1.Evaluation of family function in the inpatient of rehabilitation medicine department witn impairment of activity of daily living.

BACKGROUND: When one of family members must be treated, the others are required to get various forms of changes including modification of their proper roles in the family. Particularily when he or she can not perform his or her activities of daily living(ADL) owing to functional impairment by himself or herself, there seems to be greater dependence upon his or her family. Accordingly, we tried to give help in the treatment of patients through the comparative analysis concerning functional impairment in both groups on the assumption that there is difference of family function between two family groups which have a patient in family, or not. METHODS: This study was carried out to 83 inpatients in Sun Hospital in Dae Jeon and local rehabilitation clinics in Iksan from 1. July. 1995 to 31. Jun. 1996. We evaluated activites of daily living by using modified Barthel Index(MBI) and classified them into dependent group with 60 points or less(6roup I ), and independent group with more than 60 points(Group II). We also used Family APGAR score and FACES III to evaluate their family functions. RESULTS: MBI scores to evaluate ADL were 42.9+/-15.6 in Group I and 82.1+/-17.9 in Group II. Family APGAR scores were 6.93+/-2.52 in Group I and 7.24+/-17.9 in Group II but there was no significance between two groups. Types of family according to Family APGAR score were highly functional, morderate dysfunctional, and severly dysfunctional one in order of frequency in both groups, but there was no significance in their frequencies in both groups. Types of Family in view of cohesion was disengaged, separated, connected, and enmeshed one in order of frequency in Group I, and separated, disengaged, connected, and enmeshed one in order of frequency in Group II but there was no significance between two groups. Types of family in view of adaptability were rigid, chaotic structured,and flexible one in order of frequency in Group I, and were flexible, structured, rigid, and chaotic one in orders in Group II, which showed significance between two groups. Extremal types of family structure were large in numbers in Group I, but it didnt show significance. CONCLUSIONS: Assuming that there were large numbers of rigid and chaotic family in Group I, it is considered that tne family which has a patient with severely impaired function seems to have weaker adaptability to their family stresses than otherwise. Therefore, it is desirable that physicians who take charge of such patients provide continuous and comprehensive medical care for them including their family with greater concerns and through analysis and assessment of their family functions.

2.Two cases of supernumerary nipple.

Supernunmerary nipple is a developmental anomaly occuring alon, the course of the embryological milk lines. This entity has receieved little attention in the dermatologic literature and has been confused with a pigmented nevus in some cases. We have experienced two ease of the more unusual form of supern umerary nipple. According to the Kajavas classification, our caes are classified as polithelia pilosa and complete breast with nipple.

3.Pyoderma Gangrenosum Treated with Cyclosporine.

We present herein a case of pyoderma gangrenosum, which showed good response to cyclosporine therapy. The patient, a 47-year-old man, had suffered from a skin defect on the left anterior chest 2 months before the rapidly spreading pustules and ulcer with an inflammed, irregular undermined border appeared together with a hemorrhagic base which developed on his left anterior chest and upper abdomen. Cyclosporine was given orally in a dose of 150mg three times daily for 2 weeks, and tapering to 100mg three times daily for the next 3 weeks. Clinical improvement was observed after 14 day's treatment and all the skin lesions were replaced by granulation tissue with epithelialization after 35 day's treatment. After the 5-week period of treatment, blood urea nitrogen and creatinine rose to 29 and 1.8 mg/dl. Cyclosporine was stopped and then the patient was given prednisolone (10mg/day) in addition to dapsone(50mg/day) for 1 month. Recurrence has not been observed.

5.Acral Lentigenous Malignant Melanoma Associated with Pseudocarcinomatous Hyperplasia.

