Sinus of Valsalva*

Lung is known as a vulnerable organ to metastatic tumors. Metastasis occurs mainly through lymphatics but seldom via blood vessels. Adenocarcinoma with mucin emboli is very rare. Primary foci reported in the literature were breast, lung, ovary and pancreas. A lung biopsy from a 60-year-old male patient showed floating mucin associated with metastatic adenocarcinoma to the lung in the blood vessels. The tumor cells spreaded along the vascular endothelium as if they were vascular endothelial cells. The tumor cells and mucin in the vessel were strong positive for PAS staining. Elastic and immunohistochemical staining for smooth muscle actin highlighted the vascular wall in the mucin containing structure with infarction of the lung. The primary focus was identified in the pancreatic head through the abdominal ultra-sonographic and computer-tomographic examination.

Natural cytotoxicity mediated by natural killer (NK) cells is believed to play an important role in host anticancer defense mechanisms. The aim of this study is to compare the number of NK cells in patients with colorectal cancer and hemorrhoids, and before and after surgery in patients with colorectal cancer. Twenty colorectal cancer patients and twenty hemorrhoid ones were studied. Venous blood samples were obtained preoperatively, and on the 7th, and 14th postoperative days. Mononuclear cells were isolated over Ficoll-Hypaque gradients, and T cells, B cells, and NK cells were measured with CD3 FITC (T cell), CD 19 PE (B cell), and CD56 FITC (NK cell) antibody, The number of T cell (/mm3) was 1224, 1280, and 1125 at preoperative, 7th, and 14th postoperative day in hemorrhoid patients and 1195, 901, and 1060 in colorectal cancer patients respectively. The number of B cell (/mm3) was 243, 160, and 250 in hemorrhoid patients and 147, 78, and 113 in colorectal cancer patients. The number NK cell (/mm3) was 148, 156, and 143 in hemorrhoid patients and 129, 85, and 128 in colorectal cancer patients. There was no difference among Dukes stages in the number of NK cells. In conclusion, the number of NK cells was not changed in colorectal cancer patients compared with hemorrhoid ones. Major operation changed the number of NK cells in colorectal cancer patients.

PURPOSE: To discover an the operative technique to reduce the dog-ear formation which develops after epiblepharon and entropion repairing surgery. METHODS: This study reviewed the records of 38 consecutive patients who had undergone epiblepharon surgery. The results of the surgical correction, dog-ear formation and the complications were reviewed retrospectively. The new operative procedure adopted the sine curve-shaped skin incision instead of the conventional crescent-shaped skin incision. After resection of redundant skin and pretarsal orbicularis muscle, tarsal fixation of subcutaneous tissue and skin was performed. After the epiblepharon repair, we observed the formation of dog-ear by the modified Hotz procedure. RESULTS: There were 22 males and 16 females. The average age at surgery was 4.8 years and the average follow-up period was 22.4 weeks. There were no cases of cilia touch recurrence and dog-ear formation using the new operative procedure. CONCLUSIONS: This operative technique produced improved cosmetic result by preventing the formation of dog-ear which can develop after repair surgery for epiblepharon and entropion.

PURPOSE: To evaluate the effects of the prophylactic use of mitomycin C in low concentration and short duration (0.01%, 60 sec) in inhibiting haze formation after laser epithelial keratomileusis (LASEK). METHODS: Forty-two eyes of 71 patients with refractive error from -5.0 D to -8.50 D were enrolled in this study. The eyes were divided into 2 groups according to the use of mitomycin C. All eyes were examined for refraction, UCVA, BCVA, and the incidence and degree of corneal haze before surgery and at 1, 3, 6 months after operation. RESULTS: Fifty-one eyes were operated by LASEK with intraoperative application of 0.01% mitomycin C for 60 seconds and 20 eyes were operated by LASEK without mitomycin C. There was no statistical difference in UCVA, BCVA, and spherical equivalent between 2 groups. In the study group, the degree of corneal haze was 0.54+/-0.39. 0.38+/-0.48, 0.30+/-0.50 at 1, 3, 6 months after operation and in the control group, 0.92+/-0.58, 0.79+/-0.66, 0.50+/-0.60 at 1, 3, 6 months after operation respectively. The difference in the postoperative corneal haze between 2 groups was statistically significant at 1, 3 months after operation, but at 6 months the difference was not statistically significant. CONCLUSIONS: In this study, intraoperative single use of mitomycin C in lower concentration and short duration could reduce the formation of corneal haze effectively in early postoperative period after LASEK. A more longterm period of follow up will be required to assess the continuous effect of mitomycin C on inhibiting corneal haze after LASEK.
