1.Clinical features of benign paroxysmal positional vertigo

Objective To analyze the clinical features and causes of benign paroxysmal positional vertigo(BPPV).Methods The clinical data of 521 patients with BPPV were collected.The percentage of patients with BPPV in all the outpatients with dizziness and vertigo,the clinical characteristics of BPPV,and the prevalence of comorbidity with migraine were analyzed.Results BPPV accounted for 35.7% of patients with vertigo and 12.1% of patients with dizziness.Among the 521 patients with BPPV,158(30.3%)were male and 363(69.7%)were female,with an average age of(57.5?12.8)years old(20 to 93 years old).Right semicircular canals were involved in 323 cases(62.0%),and left semicircular canals in 187 cases(35.9%).The latency period of nystagmus of vertical canals was(3.22?2.37)s,and time of persistence was(8.31?7.98)s.The latency period of nystagmus of horizontal canals was(2.33?1.50)s,and the time of persistence was(14.77?11.40)s.There were significant differences in the latency period of nystagmus and time of persistence between the vertical and horizontal canals(P=0.001 and P=0.000).Fifteen patients(2.9%)had a history of head trauma and 39(7.5%)were complicated with migraine.Conclusion BPPV is prevalent in patients with dizziness and vertigo,and misdiagnosis and missed diagnosis should be avoided.

3.Ribozyme riboswitch based gene expression regulation systems for gene therapy applications: progress and challenges.

Robust and efficient control of therapeutic gene expression is needed for timing and dosing of gene therapy drugs in clinical applications. Ribozyme riboswitch provides a promising building block for ligand-controlled gene-regulatory system, based on its property that exhibits tunable gene regulation, design modularity, and target specificity. Ribozyme riboswitch can be used in various gene delivery vectors. In recent years, there have been breakthroughs in extending ribozyme riboswitch's application from gene-expression control to cellular function and fate control. High throughput screening platforms were established, that allow not only rapid optimization of ribozyme riboswitch in a microbial host, but also straightforward transfer of selected devices exhibiting desired activities to mammalian cell lines in a predictable manner. Mathematical models were employed successfully to explore the performance of ribozyme riboswitch quantitively and its rational design predictably. However, to progress toward gene therapy relevant applications, both precision rational design of regulatory circuits and the biocompatibility of regulatory ligand are still of crucial importance.

4.Safety evaluation of niuhuang jiedu tablet.

Realgar-containing Niuhuang Jiedu tablet (NHJD) has been applied in clinic for more than 800 years. However, because realgar contains arsenic (As), it has aroused wide concerns and controversies both at home and abroad. Currently, there are two misunderstandings about realgar-containing Chinese patent medicines. First, some people exaggerated realgar's toxicity as that of arsenic. Second, they recommended to remove realgar from traditional Chinese medicine compounds. In this paper, the authors summarized the advance in studies on NHJD, and proposed different opinions: (1) It is inappropriate to take total As as the index in safety evaluation of NHJD. (2) The toxicity of NHJD is dependent on the dose and duration of administration. (3) Realgar is an active ingredient of NHJD, and shall be deeply studied. Classic realgar-containing traditional Chinese medicine prescriptions, such as Niuhuang Jiedu tablet, shall be evaluated with rigorous modern scientific basis, with the aim to guide rational and safe application.

5.Observation on therapeutic effect of herbs-partitioned moxibustion on Parkinson disease of 54 cases.

OBJECTIVETo observe clinical therapeutic effect of herbs-partitioned moxibustion at Shenque (CV 8) on Parkinson disease.
METHODSNinety cases of Parkinson disease were randomly divided into a treatment group (n=4) and a control group (n=36) in order of visiting. The two groups were treated with routine western medicine for treatment of Parkinson disease, and herbs-partitioned moxibustion at Shenque (CV 8) was added to the treatment group. Their comprehensive therapeutic effects were observed and modified UPDRS cumulative scores were compared.
RESULTSThe total effective rate was 83.3% in the treatment group, significantly better than 58.3% in the control group (P < 0.01); the modified UPDRS cumulative score was reduced by 74.1% in the treatment group, significantly better than 16.7% in the control group (P < 0.01).
CONCLUSIONHerbs-partitioned moxibustion at Shenque (CV 8) has a certain therapeutic effect on Parkinson disease.

