2.Usefulness of Treadmill Exercise Test on Diabetes Mellitus.

We studied 193 Diabetics without any symptoms of coronary artery disease and 39 Normal healthy subjects as Control group, using treadmill exercise test and obtained following results. 1) The positive rate was significantly higher in Diabetics(19.4% in Normal ECG groups and 25.4% in total Diabetics)than Control groups(5.1%)(0.01
0.05). 5) There was no difference in HbAIC, Total cholesterol and Triglyceride between positive and negative groups(P>0.05). In summary, with exercise test, the positive rate was higher in Diabetics than in Normal control groups and it seemed to be treadmill exercise test is a useful screening method for evaluation of coronary artery disease as well as for assesment of treatment and follow up care.

3.Pararenal Pseudocyst : Report of a Case.

Pararenal pseudocyst is a collection of fluid within the renal fascia outside the renal capsule and a relatively rare clinical entity which commonly occur as a result of trauma. either accidental or operative. Histologically, the wall is composed mostly of dense collagenous fibrous connective tissue. with no cellular elements capable of cyst formation. A case of pararenal pseudocyst associated with renal hypertension developed in 6 year-old boy after blunt abdominal trauma was reported.

4.Cavernous Hemangioma of the Bladder: Report of a Case.

Cavernous hemangioma of the bladder is the rarest of all bladder tumors. Less than 75 cases were reported in the literatures since Broca reported the first case in 1869. We report a case of cavernous hemangioma of the bladder in a 7-year-old girl and literatures were reviewed.

6.Cotrel-Dubousset Instrumentation in Lumbar Spine Disorders Associated with Instability

In the surgical trestment of unstable Iumbar spinal disorders, various methods of instrumentstion have been used for stabilization of spine, correction of deformity and reduction of fracture, despite of their technical demands. This paper reports the results of 30 patients of Cotrel-Dubousset instrumentation and posterolateral fusion for the lumbar spinal disorders associated with instability. The results obtained were as follows:l. Of the 30 cases, males were 15 cases(50%), females were 15 cases(50%) and average age of the patients was 42 yesrs ranging from 16 to 65 years. 2. Types of the disorders were spondylolisthesis in 16, failed back syndrome in 6, spondylolysis in 2, spinal canal stenosis in 2, scoliosis associated with degenerative spondylitis in 2, and burst fracture in 2 cases. 3. Among the 28 cases, 11 cases were excellent and 14 cases were good in Steffee's criteria (2 cases of burst fractures were excluded). 4. In spondylolisthesis, the average percentage of slippage was changed from 20% preoperatively to 5% postoperatively and 5.2% at the final follow-up. The initial correction rate was 75% and the loss of correction was not significant in follow-up study. 5. The patients with neurological claudication were permitted ambulation from postoperative 3rd day. In the final follow-up, the 8 patients(80%) could walk for more than 1 hour without pain. 6. The complications were relatively few(3 cases of hematoma, 2 cases of marginal sloughing of skin with superficial infection and one case of transient dysuria). In this study, this method seemed to be superior to others for the treatment of lumbar spinal disorders associated with instability especially degenerative spinal disorders in the respect of stability. Simultaneously, it provides immediate postoperative rehabilitation without rigid external support.

7.Acral Lentigenous Malignant Melanoma Associated with Pseudocarcinomatous Hyperplasia.

We report a interesting case of acral lentigenous malignant melanoma associated with pseudocarcinomatous hyperplasia in a 65-year-old women A skin lesion showed a 5 x 5cm sized, black colored protruding mass with ulcerative surface on the right great toe for the last 10 years. Histopathological findings reveled hyperplasia of epidermis and the epithelium of the upper protion of the adnexa admixed with a proliferation of atypical epitheloid melanocytes that involved the mid and lower portion of the epidermis and the papillary dermis. There were dyskeratotic cells and pseudohorn cysts between the aggregates of atypical melanocytes. Immunhistochemical staining revealed that the nucleus and cytoplasm of the atypical epitheloid cells were strongly positive for S-100 protein and negative for keratin. Keratin staining revealed aggregates of positively stained kertinocytes with dyskeratosis and pseudohorn cysts admixed and trapped among negatively stained atypical melnocytes. A unusual case of malignant melanorna associated with pseudocarcinomatous hyperplasia in which conventional hematoxylin and eosin stained sections were difficult to differentiation between malignat melanoma and squamous cell carcinoma. Immunohistochemical staining for S-100 protein and keratin were essential in establesging the correct diagnosis.

Supernunmerary nipple is a developmental anomaly occuring alon, the course of the embryological milk lines. This entity has receieved little attention in the dermatologic literature and has been confused with a pigmented nevus in some cases. We have experienced two ease of the more unusual form of supern umerary nipple. According to the Kajavas classification, our caes are classified as polithelia pilosa and complete breast with nipple.

10.Comparison of MAST Chemiluminescent Assay(MAST-CLA) with Skin Prick, Test in Patients with Atopic Dermatitis.

BACKGROUND: Allergic reactions are known to be associated with symptomatic aggravation of atopic dermatitis. Skin prick tests were used as a routine in vivo screening test for the evaluation of allergic patients. Many tests to detect specific IgE antibody including RAST, FAST and MAST chemilu-minescent assay (MAST-C~LA) were also used. Previous studies revealed that the results of skin prick tests and MAST-CLA were well correlated in patients with asthma, allergic rhinitis and urticaria. OBJECTIVES: The purpose of this study was to evaluate the effectiveness of MAST-CLA compared with skin prick tests in patients with atopic dermatitis. METHODS: We performed the chemiluminescent assay with 35 allergens and skin prick tests with 23 allergens common with allergens used in MAST-CLA in 34 patients with atopic dermatitis. The positive reaction rate of allergens in each test and sensitivity, specificity, efficiency, positive predictive value and negative predictive value of MAST-CLA to the skin prick test were evaluated and MAST net volts of serum total IgE was compared with PRIST.
