OBJECTIVETo study the changes of pharmacokinetics of 6,7-dimethoxycoumarin in a rat model of alpha-naphthylisothiocyanate (ANIT)-induced experimental hepatic injury after oral administration of Yinchenhao Decoction (, YCHD) using an ultra pressure liquid chromatography (UPLC) method.

METHODSRats were divided into a normal group and a model group, after modeled by 4% ANIT (75 mg/kg) for 48 h, they were orally administrated with YCHD extract at the dose of 0.324 g/kg, and then blood was collected from their orbital sinus after different intervals. Changes in liver function were monitored by the levels of liver enzymes [alanine aminotransferase (ALT), aspartate aminotransferase (AST)] and bilirubins [total bilirubin (TBIL), direct bilirubin (DBIL)], the concentration of 6,7-dimethoxycoumarin in plasma were measured by UPLC, and the pharmaceutical parameters were calculated with DAS2.1.1 software.

RESULTSThe concentration-time curve of both normal and modeled rats after oral administration of YCHD was obtained. Their time to maximum plasma concentration (t(max)) were both 0.25 h, the maximum concentration (C(max)) were 4.533 μg/mL and 6.885 μg/mL, and their area under concentration-time curve (AUC)(0→24h) were 16.272 and 32.981, respectively. There was a 51.88% and 100.46% increase in C(max) and AUC(0-t) (P<0.05), but there showed a 45.52% and 92.93% reduction in clearance of drug and volum of distribution (P<0.05), respectively.

CONCLUSIONSHepatic injury could significantly influence the pharmacokinetics of 6,7-dimethoxycoumarin after oral administration of YCHD, the absorption and distribution process was accelerated in liver injured rats, but the metabolism and elimination process was slowed. And this may lead to a significant accumulation of 6,7-dimethoxycoumarin in the body.

Assessing response to therapy allows for prospective end point evaluation in clinical trials and serves as a guide to clinicians for making decisions. Recent prospective and randomized trials suggest the development of imaging techniques and introduction of new anti-cancer drugs. However, the revision of methods, or proposal of new methods to evaluate chemotherapeutic response, is not enough. This paper discusses the characteristics of the Response Evaluation Criteria In Solid Tumor (RECIST) version 1.1 suggested in 2009 and used widely by experts. It also contains information about possible dilemmas arising from the application of response assessment by the latest version of the response evaluation method, or recently introduced chemotherapeutic agents. Further data reveals the problems and limitations caused by applying the existing RECIST criteria to anti-cancer immune therapy, and the application of a new technique, immune related response criteria, for the response assessment of immune therapy. Lastly, the paper includes a newly developing response evaluation method and suggests its developmental direction.

PURPOSE: This study was done to apply failure mode & effect analysis (FMEA) to chemotherapy in order to reduce prescribing, dispensing and administering errors related to treatment and provide patients with a safe medical environment. METHODS: A one group pre-post test design was used to verify the effects using the tool for FMEA in chemotherapy. RESULTS: There was a statistically significant decrease in prescribing errors from 11.47% to 3.18%; administering errors decreased but they were not statistically significant. In a addition, there was no change in dispensing errors. CONCLUSION: The results show that FMEA removed risk factors that might occur during the process of chemotherapy and that it was an effective tool for prevention of negligent accident occurring in actual patients.

A novel double-chamber series piezoelectric pump has been presented and tested. The pump is a multi-layer circular planar structure, consisting of PMMA (polymethyl methacrylate) pump body, two PZT actuator membranes and three cantilever valves. The PZT actuators are driven at a phase difference of 180 degrees, which is equal to two one-chamber pumps running in series. The output performance depends on the geometrical parameters of the actuator membrane. The prototype pump, fabricated with the PZT membrane 0.18 mm in thickness and 50 mm in diameter of 50 mm, can deliver drug in either direct way (pumping liquid drug) or indirect way (pumping air to extrude liquid drug from a sealed container). The frequency-response characteristic of the two handling methods is of difference. The pump obtains optimum performance at low frequency for liquid as medium, and at high frequency for air as medium. For both the direct delivery and indirect delivery, the maximum flowrate achieved reached up to 220 ml/min and 35 ml/min, respectively; and the maximum backpressure obtained amounted to about 14 KPa and 21 KPa, respectively, at the applied voltage of 80 V with frequency of 20 Hz.

The necessity of establishing an evaluation system about the combination program of Chinese medical pharmacy and Western medical pharmacy was addressed in this paper. Besides, its contents were systematically clarified. Besides, existent problems and its future development trend were also explained. The author believed that it was necessary to perform researches on constructing the evaluation system on the basis of patients' needs and physicians' responsibilities. The ultimate goal of this system was to produce an optimal combination program of Chinese medical pharmacy and Western medical pharmacy for a specific disease. This optimal program was the results of comparing and analyzing the therapeutic efficacies of different combination programs. In this program, Chinese medical pharmacy and Western medical pharmacy combined together. On the one hand, it is safe; on the other hand, they do not produce adverse reaction. Their therapeutic effects were synergetic. Chinese medical pharmacy could not only advance the cure effects of Western medical pharmacy, but also supplement the insufficiency of Western medical pharmacy. Of course, the author put forward some assumptions only from the perspective of clinical application in this paper. The evaluation system will become perfect along with further deepening researches of basic sciences.

PURPOSE: The purpose of this study was to develop an enneagram program and to investigate the effects of the enneagram program on self-esteem, interpersonal relationship and GAF(Global assessment of Functioning Scale) in psychiatric patients. METHODS: A nonequivalent control group pre-post test design was used. Participants were 46 psychiatric patients. Participants were assigned to the experimental group(n=21) or control group(n=25). The experimental group participated in the enneagram program for 12 hours through eight sessions. The control group(n=25) received a routine nursing activity program. RESULTS: The experimental group that participated in the enneagram program showed higher scores for self-esteem(t=3.73, p=.001), interpersonal relationship(t=2.63, p=.012) and GAF(t=2.43, p=.019) compared to the control group. CONCLUSION: The results indicate that an enneagram program for psychiatric patients is effective in improving self-esteem, interpersonal relationship and overall functioning skills. Therefore, it is suggested that an active use of this enneagram program as a nursing intervention for psychiatric patients would be beneficial.
