1.The Burden of Aged Parents Caring for Adult Children with Disabilities.

PURPOSE: This study was to describe the burden of aged parents caring for adult children with disabilities and related factors. METHODS: The subjects were 123 caregivers aged over 65 who were caring for 18-year-old or older children with disabilities. The research tool of this study was a structured questionnaire on family burden. Data were collected from June 3 to 25, 2010, and analyzed by Cronbach's alpha, mean, standard deviation, t-test, and ANOVA using SAS 9.2 program. RESULTS: The major findings of this study were as follows. Elderly parents caring for adult children with disabilities perceived a moderate level of burden. The burden from concern over their children's future was highest, and economic and physical burdens were higher when the parents were younger. Burden was significantly different according to parents' characteristics such as gender, perceived health status, disease, the costs of caring for disabled children, and children's characteristics such as disability rating, health status, and ADL. CONCLUSION: In order to reduce the burden of elderly parents caring for adult children with disabilities, we need to improve their health status and assess comprehensive policies.

2.The Health-related Quality of Life for Children with a Mentally Ill Parent

Purpose:
This study is aimed to identify the health-related quality of life for children with a mentally ill parent.
Methods:
The 13 participants were school-aged children whose parents were registered at the D Regional Mental Health Welfare Center. Data were collected using one-on-one interview with illustration cards and analyzed by content analysis.
Results:
The participants were living a difficult life in anxiety amid a reversal of parent-child role, such as doing housework and taking care of their parents. The study revealed a love-hate family relationship that the participants wanted parental recognition and attention but they were frustrated by insufficient parental care and sibling conflict. Nevertheless, they only had each other themselves to trust and rely on. Their mixed health awareness and negative emotions were influenced by parents. Some of participants were exposed to dangerous environment such as domestic violence, and they need support system for help in difficult situations. Sometimes they felt happy by satisfying physiological, social, and self-esteem needs. They also showed a positive potential that they were matured more than peers through the experience of overcoming difficulties.
Conclusion
Not only were there not enough attention and support for the children with mentally ill people, but they were also exposed to an environment that threatens their physical or mental health. Therefore, to improve their health-related quality of life, there should be some integrated support of the community health system to cope with the challenges they face.
3.A Study on Dementia-related Knowledge and Attitudes in Adolescents.

PURPOSE: The purpose of this study is to examine adolescents' knowledge and attitudes towards dementia and to provide basic data for educating about knowledge of dementia and strengthening positive attitudes towards dementia. METHODS: This study used a descriptive research design. The subjects were 502 middle and high school students. A questionnaire organized by 16 questions of knowledge and 10 questions of attitudes towards dementia was used. RESULTS: The subjects' knowledge level of dementia was low, and average score was 8.89+/-2.95. The questions with low rate of correct answer were "Dementia is caused by several dozens of diseases such as internal medicine, neurology, psychiatry, etc.", "In spite of the dementia, patients can enjoy their favorite things." The subjects' average score of attitudes towards dementia was 3.72+/-0.58. Questions of negative attitudes were "I don't want to be closed to patients of dementia.", "I watch information or prevention about dementia in mass communications media." The relationship between dementia-related knowledge and attitudes showed positive correlation. CONCLUSION: The result means the mediation is necessary for improving dementia-related knowledge and strengthening positive attitudes in adolescents. The education programs should be proceeded to provide adolescents with correct information about dementia.

4.Discovery of Outpatient Care Process of a Tertiary University Hospital Using Process Mining.

OBJECTIVES: There is a need for effective processes in healthcare clinics, especially in tertiary hospitals, that consist of a set of complex steps for outpatient care, in order to provide high quality care and reduce the time cost. This study aimed to discover the potential of a process mining technique to determine an outpatient care process that can be utilized for further improvements. METHODS: The outpatient event log was defined, and the log data for a month was extracted from the hospital information system of a tertiary university hospital. That data was used in process mining to discover an outpatient care process model, and then the machine-driven model was compared with a domain expert-driven process model in terms of the accuracy of the matching rate. RESULTS: From a total of 698,158 event logs, the most frequent pattern was found to be "Consultation registration > Consultation > Consultation scheduling > Payment > Outside-hospital prescription printing" (11.05% from a total cases). The matching rate between the expert-driven process model and the machine-driven model was found to be approximately 89.01%, and most of the processes occurred with relative accuracy in accordance with the expert-driven process model. CONCLUSIONS: Knowledge regarding the process that occurs most frequently in the pattern is expected to be useful for hospital resource assignments. Through this research, we confirmed that process mining techniques can be applied in the healthcare area, and through detailed and customized analysis in the future, it can be expected to be used to improve actual outpatient care processes.

