2.An Immunohistochemical Study of CA 125, CA 19-9, and CA 15-3 in Ovarian Epithelial Tumors.

To evaluate the relationship between the expression of CA 125, CA 19-9, and CA 15-3 and ovarian epithelial tumors, immunohistochemical stainings were performed and analyzed to the types of tumor, and the degree o malignancy and differentiation in the total 42 cases. The results were summarized as follows: 1) The expression of CA 19-9 and CA 15-3 was demonstrated in most cases of benign, borderline and malignant mucinous tumors. All types tumors were negative for CA 125. 2) All types of serous tumors including benign, borderline and malignant revealed strong positive reaction for CA 125 and CA 15-3 in contrast to some weak positivity for CA 19-9. Expression of CA 125 was significantly increased in high grade serous adenocarcinomas compared with low of grade. 3) Endometrioid carcinomas revealed strong positive reaction for CA 15-3 in contrast to some positivity for CA 125 and CA 19-9. One case of Brenner tumor expressed CA 19-9 and CA 15-3 without expression of CA 125. 4) Serum CA 125 levels were elevated in 80% and CA 15-3 levels in 100% of ovarian epithelial malignancy. The correlation between serum levels and staining intensities was moderate.

3.Changes of renal function in the remaining kidney after donor nephrectomy.

To observe the changes of renal function in the remaining kidney after donor nephrectomy, we prospectively examined renal Function of 213 living donors (Donor group) and they were compared with that or other (not related to genitourinary tact) 20 operated patients (Control group). We measured the blood pressure, serum BUN. creatinine, 24-hour urine protein and creatinine clearance before and after operation. The significant differences were observed in serum BUN. creatinine, 24-hour urine protein and creatinine clearance between donor group and control group. In conclusion. there were significant changes of renal function in the remaining kidney after donor nephrectomy and we must perform follow-up study of renal function periodically.

4.Posterior Chamber Intraocular Lens Implantation in High Myopia.

We evaluated pseudophakia in high myopic patients whose an axial lenght were 26mm and over. Cataract surgery was performed with ECCE and posterior chamber intraocular lens implantations from May 1986 to May 1991 on 69 patients (80 eyes). The results were as follows; 1. Most myopic patients were good candidates for posterior chamber intraocular lens implantation regardless of axial length. 2. Posterior chamber intraocular lens implantations in high myopia were a good refractive surgery. 3. The theoretical formula had better predictabilized than the empirical formula in apredictive accuracy.

5.Comparison of the Marshall-Marchetti and endoscopic bladder neck suspension for stress incontinence.

The MsrshaII-Marchetti and endoscopic bladder neck suspension procedure are successful procedures for the treatment of genuine stress incontinence. We experienced 32 cases of stress incontinence surgically treated by the Marshall-Marchetti procedure (7 cases) during the period from January 1973 to 1981 and endoscopic bladder neck suspension procedure (25 cases) from l982 to June l990. Following results were obtained. I. Patients' ages ranged from 15 to 63 years (average 46.6 years), and most patients were rnultiparous with an average of 3.5 deliveries. 2. On the chain cystourethrogram. 25 cases (78.1%) belonged to type I , and seven case.s (2l.9%) to type I[ according to Green`s classification. 3. The values of postoperative days on catheter, operative time and postoperative hospitalization in the Marshall-Marchetti procedure were 6.7+/-3.3 days, 112.9+/-20.6 minutes and 8.7+/-0.8 days, and in the endoscopic bladder neck suspension procedure 3.3+/-2.0, 87.9+/-22.9 and 45+/-2.0 respectively (P<0.05). 4. Postoperative complications were minimal except urinary retention in I4 out of 25 cases (56%) undergoing the endoscopic bladder neck suspension procedure, and nine of them (64.3%) were restored to normal voiding pattern within two weeks by intermittent catheterization. 5. The success rate was 85.7% in the Marshall-Marchetti, and 91.3% in the endoscopic bladder neck suspension procedure with a minimum follow up of six months. With above results. we concluded that the endoscopic bladder neck suspension procedure had advantages including operative simplicity, extensive indication, a high rate of cure, short hospitalization and postoperative catheter drainage compared with the Marshall-Marchetti procedure.

