2.Mycological and Clinical Study on Dermatophytoses in Infants and Preschoolers.

The clinical and mycological study was made with 38 cases of dermatophytoses in infants and preschoolers in dermatologic clinic of St. Pauls Hospital, Catholic Medical College, from September, 1982 to April, 1984. Following results are obtained: The incidence of dermatophytoses under the age 6 was 2. 2% among 1, 711 out- patients of the sarne age group. 2. The incidence of involving sites was the highest in tinea(T.) corporis(37. 2%) and T. faciale(37, 2%) and then T. capitis(18. 7g). In 0-under 2 groups, the highest incidence in T. corporis(54. 5%) and then T. faciale(3l. 8%.). In 4-under 5 group, the highest incidence in T. capitis(55. 6%). 3 Fungi cultured from their skin lesions were 36 strains belonging to 6 species. These were M. canis(55. 6%), T. rabrum(27. 8%), granular type of T.mentagrophyte(8.3%) and interdigital type of T. mentagrophyte(27. 8%), M gypseum (2.8%). and E. floccosum(2.8%) in order. 4. Arnong 1Q strains of T. rabrum, anthropophilic fungi, 8 strains(80. 0%) were frorn 0-under 2 age groups, E. floccosum, and interdigital type of T. mentagrophyte were also from 0-under 1 age group. Among ZO strains of M, canis, zoophilic fungi, 13 strains(65. 0%) were from the age groups of 2-under 5. Granular type of T. mentagrophytc, 2 among 3 strains were from 4-under 5 age group, 5. The incidence of causative fungi in each sites were as follows: In T. corporis, T. rubrum(50. O%), and M canis(18. 8%) in order. In T. faciale, M. canis(62. 5%), T. rabrum(12. 5%), T. mentagrophyte(12. 5%), and M. gypseum(6. 3%) in order. In T. capitis, all cultured fungi were M. canis

3.Histopathologic Study of Primary Gastrointestinal Lymphoma: Gross and immunohistochemical analysis of 30 cases.

A histopathologic study including iramunohistochemical stains was made in 30 patients who were presented with gastrointestinal lymphoma. The occurrence was 13 in the stomach, 8 in the ileocecum, 7 in the small intestine and 2 in the colon. The disease more frequently affected males than females and the average ages were 53 years in the patients of gastric lymphoma and 44 years in the patients of intestinal lymphoma. Gastric lymphomas were usually presented with a single lesion, and the antrum and/or body were the most common sites. But intestinal lymphomas were presented with a single or multiple lesion, and the ileocecum was the most common site. The most common gross type of gastrointestinal lymphomas was the ulceroinfiltrating type and most are of the diffuse large noncleaved cell type of B-cell lymphoma, histologically. There were 2 cases of T-cell lymphoma presented in the intestine as the superficially ulcerative gross pattern and diffuse immunoblastic cell type. The distinct MALToma was seen in only one case of stomach but the feature was partially remained in each two cases of stomach and intestine. Their coexistent findings may suggest that diffuse large of immunoblastic component arises through blastic transformation of the low-grade M ALToma component.

6.Incontinentia Pigmenti: Report of 4 Cases in a Family.

Four cases of incontinentia pigmenti (Bloch-Sulzberger type) in a mother and three her daughter were presented and literatures were reviewed. In all three daughters vesicles appeared on toes at three weeks after birth and ehanged to pigmented patches surrounded by erythematous verrucous margin, which were then gradually spread to whole body. Pigmentation is dirty, irregular and bizarr. Severe itching sensation was complained and vesicles or bulla were formed easily due to scratching and external trauma. In mother, onset of the disease was not clear but generalized dirty pigmentation is showed. Dentition was delayed in 3rd daughter (Dental eruption was begun at 10 month old and 11 teeth was showed at now, 2 year old). Histopathological findings are compatible with incontinentia pigmenti and epidermo-dermaI separation is observed in a experimentally induced bullae, indicating a characterized epidermolysis bullosa. Strict limitation in the occurence of disease in females of one family, accompanying with epidermolysis bullosa simplex is unusual and interesting in genetical aspects and pathogenesis.

