1.Extracorporeal Membrane Oxygenation for Complicated Scrub Typhus.

Scrub typhus is a mite-borne infectious disease caused by Orientia tsutsugamushi . Although early diagnosis and appropriate antibiotic therapy improve the prognosis for the majority of patients, life-threatening complications are not uncommon. Here, we present a case of successful veno-veno-type extracorporeal membrane oxygenation for scrub typhus-induced complications, including acute respiratory distress syndrome, myocarditis and multi-organ dysfunction. To our knowledge, this is the first case report of successful extracorporeal membrane oxygenation in complicated scrub typhus in Korea.

3.Hypercalciuria in High Risk Neonates.

The incidence and risk factors for hypercalciuria were studied in 23 sick preterm (Group I), 36 well preterm (Group II), 16 sick term(Group III)and 41 well term(Group IV) neonates who were admitted to NICU and nursery of Ewha Woman's University Hospital from May 1988 to February 1992. The results were as follws: 1) The incidences of hypercalciuria were 73.9% in sick preterm (Group I) and 56.3% in sick term (Group III) which were significantly higher than 13.9% of control preterm (Group II) and 7.3% of control term (Group IV). Maximal urinary Ca/Cr ration were 0.63+/-0.503 in group I and 0.50+/-0.513 in group III, significantly higher than 0.19+/-0.142 of control term and 0.17+/-0.131 of control term. 2) In sick neonates with hypercalcuria, birth wight and gestational age were significantly lower and the frequencies of frosemide and intravenous calcium therpy were significantly higher than those of sick neonates without hypercalciuria. 3) In sick neonates, furosemide increased the incidence of hypercalciuria to 90.9%, significantly higher than 57.1% in neonates without furosemide. Maximal urine Ca/Cr ratio 24 hour urine calcium were also significantly higher in neonates with furosemide than those of neonates without furosemide. 4) In sick neonates, intravenous calcium gluconate increased the incidence of hypercalciuria to 86.6%, significantly higher than 54.2% in neonates without intravenous calcium. Maximal urine Ca/Cr ratio and 24 hour urine calcium were also significantly higher in neonates treated with IV calcium than those of neonates without IV calcium. 5) Calcium intake and maximal random urine Ca/Cr ratio were positively correlated (r=0.46, p=o.037). In conclusion, the majority of sick newborns in our intensive care unit were hypercalciuric. Sick condition, premature, furosemide and intravenous calcium gluconate were the risk factors that could lead to hypercalciuria. Use of these agents in sick neonates especially preterm neonates requires careful monitoring of urine calcium excretion.

4.An Urinoma in a Premature Infant with Renal Cadidiasis.

We report a case of fine needle aspiration cytology of endodermal sinus tumor of the ovary. A 13-year-old girl complained of abdominal mass and pain. The abdominal sonography revealed a well-demarcated huge mass, which was solid and multiseptated. Percutaneous fine needle aspiration was performed from the mass. The smears revealed moderate cellularity, which was arranged in sheets or clumps of pleomorphic malignant cells on mucoid background. The valuable characteristic features of tumor cells were papillary configuration, vacuolated cytoplasm and intracellular and extracellular hyaline globules. The diagnosis was confirmed later by histologic study of surgical resection of the specimen.

6.Antimicrobial Effects of Nano-Silver Gauze against Common Bacterial Isolates.

BACKGROUND: Silver has been used for disinfection and sterilization. We aimed to confirm the in-vitro antibacterial effects of nanocrystalline silver-coated gauze. METHODS: Fourteen clinical isolates each of Escherichia coli and Acinetobacter baumannii were used. Bacterial suspensions made in tryptic soy broth were exposed to Ordinary and silver-coated gauze. Bacteria were then harvested from the gauze immediately and after 24 h incubation, cultured on blood agar plates and eunmerated for viable counts. The number of colonies was converted into common logarithms for comparison. RESULTS: The number of colonies recovered from silver-coated gauze was significantly lower than those recovered from ordinary gauze when harvested immediately after exposure (E. coli, 3.06 vs 1.73; A. baumannii, 3.13 vs 1.98; P<0.001). After 24 h incubation of exposed gauze, silver-coated gauze produced less than 1 CFU/mL, whereas ordinary gauze produced a number of colonies significantly higher than it did immediately after exposure (E. coli, 4.13; A. baumannii, 4.46; P<0.001). Conclusion: Compared with ordinary gauze, silver-coated gauze was shown to have 99.99% antibacterial effect.

8.Relationships of Self-esteem to Job Satisfaction and Turnover Intention among Ship-building Supply Workers.

PURPOSE: This is a descriptive correlation study to identify self-esteem, job satisfaction, and turnover intention among ship-building supply workers. METHODS: The subjects were 199 ship-building supply workers at two small & medium-sized ship-building supply companies in K city. The data were collected from March 1st through 31st, 2013. The collected data were analyzed using the SAS program through t-test, ANOVA, Scheffe test, and Pearson's correlation coefficients. RESULTS: There was a positive correlation between self-esteem and job satisfaction (r=.29, p<.001), a negative correlation between self-esteem and turnover intention (r=-.20, p<.001). Also there was a negative correlation between job satisfaction and turnover intention (r= -.58, p<.001). CONCLUSION: This finding identified that the self-esteem and job satisfaction were an important variable related to the turnover intention. Therefore, organizational managers need to develop strategies for improving workers' self-esteem and job satisfaction to prevent and decrease worker's turnover intention.

10.A study on the Surgical Treatment of Perilumar Fracture-Dislocation

We studied thirteen, all male, patients who had trans-scaphoid perilunar dislocation(stage I, 5 cases) and dislocation of lunate(stage IV, 8 cases). Age of patient was ranged from 25 years to 49 years(avr. 36 years)and the most common cause of injury was fall down(6 cases, 46%). Seven acute cases, diagnosed and treated within 3 weeks from injury, were treated by open reduction and K-wire fixation(5 cases), and K-wire fixation and bone graft(1 case) and Herbert screw for fracture of scaphoid(1 case). Six chronic cases diagnosed and treated after 3 weeks from injury were treated by open reduction and K-wire fixation(1 case), K-wire fixation and bone graft(1 case), removal of lunate(2 cases), and arthroplasty using muscle after removal of lunate(2 cases). Thirteen cases were followed up for average 33 months(from 13 months to 81 months). Grip power was evaluated as excellent or good in 8 cases and fair in 5 cases. Range of motion of wrist joint was decreased 21 degrees in flexion, 22 degrees in extension, 4 degrees in ulnar deviation, and 5 degrees in radial deviation of wrist joint, compared with uninjured side. Carpal height and carpal height ratio were decreased to 33.1 and 0.51, compared with normal 35.8(p=0.001) and 0.55(P < 0.001), respectively. Between 2 groups of acute and chronic cases, no significant differences were noted in carpal height or carpal height ratio. The surgical treatment of perilunar fracture-dislocation has not simple option, therefore, we would recommend the early diagnosis and exact anatomical reduction.
