3.Von Recklinghausen' s Disease with Plexiform Neurofibroma , Giant Pigmentation , and Skeletal Abnormalities.

Plexiform neurofibroma is considered a pathognomic of Von Recklinghousen's disease, which involves the deep and large nerve trunk. These are large irregular nerve fascicles which result from an increase in endoneural matrix within individual nerve facicles, without an increased number of nerve fibers. We experenced a case of Von Recklinghausen's disease in a 24 year-old male who had variable cutaneous skeletal, and CNS lesions. He presented multiple neurofibromas, cafe-au-lait spots, and axillary freckles as common cutaneous lesions of NF-I and giant pigmentation, sacral hypertrichosis, and plexiform neurofibroma as unusual cutaneous lesions. Also he had a scoliosis, bowing deformity of the humerous and wedging deformity of the body of the 5th cervical spine as a skeletal manifestation and cortical calcification in the occipital area as a CNS manifestation.

Stabilization of the unstable spine created by the posterior decompression is as important as the decompression itself in the treatment of spinal stenosis. The purpose of this study is to evaluate the effectiveness of C-D pedicle screw fixation in stabilization after lumbar decompression, in reduction of spondylolisthesis and in restoration of the lumbar sagittal curvature. C-D pedicle screw fixation was performed in 102 spinal stenosis patients after posterior lumbar decompression and fusion during the period from March 1987 to December 1988. Their age ranged from 15 to 72 years with an average of 49.1 years. There were 34 males and 68 females. The follow up was from 6 to 21 months with an average of 12.5 months. The causes of spinal stenosis were degenerative in 50 patients, spondylolisthesis in 39, iatrogenic in 9 and degenerative lumbar scoliosis in 4. Objective clinical results showed significant improvement of claudication, SLR limitation, motor weakness, sensory and DTR changes in most patients. Following results were obtained from the study of C-D pedicle screw fixation after posterior decompression in lumbar spinal stenosis.1. C-D pedicle screws provide the secure fixation that allows early ambulation and shorter hospital stay. 2. C-D pedicle screws enable the reduction of spodylolisthesis at the time of posterior stabilization. 3. C-D pedicle screw fixation is successful in the restoration and maintenance of sagittal curvature of the lumbar spine. 4. C-D pedicle screw fixation enables the correction of scoliosis at the time of posterior decompression.

5.The Efficacy of Stereotactic Vacuum-assisted Biopsy and Needle Localization Vacuum-assisted Biopsy for Diagnosing Breast Microcalcification.

PURPOSE: This study was conducted to evaluate the efficacy of 2 percutaneous breast biopsy techniques for diagnosing microcalcification: stereotactic Vacuum-assisted biopsy (VAB) and needle localization VAB. METHODS: Between November 2002 and September 2007, 138 patients underwent percutaneous breast biopsy for microcalcification. Of these, 59 patients underwent needle localization VAB and 79 patients underwent stereotactic VAB with using a prone-table mammographic unit, respectively. Patients with focally clustered microcalcification underwent stereotactic VAB. Patients with diffuse or deep seated microcalcification and patients with small breast underwent needle localization VAB. We retrospectively reviewed the characteristics of the lesions and the percutaneous biopsy results for all the cases. RESULTS: Percutaneous biopsy was successful in 135 cases (97.8%). Of the 135 successfully performed cases, 34 cases (25.2%) were malignant and there were 4 (11.8%) underestimations. For the stereotactic VAB group, 13 of the successfully performed 76 cases (17.1%) were malignant and there were 2 (15.4%) underestimations. In needle localization VAB group, 21 (36.6%) of the 59 cases were malignant and there were 2 (9.5%) underestimations. There was no major complication in all the cases. CONCLUSION: With using stereotactic VAB and needle localization VAB, we can biopsy accurately and safely in almost all cases with leaving only minimal scar. So, percutaneous biopsy can be considered as a substitute for surgical biopsy for microcalcification of the breast.

8.A Case of Pruritic Folliculitis of Pregnancy.

Pruritic folliculitis of pregnancy of pregnancy is a specific dermatoses of pregnancy which is characterized by erythematous follicular papules and pustules between the fourth and ninth month of pregnancy. It has usually resolved by 4 weeks of postpartum and has no adverse implications for mother and baby. We report a case of pruritic folliculitis of pregnancy in a 24-year-old primigravida woman who had had monomorphic erythematous follicular papules and pustules on her anterior chest wall and back. Histopathological findings of erythematous papules showed acute folliculitis and perifolliculitis. The skin lesions improved rapidly with 2 weeks of delivery without treatment.

BACKGROUND: Topical irritants disrupt the cutaneous permeability barrier through the removal of stratum comeum lipids. This perturbation of barrier integrity stimulates a variety of homeostatic repair responses that ultimately result in the normalization of bamer function. Object To measure the effect of desoxymethasone ointment, vaseline and hydrobase on the barrier recovery of benzalkonium chloride (BKC) imtated skin. MATERIALS AND METHODS: The left flank skin of 2-3 monthold hairless mice was treated with BKC and then desoxymethasone ointment, vaseline and hydrobase were applied. Transepidermal water loss (TEWL) was checked after 0, 3, 6, 9, 12, 15, 18, 21 and 24 hours. Electron microscopic examination was performed after 3 and 24 hours after desoxymethasone, vaseline and hydrobase had been applied. RESULTS: The recovery of TEWL was most prominantly observed in the desoxymethasone ointment treated group followed by vaseline and hydrobase. Electron microscopic examination using ruthenium tetroxide fixation revealed that secretion and numbers of lamellar bodies and complete formatice of lipid bilayers were most prominent at desoxymethasone ointment and vaseline treated group. CONCLUSION: Desoxymethasone ointment, vaseline and hydrobase can be good agents in improving bamer recovery after exposure to irritant material.

10.A Case of Congenital Solitary Morphea Profunda.

A 4-year-old boy has had a solitary sclerotic depressed plaque on the right anterior chest since birth. The histopathologic findings are consistent with morphea profunda: thickening, hyalinization, and homogenization of collagen bundles in the dermis and subcutaneous tissues, admixture with a prominent lymphocytic and plasma cell infiltrate, and sweat glands en-trapped between the thickened collagen bundles. We report a case of congenital solitary morphea profunda.