6.Reference Range of Values of Peritoneal Equilibration Test and Changes in Peritoneal Kinetics in Korean Long-Term CAPD Patients.

PURPOSE: The aim of this study was to define the reference range of the peritoneal solute transfer in Korean CAPD patients and to investigate the change in peritoneal kinetics in long-term patients. METHODS: Routine PET was done 14 days after catheter break-in in 102 new CAPD patients and using our reference range of PET, the patients were categorized into 4 groups as described by Twardow- ski. In 35 long-term (>3 yrs) CAPD patients, D/Pcr, D/D0 glucose and drain volume in last follow- up (mean duration, 60+/-23 months) PET were compared with those in initial PET. RESULTS: Dialysate to Plasma creatinine ratio (D/ Pcr) at 0 hour, 2 hours and 4 hours was 0.045+/-0.062, 0.408+/-0.151 and 0.612+/-0.154, respectively. D/D0 glucose at 2 hours and 4 hours was 0.594+/-0.129 and 0.398+/-0.121, respectively. Peritoneal solute transfer rate in Korean patients was slightly lower than previously reported results in North American patients (P=0.08). Mean drain volume was 2360+/-270ml and mean residual volume was 436+/-178ml. Drain volume was well correlated with D/D0 glucose (positively, r=0.375, P<0.01) and D/Pcr (negatively, r=-0.345, P<0.01). There were no difference in peritoneal solute transfer, drain volume and residual volume according to sex, age, body surface area and diabetes. In initially high and high average trans- porters, solute transfer rate decreased significantly in followup PET. Out of 7 initially high transporters, 3 remained in high and 4 became high average transporters. Out of 12 initially high average transporters, 3 became low average transporters. Out of 8 initially low average transporters, 2 became high average transporters. And out of 8 initially low transporters, 2 became high average and 1 became high transporters. The number of episodes of perito- nitis do not significantly correlated with the changes of peritoneal solute transport rate. CONCLUSION: These data suggest that peritoneal solute transfer rate in Korean patients is slighthly lower compared with that in North American patients and in long-term CAPD patients the peri- toneal solute transport rate changed to centropetal directions (towards to mean) according to initial PET result.

7.The Change of Late Potential in Acute Myocardial Infarction and the Influence of Patency of Infarct-Related Artery on Its Development.

BACKGROUND: The restoration of anterograde flow in the infarct-related artery(IRA) may improve electrophysiological consequences and survival. Patients with occluded IRA are more likely than those with patent IRA to have late potentials on signal-averaged electrocardiogram(SAEKG). The natural history of late potentials in the acute phase after AMI was reported to vary in recent studies and requires further investigation. Therefore, we investigated 1) whether parameters of SAEKG and incidence of late potentials are changed in 2 weeks after AMI, and 2) whether the status of IRA, ejection fraction, ventricular premature beats(VPB) on Holter and the use of thrombolytic agent can influence the developement of late potentials. METHOD: SAEKGs were performed in 40 patients with acute myocardial infarction(AMI, mean age : 56.6yr) and 20 normal controls(mean age : 53.0yr). SAEKGs were recorded first in 48 hours and then 2 weeks after AMI. Late potential were considered to be positive if two of the three following criteria were identified : 1) duration of filtered QRS complex>118ms, 2) duration of HFLA signal>40 ms, and 3) RMS voltage<20microV. RESULT: 1) The duration of filtered QRS complex and HFLA signal in patients with AMI in 48 hours and 2 weeks were longer than in the controls(p<0.01, p<0.05. respectively), and the RMS voltage in only 2 weeks was lower than in the controls(p<0.05). 2) There was no significant difference in the 3 parameters of SAEKG between 48 hours and 2 weeks after AMI, but the incidence of late potentials increased from 35%(14/40) to 42.5%(17/40). Of the 26 patients with a normal initial SAEKG, 3(11.5%) had late potentials in 2 weeks, whereas none of the patients with positive late potentials on initial SAEKG was normalized in 2 weeks. 3) The ejection fraction, the degree of VPB on the Holter EKG and the history of thrombolyic therapy in patients whose late potentials were positive were not different from those in patients with normal SAEKG. 4) In the 3 parameters of SAEKG, RMS voltage in patients with occluded IRA was significantly lower than in those with open IRA(p<0.05), and the incidence of late potentials were higher in the patients with occluded IRA(75%) than in those with open IRA(28.6%)(p<0.05). CONCLUSION: The incidence of late potentials on SAEKG after AMI was modestly increased in 2 weeks, but the development of late potentials was not influenced by the degree of frequently in patients with occluded IRA. These findings may indicate that the development of late potential after AMI tends to increase even in 2 weeks, and that the occlusion in IRA may unfavorably alter the electrophysiological state identified by SAEKG.

