2.Treatment and Clinical Results of Coats` Disease.

To evaluate the clinical courses and the results of treatment of Coats`disease, we retrospectively investigated the medical records of 30 patients (27males,3 females). The age at diagnosis, bilaterality, chief complaints, associated abnormalities, retinal distributions of vascular change and exudate, and methods and results of treatment were reviewed. The average age at diagnosis was 4.6 years (range:1.0~8.2 years) and all cases were unilateral. Exotropia was the most common presenting signs. Following treatment, 60.0%of eyes were clinically worsened in subretinal fluid drainage/vitrectomy group, 6.7% in laser/cryotherapy group,and 28.6%in no treatment group. These results suggest that laser/cryotherapy may treat effectively the early change in Coats`disease, and making a diagnosis and treatment before the development of bullous retinal detachment may be advisable.

3.Analysis of fluid in simple renal cyst.

We assessed cyst fluid of 14 cases of simple renal cyst. The aspirates were analyzed for color, lipid. protein, glucose, lactic acid dehydrogenase(LDH), amylase, electrolytes and Papanicolaou smears and cell blocks were performed. A clear amber fluid with minimal lipid content, LDH lower than that of blood and a normal cell pattern on Papanicolaou stain and cell block which are characteristics of transudate were seen in all cases. Although the number of cases are insufficient, it would appear that the content of simple renal cyst is identical to that of transudate.

4.Variation in Biochemical Composition among Indian Isolates of Sclerotinia sclerotiorum.

Biochemical variability among 20 Indian isolates of Sclerotinia sclerotiorum collected from different hosts/soil samples from different localities in India is reported. High Performance Liquid Chromatographic (HPLC) analysis of ethyl acetate fraction of culture filtrate, mycelia, sclerotia and sclerotial exudate showed 15~23 peaks but only 11 could be identified. They were tannic, gallic, oxalic, caffeic, vanillic, ferulic, O-coumeric, chlorogenic, cinnamic, salicylic and gentisic acids. The amount of phenolic compounds varied among the culture filtrates, mycelia, sclerotia and sclerotial exudates of S. sclerotiorum.

5.Bronchogenic cyst masquerading as malignant pericardial effusion with tamponade.

We report on a rare case involving a 23-year-old female patient with mediastinal cystic mass complicated with acute pericarditis and cardiac tamponade. Pericardial fluid demonstrated lymphocyte-predominant exudate and the level of carcinoembryonic antigen (CEA) was unexpectedly elevated. Successive aspiration of mediastinal cystic mass revealed a very high level of CEA (>100,000 U/mL) and carbohydrate antigen 19-9 (>15,000 ng/mL). This patient was clinically diagnosed as an infected bronchogenic cyst complicated with pericarditis and cardiac tamponade. The treatment resulted in alleviation of her symptoms.

7.Enhanced CT in the Patients with Pleural Effusion: Differential Findings between Exudates and Transudates.

PURPOSE: To evaluate the differential findings of CT in the differention of pleural exudates and transudates. MATERIALS AND METHODS: One hundred and thirteen consecutive patients (113 effusions) underwent enhanced thoracic CT ; the scans were evaluated for the presence or absence and appearance of enhancing parietal pleural thickening and extrapleural fat thickening. Thoracentesis was performed to measure pleural and serum total protein andlactate dehydrogenase(LDH) values. Effusions were classified as exudates by using Light's criteria. RESULTS: Eighty-eight effusions were exudates and 25 were transudates. Eighty-three of the 88 exudates (93%) were associated with enhanced parietal pleural thickening ; seventy of the 88 (80%) were associated with extrapleural fat thickening. Four of the 25 transudates were associated with parietal pleural thickening and extrapleural fat thickening, both of which were the most important factors in differentiating beteen pleural exudates and transudates(p<0.05). CONCLUSION: Parietal pleural thickening and extrapleural fat thickening on contrast-enhanced CT almost always in dicate the presence of pleural exudates.

8.Microbiological Characteristics according to Transudative and Exudative Effusion in Pleural Fluid Culture.

A total of 1,132 pleural fluid culture results obtained from October 2012 to July 2014 were analyzed to elucidate the microbiological characteristics according to transudative and exudative pleural fluid. The pleural fluid cultures were performed using aerobic and anaerobic blood culture bottles. The blood and pleural fluid for total protein, lactate dehydrogenase, and glucose measurement were submitted to laboratory at the same time with pleural fluid cultures. The rates for culture positivity, anaerobes isolation, and polymicrobials between transudative and exudative pleural fluid were 5.2% vs. 10.4%, 14.8% vs. 7.8%, and 14.8% vs. 10.9%.

9.The Clinical Analysis and Treatment Results of Coats' Disease in Children.

PURPOSE: To evaluate the clinical characteristics and treatment result of Coats' disease in children. METHODS: Data on demographics, clinical presentation, and ocular findings were analyzed for 67 eyes of 67 patients with Coats' disease by retrospective chart review. Among these, treatment results were analyzed from 56 eyes which had received local treatment or subretinal fluid drainage (SRFD). Treatment was considered successful when telangiectatic vessels or exudates had regressed after local treatment or when pupillary block glaucoma was treated or prevented after SRFD. RESULTS: Mean age at diagnosis was 5.0 years (5 months-15 years) and 90% were males. 67% of retinal telangiectasias were located in the temporal. There were exudative retinal detachments in 46 (69%) eyes. Among them, SRFD was performed in 19 (28%) to treat or prevent pupillary block glaucoma. The mean age at diagnosis of patients that underwent treatment was 5.1 years. In 37 of 56 patients, 95% of retinal telangiectasias were regressed after primary local treatment, and 21 eyes (31%) underwent SRFD. Management of glaucoma was achieved after SRFD in 16 patients, and one more SRFD was needed after primary SRFD in 5 patients. CONCLUSIONS: In Coats' disease, it is necessary to eradicate retinal telangiectasia with local treatment and to follow up carefully for secondary glaucoma, which can be treated with SRFD in cases of severe exudative retinal detachment.

10.Intravitreal Bevacizumab Injection for Serous Retinal Detachment Associated with Leber's Idiopathic Stellate Neuroretinitis.

PURPOSE: To report the effectiveness of intravitreal bevacizumab treatment for serous retinal detachment associated with Leber's idiopathic stellate neuroretinitis. CASE SUMMARY: A 56-year-old male visited our clinic complaining of visual disturbance for three days in his right eye. His best corrected visual acuity was 0.5. Relative afferent pupillary defect and pain when moving eyes were noted in his right eye as well as inflammatory cells in the vitreous cavity. On funduscopic examination, disc swelling with hemorrhage and stellate-shaped hard exudates were noted at the perifovea. Ishihara color vision test showed anomalous trichromacy in his right eye. Hyperfluorescence around the disc was observed on fundus fluorescein angiography. Optical coherence tomography showed disc swelling with serous retinal detachment at the fovea. Inferior altitudinal scotoma was noted on visual field examination. The patient underwent intravitreal bevacizumab injection and topical steroid medication. After six days, the patient's symptoms and disc swelling improved, and decreased subretinal fluid was observed. After six weeks, his best corrected visual acuity was 1.0. Nine weeks later, visual field examination showed nonspecific scotoma. CONCLUSIONS: Intravitreal bevacizumab injection is useful for treating serous retinal detachment associated with Leber's idiopathic stellate neuroretinitis.
