Methods:Forty-two patients with dementia were collected from psychiatric out-clinic consecutively. Trained investigators did home visit to collect data on severity of the disease (CDR, MMSE), pathological behavior (BEHAVE-AD), activity of daily living (ADCS-ADL) and burdens of their spouse as caregiver (CBI). The caregivers' anxiety, depression (Zung's Anxiety Scale and Depression Scale) and knowledge about dementia were also collected.Results:The caregivers felt big burden (BDI total score: 30.7?15.1). Nearly half of them (42.9%) were in depression and one quarter (23.8%) were anxious. They were lack of knowledge about dementia and unskillful in care-giving. 26.2% caregivers thought dementia is curable or equal to normal aging, 57.1% of them did not believe it is an improving disease. Objective To construct an eukaryotic expression vector of human sTNFR1 and to investigate its inhibitory effects to the bioactivity of TNF-?.Methods The total RNA was extracted from HeLa cells and used as a template to amplify human sTNFR1 gene by reverse transcription polymerase chain reaction(RT-PCR).The PCR products were cloned into T vector and sub-cloned into vector pcDNA3.1(-),an eukaryotic expression vector.The recombinant plasmid pcDNA3.1(-)-sTNFR1 was transfected into QSG7701 cells by using lipofectamine,RT-PCR was performed to detect the expression of sTNFR1,MTT was used to observe sTNFR1 gene 's inhibitory effect on TNF-?.Results QSG7701 has a higer expression level of sTNFR1 mRNA than pcDNA3.1(-) trancfected control.The cytotoxic effect of TNF-? was inhibited to the extent of 64.8% when its concentration was 100 ?g/L.Conclusion We constructed the eukaryotic expression vector containing human sTNFR1 gene and the cytotoxicity of TNF-? is inhibited in pcDNA3.1(-)-sTNFR1/QSG7701 cells.
9.Cardiac Screening in the First Trimester Using Three-dimensional Ultrasound with Spatiotemporal Image Correlation

[Objective] To evaluate the value of cardiac screening in the first trimester by using three-dimensional ultrasound with spatiotemporal image correlation (STIC) and to analyze the value of different views for cardiac assessment.[Methods] This study retrospectively reviewed the eighty-seven fetuses with congenital heart diseases which were diagnosed in our institution between January 4th 2013 and June 30th 2016.We compared the detection rate of prenatal ultrasound screening with STIC in the first trimester and the results of postnatal echocardiography or autopsy.In addition,the detection rates of the different planes,including four-chamber view (4CV),outlet of the left ventricle (OLV),outlet of the right ventricle (ORV) and three vessels-trachea view (3VT) were analyzed.[Results] Severe cardiac anomalies were identified in 62 fetuses (71.3%) and non-severe cardiac anomalies were identified in 25 fetuses (28.7%).Seventy-eight cases (89.7%,78/87) with congenital heart diseases could be detected by using three-dimensional ultrasound with STIC.The detection ratesfor 4CV,4CV+OLV,4CV+ORV,4CV+3VT,4CV+OLV+ORV and 4CV+OLV+ORV+3VT were 48.3%,64.4%,66.7%,79.3%,69.0% and 89.7% respectively.[Conclusions] Three-dimensional ultrasound with STIC could detect congenital heart disease in the first trinester.The four-chamber view plus outlet of the left ventricle,outlet of the right ventricle and three vessels-trachea view could increase the detection rates of congenital heart diseases.
10.The predictive value of serum cystatin C in in-stent restenosis

Objective To investigate the relationship between serum cystatin C (Cys C) level and the occurrence of in-stent restenosis after coronary drug-eluting stent implantation.Methods A retrospective analysis of coronary drug-eluting stent implantation was curried out in 592 patients.All patients were from Shengjing Hospital of China Medical University by coronary angiography,and were divided into restenosis group (142 cases) and non restenosis group (450 cases).The Cys C levels before and after surgery and follow-up angiography were analyzed.The biochemical indicators,and transcatheter arterial restenosis were analyzed.Results Either before or after operation and follow-up angiography,serum cystatin C levels were higher in restenosis group patients than that in non restenosis group [before operation:(1.622 ± 1.063) mg/L vs (1.369 ± 0.860) mg/L;after the operation:(1.769 ± 1.062) mg/L vs (1.458 ± 0.883) mg/L;review:(1.924 ± 1.085) mg/L vs (1.440 ± 0.874) mg/L;P < 0.05].Multivariate COX regression analysis showed that serum cystatin C level was an independent risk factor of in-stent restenosis in drugeluting area (P < 0.05).Conclusions The serum cystatin C level is closely related to in-stent restenosis in drug-eluting stents area.High level of serum cystatin C is an independent risk factor of in-stent restenosis after drug-eluting stents implantation.