Some champers besides Miraculous Pivot: Channels and Vessels in Huangdi's Internal Classic are involved in diseases of channels and vessels, and most of them which mainly discuss diseases of channels and vessels are explained in the type of six channels of feet. Studies have showed that the diseases of six channels of the feet are closely related with diseases of channels and vessels in the medical books of bamboo slips and silk cloth, indicating that on the one hand, the studies on diseases of channels and vessels are not only limited in Miraculous Pivot: Channels and Vessels, and on the other hand, the influence of the literature of channels and vessels in the medical books of bamboo slips and silk cloth is quite profound and lasting. Widening the field of vision of investigating concept of diseases of channels and vessels, and looking closely at the course of history from academic development are favorable to deeply understanding diseases of channels and vessels, even early theories of channels and vessels.

8.Regression analysis on influence factors of orthokeratology controlling juvenile myopia progression

?AlM:To explore the influence factors of orthokeratology controlling development of juvenile myopia.
?Methods: A retrospective analysis of 86 cases ( 169 eyes ) who performed orthokeratology from November 2012 to November 2013 in our hospital was carried out. Ater 2a treatment, diopter increased > -0. 25D as invalid control group (54 eyes), diopter increased ≤ -0. 25D as effective control group ( 115 eyes ) . The difference of various factors between two groups were compared.
?RESULTS: Univariate comparison showed that the two groups had no statistical significance (P>0. 05) in terms of gender, whether wear glasses in the past, anterior corneal diameter before treatment, central corneal thickness, intraocular pressure and anterior chamber depth. However, the two groups had statistical significance (P<0. 05) in the terms of age, dioptre before trement, corneal curvature, and axial length basis. Logistic regression analysis showed that foundation axial length, age and corneal curvature were in line with regression model ( P<0. 05 ), these were independent factors that orthokeratology lens control juvenile myopia development.
? CONCLUSlON: lndependent factors affecting orthokeratology control juvenile myopia development is foundation axial length, age and corneal curvature. For juvenile patients, the longer of foundation axial length, the longer the age, the greater the curvature of cornea, orthokeratology can better control the myopia’s development.
9.Clinical characteristics and long-term outcome of PCI in patients with chronic renal insufficiency

Objective To determine clinical characteristics and the effects of drug-eluting stents on the occurrence of major adverse cardiac events during percuteneous coronary artery interventional(PCI)and long- term outcomes in patients with chronic renal insufficiency(CRI).Methods Nine hundreds and seventy three patients with angiographically-documented coronary artery disease(lumen inner diameter narrowing>50%), included 516 patients complicated with experienced renal impairment(CRI group)and 457 with normal renal function(control group).Baseline clinical data and coronary angiographic features were recorded.Results Comparing with control group,patients in CRI group were older with higher incidence of hypertension or diabetes and simultaneously complicated by reduced left ventricular ejection fraction,and more complex coronary lesions(type C).During follow-up(mean 17 months),the mortality was significantly higher in CRI than in control group(6.2% vs 3.3%,P<0.05),but the former with CRI was significantly lower by using drug-eluting stents in comparing with bare-metal stents(4.1% vs 8.5%,P<0.05).Conclusion Patients with CRI often complicated with severe coronary artery disease,the mortality after PCI would be significantly reduced by using drug-eluting stents.
10.Identification and Simulation Mutation of a High-productive Strain of Poly(?-glutamic acid) Independent of Glutamic Acid

A high productive poly ?-glutamic acid(?-PGA) strain PGA-N in a culture medium containing no L-glutamine was isolated from fermentation products.With the following identifications of colony mor-phology,physiological and biochemistry experiments,and genetics,the strain PGA-N was classified as a Bacillus licheniformis.According to the product environment,the base culture medium having no L-glutamine was simulated.In order to enhance the production of the strain PGA-N,the fermentation condi-tions,such as carbon source,nitrogen source,were optimized and the ?-glutamic acid production reached 5.16 g/L after getting the optimum formulation of this culture medium.PGA-N was mutagenized with com-bination of NTG and UV.A mutant PGA-N-C10 was screened which PGA production was increased from 5.16 g/L to 8.82 g/L.The study also investigated the effects of agitation speed on the cell biomass,?-PGA production and the ?-PGA molecular weight.The ?-PGA yield of PGA-N-C10 was as high as 11.00 g/L when the agitation speed was 400 r/min.