7.The Study for Apical Pulse Measurement Technique Through Hospitalized Children.

The purpose of this study was to determine the most accurate technique measuring the apical pulse rate, using three counting duration 15, 30 and 60 seconds, and two methods start '0' and start '1'. The instrument used in the study was the EKG moniter, stethoscope and stopwatch. Data was analyzed by utilizing SPSSWIN program. General characteristics of the subjects were analyzed by frequency, percentile, mean, SD. The subject of this research is made up of 46 children and 20 nurses. The children were infants, & under the age of 5. They were hospitalised in PICU & NICU in 2 tertiary hospitals in seoul from Jan. 1. 1998 to Sep. 10. 1998. The measurement of starting 1 & measurement of starting '0' used the T-test to find out the measurement error. Apical pulse duration of 15, 30, 60 seconds were used to find out measurement error, the measurement error depend on experience of Nurse were analyzed by using ANOVA. The result of this study are as follows : 1. When comparing the starting poin of apical pulse 0 & 1, starting with 1 the measurement error is less, but not statically significant. 2. When counting the apical pulse by 15, 30, 60 sec. 60 seconds counting duration was more accurate, but not statistically significant. 3. The mean of measure error : Group under 100/ min, is 10.33 : from 100 or 119/min, is 8.30 : from 120 to 139/min, is 4.76 ; from 140 to 159/min, is 6.09 : above 160, is 17.83. The differences of these groups are statistically significant. When 60sec were counted, under 140/min the mean of measurement error is 3.4. Also when 30 seconds were counted from 140/min to 159/min the measurement error is 7.14, above 160/min the measurement error is 16.4. That measurement mean is the smallest than the other durations. During the 15 sec. count the measurement error was the largest of them all. 4. By the experience of the nurses, the apical pulse count measurement error was discovered. Under a year experience this measurement error was the largest(11.09), 1 year to under 3 years, the error is the smallest(4.86). 3 year to under 6 years the error is 8.33, 5 years above the error is 6.11 but this is not statistical significant. Under a year experience when counting 15, 30, 60 seconds the error is the largest. The group of the nurses from a year to under 3 years, the measurement error is the smallest of all the groups. The result of the study is to determine the technique measuring the apical pulse rate, Hargest (1974), starting point '0' is not proved. When the pulse rate increases the 30 sec measurement rate is accurate. Under 140/min the 60 sec measurement rate is the most accurate. Depending on the nurses experiences, there is a variable difference to the apical pulse rate measurement. Especially new nurses training courses should enforce the children's pulse rate count and the basic vital signs.

8.A Case of Orbital Glioma.
Journal of the Korean Ophthalmological Society 1972;13(1):49-52
A 57 year old male, complaining of exophthalmos, disturbance of ocular motility and visual acuity in the right eye, was admitted under the disgnosis of orbital tumor. The tumor was removed by Kronlein procedure and verified as glioma microscopically. The following seven months period of postoperative observations revealed degenerative changes in the retina, atrophy of the optic nerve and enophthalmos in the affected side but no signs of recurrence.

9.Facial Cutaneous Mycosis by Paecilomyces lilacinus.

A 19-year-old man noticed a very slowly spreading erythematous macular lesion on his cheek when he was 5 years old. This asymptomatic and well circumscribed patch was covered with fine scales. He sought first medical attention at the age of 12 years and diagnosis was cutaneous tuberculosis was made by skin biopsy. Seven years after the second biopsy was made and it revealed a few spores within giant cells in the upper dermis. By mycologic study, it was identified as Paecilomyces lilacinus. The patient was put on the treatment of topical and oral antifungal agents. The skin lesion showed signs of improvement after 12 weeks of treatment.