5.Antibody Responses in Hematopoietic Cell Transplantation Recipients after Vaccination Against Haemophilus Influenzae Type b and Streptococcus pneumoniae.

PURPOSE: Hematopoietic cell transplantation (HCT) recipients are vulnerable to invasive infection by Haemophilus influenzae type b (Hib) and Streptococcus pneumoniae (Sp). This study was performed to evaluate immune responses after Hib and Sp vaccination in Korean pediatric HCT recipients. METHODS: Patients were prospectively enrolled at Samsung Medical Center during 2009-2011. ELISA tests to detect anti-PRP IgG antibody and antibodies to Sp serotypes 4, 6B, 9V, 14, 18C, 19F, and 23F were performed at the Center for Vaccine Evaluation and Study, Ewha Medical Research Institute. RESULTS: Ten patients (two allogeneic, eight autologous recipients) with median age 5.4 years (range 2.7-12.2 years) were enrolled. Before Hib vaccination, 60% of patients' anti-PRP IgG titers were below 0.15 microg/mL. After vaccination, 100% of patients' anti-PRP IgG titers increased above 0.15 microg/mL (cut-off value for detection) and 1.0 microg/mL (cut-off value for seroprotection). For pneumococcus, in 2-5 year-old patients, pre-vaccination geometric mean concentrations (GMCs) of IgG for six serotypes (4, 6B, 9V, 14, 18C, and 23F) were below 0.35 microg/mL and at 5 months post-vaccination GMCs of IgG for all seven serotypes increased to above 0.35 microg/mL. In patients older than 5 years, pre-vaccination GMCs of IgG for four serotypes (4, 9V, 14, and 23F) were below 0.35 microg/mL and at 3 months post-vaccination GMCs of IgG for all seven serotypes increased to above 0.35 microg/mL. CONCLUSION: Most HCT recipients had low or no protective antibodies to Hib and Sp before vaccination, but showed good immune responses to protective levels after vaccination.

6.Differential expression of microRNAs in the saliva of patients with aggressive periodontitis: a pilot study of potential biomarkers for aggressive periodontitis

Purpose:
The aim of this study was to compare microRNA (miRNA) gene expression in saliva using miRNA polymerase chain reaction (PCR) arrays in healthy and aggressive periodontitis (AP) patients.
Methods:
PCR arrays of 84 miRNAs related to the human inflammatory response and autoimmunity from the saliva samples of 4 patients with AP and 4 healthy controls were performed. The functions and diseases related to the miRNAs were obtained using TAM 2.0. Experimentally validated targets of differentially expressed miRNAs were obtained from mirTarBase. Gene ontology terms and pathways were analyzed using ConsensusPathDB.
Results:
Four downregulated miRNAs (hsa-let-7a-5p, hsa-let-7f-5p, hsa-miR-181b-5p, and hsa-miR-23b-3p) were identified in patients with AP. These miRNAs are associated with cell death and innate immunity, and they target genes associated with osteoclast development and function.
Conclusions
This study is the first analysis of miRNAs in the saliva of patients with AP.Identifying discriminatory human salivary miRNA biomarkers reflective of periodontal disease in a non-invasive screening assay is crucial for the development of salivary diagnostics. These data provide a first step towards the discovery of key salivary miRNA biomarkers for AP.
7.Variation of Attenuation Value of Pancreas at Dual Phase MDCT: Comparison of the Bolus-tracking Technique vs. the Fixed Scan Delay Protocol.