We report a interesting case of acral lentigenous malignant melanoma associated with pseudocarcinomatous hyperplasia in a 65-year-old women A skin lesion showed a 5 x 5cm sized, black colored protruding mass with ulcerative surface on the right great toe for the last 10 years. Histopathological findings reveled hyperplasia of epidermis and the epithelium of the upper protion of the adnexa admixed with a proliferation of atypical epitheloid melanocytes that involved the mid and lower portion of the epidermis and the papillary dermis. There were dyskeratotic cells and pseudohorn cysts between the aggregates of atypical melanocytes. Immunhistochemical staining revealed that the nucleus and cytoplasm of the atypical epitheloid cells were strongly positive for S-100 protein and negative for keratin. Keratin staining revealed aggregates of positively stained kertinocytes with dyskeratosis and pseudohorn cysts admixed and trapped among negatively stained atypical melnocytes. A unusual case of malignant melanorna associated with pseudocarcinomatous hyperplasia in which conventional hematoxylin and eosin stained sections were difficult to differentiation between malignat melanoma and squamous cell carcinoma. Immunohistochemical staining for S-100 protein and keratin were essential in establesging the correct diagnosis.

7.VERSATILITY OF RADIAL FOREARM FREE FLAP.

Microsurgical techniques have developed numerous territories suitable for free tissue transfer. However, there are so many flaps in plastic and reconstructive surgical field, so no one dare to say which flap is the best. The radiovolar area of the forearm constitutes a versatile source for composite tissue free flap reconstruction for many areas of the body. The skin is thin and relatively hairless, and the vascular pedicle is long and large caliber The flap can be harvested conjunction with tendons and bone. The skin can be reliably reinnervated. We have used 32 radial forearm free flaps to reconstruct defect in various areas of the body resulting from tumor ablation, postburn scar contracture and penile amputation injury. We had two flap failures, one developed necrosis from venous thrombus formation, the other from the infection due to sepsis. Thirty of the 32 flaps survived completely. Overall survival rate was 94%. The donor sites have been relatively inconspicuous.

8.The relationship between health habit and stress amount in life events.

BACKGROUND: Chronic stress is associated with physical illness such as coronary artery disease and hypertension. Daily life habits being able to affect physical health and are associated with mental stress. We studied the relationship between physical illness and stress amount so far. However, the investigation about the stress and the health habits to affect disease directly was not studied enough. Eventually we tested this investigation to know the relationship between stress amount and health habits. METHODS: We measured stress amount and health habits for a month(from May 1 to May 31, 1997) through the people who had already received health screening and were working as researchers. We used 147 results out of 212. Health habits were investigated about 6 articles out of "Breslow's 7 Health habits" and stress amount was measured by evaluation scale made by Lee, Pyoung-sook. RESULTS: There were no differences in amounts of stress of subjects according to age, years in work, hours of sleeping, having breakfast or not, the frequency of exercise and body mass index(BMI). But there were significant differences in amounts of stress of subjects according to drinking amount, smoking or not, and health habit index(P<0.01). CONCLUSIONS: Drinking and smoking appeared to be health habits related to the amounts of stress. Prospective studies are needed ? find a causal relationship between health habits and stress amount.

10.Suicidality and Related Psychopathology across Different Stages of Schizophrenia

Objectives:
This study aimed to investigate suicidal behaviors and the related psychopathology across the different stages of schizophrenia.
Methods:
We recruited 131 patients with schizophrenia and categorized them into two groups, according to the duration of illness (DI) as follows: ≤10 years (n=39) and >10 years (n=92). Psychopathology and suicidality were assessed using the 18-item Brief Psychiatric Rating Scale (BPRS-18) and the suicidality module from the Mini-International Neuropsychiatric Interview, respectively.
Results:
One-quarter of the patients with a DI ≤10 years and nearly one-sixth of the patients with a DI >10 years experienced suicidal behaviors in the previous month. Suicidality scores were significantly associated with the “affect” factor scores of the BPRS-18 in patients with a DI ≤10 years (β=0.55, p=0.003) and with the “resistance” factor scores in patients with a DI of >10 years (β=0.29, p=0.006).
Conclusion
The present study demonstrated that psychopathological factors were differentially associated with suicidality in patients with schizophrenia according to the illness stage. Our findings suggest that for effective suicide prevention, different approaches are required for the management of each stage of schizophrenia.
