3.Metastatic Adenocarcinoma with Mucin Emboli in the Lung: A case report.

Lung is known as a vulnerable organ to metastatic tumors. Metastasis occurs mainly through lymphatics but seldom via blood vessels. Adenocarcinoma with mucin emboli is very rare. Primary foci reported in the literature were breast, lung, ovary and pancreas. A lung biopsy from a 60-year-old male patient showed floating mucin associated with metastatic adenocarcinoma to the lung in the blood vessels. The tumor cells spreaded along the vascular endothelium as if they were vascular endothelial cells. The tumor cells and mucin in the vessel were strong positive for PAS staining. Elastic and immunohistochemical staining for smooth muscle actin highlighted the vascular wall in the mucin containing structure with infarction of the lung. The primary focus was identified in the pancreatic head through the abdominal ultra-sonographic and computer-tomographic examination.

Klippel-Trenaunay syndrome (KTS) is a rare congenital disorder and is essentially a disorder of capillary, venous, and lymphatic malformations. Hematochezia is the most common symptom associated with intestinal hemangiomatosis and remains one of the life-threatening emergencies in KTS. We reported one patient of KTS presented with rectal bleeding and severe anemia who was successfully managed by sphincter-saving operation.

6.Current Status of Stoma Researches in Korea.

Stoma operations are one of the most frequently performed operation by the colorectal surgeons. And the consequences of the stoma operation of the patients are well known. However, most colorectal surgeons who performed stoma operation were not aware of the problems that are part of the rehabilitation of the ostomates. In order to elucidate the current status of the stoma studies in Korea, literature reviews were performed by using the Korean medical literature database. Most of the Korean literatures written by the colorectal surgeons are related with the postoperative complications of stoma surgeries. Very few papers are concerned with rehabilitation issues including postoperative adaptation of ostomates, quality of life, and stoma nursing care. Recently, more and more papers concerning the stoma rehabilitation issues were published and the interests in the patient's adjustment to a change in body function are growing. Stoma rehabilitation program should be the part of the nations' welfare program which would be successful with the active participation of the colorectal surgeons, enterostomal therapists and ostomates. We, colorectal surgeons, and the Korean Society of Coloproctology should do the major role in stoma-related researches and cooperate with the enterostomal therapists and ostomates organizations in order to establish and develop the stoma rehabilitation program in Korea.

7.Intra-articular Avulsion Fractures of the Malleolus in Chronic Ankle Pain.

PURPOSE: We report our results of arthroscopic treatment of symptomatic avulsion fracture of the malleolus in chronic ankle pain, and also analyzed the clinical and radiological features for evaluating the good candidate for arthroscopic treatment. MATERIALS AND METHODS: Fourteen patients who were diagnosed with intra-articular avulsion fractures of the malleolus received arthroscopic surgery and were followed up for at least a year. The clinical and radiological characters including MRI and arthroscopic findings were reviewed. Clinical assessments were done according to the AOFAS score system. RESULTS: There was a history of inversion type of the injury in most cases and local tenderness of lesion site was a unique. MRI study showed thickened anterior talofibular ligament (ATFL) in 8 cases (57%) and discontinued ATFL in 3 cases (21%). Enhanced signal surrounding soft tissue corresponding to synovial inflammation and impingement was found in 12 cases (86%). Preoperative score of all patients were 74.0+/-5.5, which improved to 89.3+/-6.7 at the follow-up after the treatment (P<0.001). CONCLUSION: Most patients had history of injury and localized tenderness in the area coinciding with radiological findings. Thickened ATFL and contrast enhancement around the ossicle were frequently found. Symptomatic avulsion fractures of the malleolus associated with the clinical and radiological findings above could be a good candidate for arthroscopic treatment.

9.Local Resection for Treatment of Early Colorectal Cancer.

PURPOSE: Early colorectal cancer is defined as invasive tumor, limited to the mucosa or submucosa. The incidence of early colorectal cancer detection has been increased due to well designed screening technology and development of colonoscopy. The novel treatment of early colorectal cancer is still not settled despite of this advancement. We performed retrospective study about outcomes of colorectal cancer after radical resection or local resection. METHODS: Sixty two patients, diagnosed as early colorectal cancers by pathology, were selected for this case study. The hospital records were reviewed retrospectively and the following was found: Twenty four patients received local resection such as colonoscopic polypectomy or local resection of colon. Remaining thirty-eight patients received radical resection. The clinicopathologic features of two groups were analyzed statically and survival rate was compared. RESULTS: The clinical features were similar between two groups including sex, age, stage, tumor size and differentiation. The median follow-up duration was 47.3 months (range: 2~152 months). Survival rate was not different according to resection type. Recurrent cases were one patient from each group. They were all submucosal tumors. CONCLUSIONS: The local resection is safe treatment modality for early colorectal cancer. However, case selection for local resection should be cautious because submucosal cases have more recurrent potential. Longterm follow-up will be needed to achieve safety of early colorectal cancer.

10.A Comparative Analysis of Heart Rate Variability of Electrocardiogram and Pulse-wave Using Time Series.

The estimation of power spectrum based on the heart rate variability is studied to evaluate the variation of the tension and physiological workloads by the activity of the autonomic system as evaluation parameter. In this paper, we made an attempt to apply the pulse wave to estimate the variation of physiological condition quantitatively instead of the evaluation of power spectrum of HRV through electrocardiogram in the past by comparing and analyzing time series data for the similarity of the heart rate of electrocardiogram and pulse wave. For the similarity of two signals, we can get the likeness by calculating an average, a variance, a correlation coefficient, and the power peak and the shape of power spectral density from the time series data. In the experimental result, it is shown that same subject have a similar variation of time series and power spectrum density for electrocardiogram and pulse wave. As a result, it is expected to estimate a change of the tension and physical condition quantitatively through the evaluation of a power ~pectrum of HRV by Pulse Wave in the future.