8.A Comparison of the Corticosteroid Intralesional Injections Alone versus Corticosteroid Intralesional Injections with Cryotherapy on Hypertrophic Scars and Keloids after Thyroidectomy: A Split-scar Study.

BACKGROUND: Various treatment modalities for hypertrophic scars and keloids have been used. However, there is no consensus as to what the optimum approach should be. Most common treatments are corticosteroid intralesional injection (ILI) and cryotherapy as well as combination of these two modalities. To this date, however, there are no prospectively comparative, scar-split studies between steroid ILI monotherapy and combination of steroid ILI and cryotherapy. OBJECTIVE: The purpose of this article is to investigate and compare the efficacy of steroid ILI monotherapy and the combination of steroid ILI and cryotherapy. METHODS: Eighteen women who had thyroid operation scars were recruited. Patients received steroid ILI with cryotherapy on the right half, and steroid ILI monotherapy on the left half of the scar. Patients were treated for four sessions with three weeks of intervals. Subjects were evaluated on their scar status with the modified Vancouver scar scale (MVSS) and scar redness by using colorimeter at baseline and every visit day. RESULTS: After four treatment sessions, MVSS was significantly improved on both sides of scar. Significant improvement was observed after one treatment session on the right half, and after two treatment sessions on the left half. There was no significant difference between left and right side after four sessions of treatment. The scar redness of both sides of scar showed no significant differences between the baseline and at the end of the study. CONCLUSION: Both corticosteroid ILI with cryotherapy and corticosteroid ILI monotherapy are effective treatment modalities for hypertrophic scars. However, the results of the present study suggest that a combination therapy might lead to more rapid improvements.

9.Expression of Osteoprotegerin and RANK Ligand in Breast Cancer Bone Metastasis.

Bone destruction is primarily mediated by osteoclastic bone resorption, and cancer cells stimulate the formation and activation of osteoclasts next to metastatic foci. Accumulating evidences indicate that receptor activator of NF-kB ligand (RANKL) is the ultimate extracellular mediator that stimulates osteoclast differentiation into mature osteoclasts. In contrast, osteoprotegerin (OPG) inhibits osteoclast development. In order to elucidate a mechanism for cancer-induced osteoclastogenesis, cells from a human breast cancer line, MDA-MB-231, were directly co-cultured with ST2, MC3T3-E1, or with primary mouse calvarial cells. Osteoclast-like cells and tartarate resistant acid phosphatase (TRAP) activities were then quantitated. We examined these cell lines and samples from breast cancer by RT-PCR for the expressions of OPG and RANKL mRNA. Compared to controls, co-culture of MDA-MB-231 cells with stromal or osteoblastic cells induced an increase in number of osteoclasts and TRAP activities. MDA-MB-231 cells alone or breast cancer samples did not express RANKL mRNA. However, co-culture of these cancer cells with stromal or osteoblastic cells induced RANKL mRNA expression and decreased OPG mRNA expression. These experiments demonstrate that direct interactions between breast cancer and stromal or osteoblastic cells induce osteoclastogenesis in vitro through modulating RANKL expression.
