1.Effects of Ischemia on the Specific Activity of Ca++ - Mg++ -Activated ATPase in the Subcellular Fraction of Renal Tissues in Rabbits.

Calcium has been implicated as a primary pathogenetic mediator of cellular injury under conditions of oxygen and substrate deprivation in the kidney aswell as other tissues. It has been proposed that calcium entry from the external medium increases intracellular free calcium to toxic levels there-by converting cellular injury from a potentially reversible to an irreversible state. Cellular calcium homeostasis is a fundamental prerequisite for many coordinate cellular functions. The regulation of cellular Ca++ -metabolism is maintained mainly by Ca++ -ATPase. The purpose of the present study was to determine the effects of ischemia on the subcellular specific activity of Ca++ -ATPase of renal tissues in the rabbits. 1. In both normal cortical and medullary tissues, the specific activities Ca++ -ATPase were higher in mitochondria than in cytosol. 2. In cortical tissue, the specific activities of mitochondrial Ca++ -ATPase were increased with duration of ischemic time under normothermia. But the specific activities were decreased comparing to those of control under hypothermia, and the activities were not different with those of control in Verapamil treated group. 3. The specific activities of Ca++ -ATPase of cortical cytosol were increased as in mitochondria under normothermia. And the activities were similar with those of control in hypothermic and Verapamil treated groups. 4. In medullary tissues, the specific activities of mitochondrial Ca++ -ATPase were increased with duration of ischemic time under normothermia. In Verapamil treated group, the activities were decreased comparing to those of control. And there was no difference of activities between hypothermia and control groups. 5. As in cortical tissues, the specific activities of Ca++ -ATPase of medullary, cytosol were increased in normothermic group. And the activities were similar with those of control in hypothermic and Verapamil treated groups. Above results suggested that calcium contents of mitochondria and cytosol were increased with duration of ischemic time under normothermia. Hypothermia and Verapamil treatment seemed to be beneficial effects in prevention of calcium accumulation in the renal tissues.

5.Ectopic Salivary Gland Tissue of The Neck.

An 8-year-old girl had a rice sized erythematous nodule secreting mucoid fluid situated above the right sternoclavicular joint area since birth. Histopathological findings of the lesion revealed predominant mucous acini, serous acini, demilunes of mixed acini and excretory ducts in the deep dermis compatible with salivary gland. Electron microscopic findings revealed a lumen and serous cells containing serous granules and rough endoplasmic reticulurn. Recurrence has not been noted following total excision of the nodule.

6.Clinical Study of Intra-articular Fracture of Distal Radius

The treatment of intra-articular fractures of distal radius is difficult and has various problems. Maximum recovery of wrist function is dependent on accurate and stable reduction of the radial articular surface. We analysed one hundred and five cases of intra-articular fracture of distal radius followed for more than 6 months at Yonsei University from January 1980 to September 1988 and studied the result according to the type of treatment. The results of this study were as follows; 1. The incidence was high in female who aged over 6th decade, and in male who is active aged. 2. The main cause of this injury was fall down(34.3%). 3. In a group of closed reduction and cast immobilization, the functional result was satisfactory in 45.4%. 4. In a group of closed reduction and percutaneous pinning, the functional result was satisfactory in 50%. 5. In a group of open reduction and internal fixation, the functional result was satisfactory in 83. 3%. 6. In a group of external fixation, the functional result was satisfactory in 33.3%. 7. Overall result was mainly correlated with initial severity of the injury and anatomical reduction of the intra-articular surface.

8.Bilateral Naviculo-Medial Cuneiform Coalition: One Case Report

We report one case of bilateral naviculo-medial cuneiform coalition in ten year old girl. She complained of pain in the medial aspect of the mid-foot related to long periods of standing and activity. Routine roentgenographs, specifically lateral views showed a bony bridge between the navicular bone and medial cuneiform. A biopsy revealed as fibrocartilaginous tissue. The bony bridge consists about one third in lateral view and 30 degree cephalad tilting in the antero-posterior view. Bilateral resection arthroplasty, with interposing flesh muscle fibers of the abductor hallucis, was performed for restoration of motion in the naviculo-medial cuneiform joint and relief of localized pain. Four months after surgery, pain around the mid-foot had almost complete subsided. One year postoperative, neither recurrence nor disability was observed. We will discuss the diagnosis and treatment of this rare case of bilateral naviculo-medial cuneiform coalition.

9.A Case of Multiple Trichilemmal Cysts.

A case of multiple trichilemmal cysts (TC) is presented. TC is known to be a kind of keratinous cyst with lining cells showing trichilemmal keratiniration. A 63-year-old female patient presented with a 30 year duration of increasing in size and number of twenty five nodular lesions on the scalp. All twenty five TC on the scalp were totally excised and examined microscopically. However, the evidence of proliferation or malignant change like the previous case reports was not found in our case.

10.A Clinical Study on the Effect of a Facial Cleanser consisting of 1 % Triclosan and 0 . 5 % Ku Shen on Acne vulgaris.

BACKGROUND: Acne is a relatively common disorder, especially in the adolescent. The condition is characterizecl by comedones, papules and pusi:ules. Acne patients frequently wash their face. Cleansing with an effective agent is therapeutic and preventive for patients suffering from acne vulgaris. OBJECTIVE: The study was designed to compare thc efficacy and safety of a facial clemser consisting of 1% triclosan and 0.5% Ku Shen and a in in the treatment of facial acne vulgaris. METHODS: The study was camed out on two groups of people: a treatment and a control group. Efficacy and safety were assessed at baseline and at weeks 1, 2, 3, 4, 6, 8, 10 and 12. Efficacy was determined by investigating counts of non-inflamatory open and closed comedones, and inflamatory papules and pustules. Global improvement was also assessed. RESULTS: At week 12, the mean counts in the differe,nt lesions were as follows; 11.2 versus 17.2 for total lesions (p<0.05); 9.4 versus 11.3 for non-inflammatory lesions (p<0.05); 1.8 versus 5.9 for inflammatory lesions (p<0.05) in the treatment and control group, respectively. A Statistically significant difference was observed in patients overall self-assessment (p<0.05). The Group applying the facial cleanser with 1% triclosan and 0.5% Ku Shen felt significantly better than the one applying the control facial cleanser. Some patients developed mild and transient local side effects. CONCLUSION: Cleansing three times a day with a facial cleanser consisting of l% triclosan and 0.5% Ku Shen was found to be effective ancl safe for patients suffering from acne vulgaris.