PURPOSE: To clarify the difference between the bolus-tracking technique and a fixed scan delay protocol in the achievement of the optimal pancreatic phase of the pancreas with MDCT. MATERIALS AND METHODS: 526 patients underwent pancreatic and portal venous phase imaging of the pancreas using 16-channel MDCT. All the examinations were randomized into either scanning using a bolus-tracking technique with a scan delay of 20s after the aorta was enhanced > 100 HU (groups 1 and 2) or scanning with a scan delay of 38 s from the beginning of the injection (groups 3 and 4). A contrast material of 300 mgI/mL (groups 1 and 3) or 370 mgI/mL (groups 2 and 4) at an injection speed of 3 mL/sec was injected at 2 mL/kg body weight. The pancreatic CT attenuation values were compared. RESULTS: The scan delay times of the pancreatic phase in groups 1 and 2 were 38+/-3.8s and 37.4+/-3.4s, respectively. At the pancreatic phase, the pancreatic attenuation values of groups 1 and 2 were slightly higher than those of groups 3 and 4 (115.5+/-15.4 vs 111.7+/-15.1HU; p=0.093, 128.3+/-17.1 vs 119+/-17.1HU; p=0.003). There was no significant difference between groups at the portal venous phase. CONCLUSION: The use of a bolus-tracking technique in the optimal pancreatic phase of pancreatic CT does not significantly improve the pancreatic enhancement but does at higher iodine concentrations.

8.Embolization of Cerebral Pial Arteriovenous Fistula Under Balloon-assisted Flow Control Using NBCA: a Case Report.

Intracranial pial arteriovenous fistulas are rare vascular lesions of the brain. These lesions are composed of one or more direct arterial feeders with a single draining vein and usually have associated venous varix or giant venous aneurysms. Pial fistulas can be treated by endovascular occlusion or by microneurosurgery and endovascular treatment offers a simple and safe option. We report a case of pial fistulas treated with embolization using n-butyl cyanoacrylate (NBCA) and Lipiodol mixture in association with a balloon in the afferent artery to slow down the flow within the fistula.

9.Genetic Diversity of Echovirus 6 Strains Circulating in Korea.

Echovirus 6 (ECV6) is the prevalent serotype detected in aseptic meningitis cases in Korea. To analyze the genetic variation of ECV6 isolates recently circulating in Korea, we determined the partial sequence of the VP1 capsid gene from 22 Korean ECV6 isolates and performed pairwise analysis against 42 reference strains from the GenBank database using MegAlign. The 22 Korean ECV6 isolates formed 3 distinct genetic clusters: Kor-lineage I, II, and III. The Korean ECV6 strains showed significant genetic diversity with 14.8~22.8% nucleotide divergence among the 3 different lineages. These ECV6 Kor-lineages were demonstrated to belong to different genetic clusters using VP1 sequence-based phylogenetic analysis, implying that the recently circulating Korean ECV6 strains have potential antigenic variation.

10.Analytical Performance of Bio-Rad D-100 on a Hemoglobin A1c Assay.

BACKGROUND: Hemoglobin A1c (HbA1c) is considered a marker useful for the follow-up and diagnosis of diabetes and implies the importance of reliable assay methods that are traceable to a reference method. We evaluated analytical performance of a new high-performance liquid chromatography system for the HbA1c assay: D-100 from Bio-Rad Laboratories (USA). METHODS: We evaluated precision, linearity, and carry-over of D-100, according to the Clinical and Laboratory Standards Institute's guidelines. Comparative analysis of D-100 with Integra 800 (Roche Diagnostics, Germany) and Capillarys 3 (Sebia, France) was conducted. Additionally, we evaluated the throughput of the three instruments. RESULTS: Precision of low- and high-concentration controls in D-100 showed a CV of less than 1%. The linearity was excellent (R²=0.999) in the range of 3.51-18.7%, and carry-over was not observed. HbA1c results of D-100 (n=144) showed good correlation with those of Integra 800 (r=0.993) and Capillarys 3 (r=0.996). The % bias between D-100 and Integra 800 or Capillarys 3 was within the allowable range at all 3 medical decision levels (5.7%, 6.5%, and 10.0%). Elapsed time in the analysis of the first sample by D-100 was shorter than that of Integra 800 (2.4 vs. 11.1 minutes), but subsequent samples took more time (0.8 vs. 0.3 minutes per sample). CONCLUSIONS: D-100 showed reliable analytical performance with good precision and linearity, minimal carry-over, and acceptable comparative characteristics relative to other instruments. D-100 is expected to be useful for clinical measurements of HbA1c for diabetes diagnosis and theranostics.
