2.Relationships between Attitude Towards Treatment, Distress, and Decision Conflict among Cancer Patients Receiving Chemotherapy

PURPOSE: The purpose of this study was to find the factors affecting the decisional conflict of patients undergoing chemotherapy.METHODS: Participants were 137 patients undergoing chemotherapy in a hospital. The Quality Quantity Questionnaire, a distress thermometer and the Decisional Conflict Scale were used to gather self-reported data. The collected data were analyzed using descriptive statistics, t-tests, ANOVA, post-hoc Scheffe's tests, Pearson's correlations coefficients and stepwise multiple regression.RESULTS: The decisional conflict showed that the score for severe to high distress groups were higher than that of the mild distress group. The levels of distress and satisfaction of chemotherapy decisional conflict were positively correlated (p=.043). Decisional conflict was significantly affected by age (p=.041), the burden of treatment costs (p=.047), the support system(p=.049), the current feeling of health(p=.024), and comorbidity (p=.039). The significant predictors of decisional conflict were support system, current feeling of health, age, and the burden of treatment costs, which explained 15% of the variance.CONCLUSION: To improve the decisional conflict patients who are receiving chemotherapy, it is deemed to be necessary to develop an intervention program, for oncology nurses to use regarding decisional conflict over cancer treatment in consideration of the variables we have identified.

4.Congenital Pseundarthrosis of the Tibia: 3 Cases Report

Three cases of congenital pseudarthrosis of the tibia were treated at Department of Orthopedic Surgery, Seoul national University Hospital during last four years. In all cases, many Cafe-au-lait spots and subcutaneous nodules were shown on the whoIe body skin, which are suggestive of neuro-fibromatosis. Case 1, A thirteen years old boy was treated with plate and screw fixation with massive iliac cancellous and cortical graft. In a follow-up of three and one half years, solid bony union has been obtained with successful result. Case 2, A 5 years old boy was treated with Boyds dual cortical graft with plate and screw fixation. In a follow-up to eight months, bony union were satisfactory. Case 3, A six months old boy who is young brother of case 2, was treated with Charnleys intramedullary nailing and iliac cancellous and cortical graft. The patient is still in the Iong leg cast, six months postoperatively. The evidence of familial and hereditary factor in pedigree is elicited on case 2 and 3. This congenital pseudarthrosis of the tibia in the sibling is regared as the first report in Korean literature.

5.Evaluation of Public Health Nutrition Education Program for High School Girls.

Impact and process evaluations were performed in order to verify the effectiveness of a public health nutrition program developed for child-bearing aged women in Korea. Participants included 58 high school girls who were divided into two groups. Each group received four 50 - 60 minute nutrition education lectures regarding healthy eating, osteoporosis, constipation and nutrition labeling in every two weeks. Each session took 50 - 60 minutes. Regarding nutrition knowledge, there was a significant increase of degree of perception (p = 0.0004), but no change in degree of accuracy after implementation (p = 0.9522). Nutrition education was also effective in attitude change, showing more participants were ready to change their eating behaviors in terms of meal regularity (p = 0.0455) and less processed food intake (p = 0.0143). After implementing nutrition education, effective behavioral changes were observed in milk consumption (p = 0.0037) and meal regularity (p = 0.0882) as well as daily activity such as stair use (p = 0.0701). However, nutrition education had no effect on body mass index and perceived health status. In process evaluation conducted by a 9 item questionnaire, grand mean score was 4.17 +/- 0.72 out of 5. Proportion of items with scores higher than 4 ranged 68-91%. These results suggest that the nutrition education program used in this study was effective and useful. For a wider use of this program, more research was recommend for a strategy development of program diffuse.

7.A Case of Palisaded Encapsulated Neuroma of the Eyelid.

Palisaded encapsulated neuroma (PEN) is a rare intraneural neuroma. It usually occurs as a solitary asymptomatic skin-colored papule, and commonly affects the butterfly area of the face of middle-aged adults. We report a case of PEN which developed on the right upper eyelid of a 30-year-old man. It was a 3 mm-sized skin colored papule, and histologically, there was a well- defined encapsulated nodule in the dermis, composed of spindle cells with basophilic and plump nuclei in a palisading pattern. On immunohistochemical staining, the tumor cells of the nodule were positive for S-100 protein, while the capsule of the nodule was positive for epithelial membrane antigen.

8.Effect of Ambulatory Chemotherapy (Portable Infusion Pump Use) Video Education on Knowledge, Self-efficacy and Anxiety of Colorectal Cancer Patients

Purpose:
The purpose of this study is to identify changes in knowledge, self-efficacy and anxiety achieved through the provision of video education on chemotherapy with a portable infusion pump to colorectal cancer patients.
Methods:
This study is a nonequivalent control group and non-synchronized design as a quasi-experimental study. Participants in this study were colorectal cancer patients who received chemotherapy with a portable infusion pump through an ambulatory care unit. For 48 subjects, the education was divided into printed materials and videos, and knowledge, self-efficacy, and anxiety were measured. Data were analyzed using independent t test, paired t-test, and Wilcoxon’s signed rank test.
Results:
In the video education group, knowledge (Z=-4.09, p<.001) and self-efficacy (Z=-2.72, p=.012) significantly increased after education, and anxiety significantly decreased after education (Z=2.24, p=.035). However, there was no difference in knowledge (t=0.09, p=.931), self-efficacy (t=1.22, p=.229), and anxiety (t=-1.16, p=.250) between the groups after education.
Conclusion
To improve the quality of life of cancer patients, it is necessary to promote self-efficacy and reduce anxiety. The results of this study suggest that more diverse educational methods should be attempted to improve knowledge and self-efficacy and reduce anxiety in colorectal cancer patients.
9.A Case of Bronchiolitis Obliterans Organizing Pneumonia with a Solitary Pulmonary Nodule in a Child.

Bronchiolitis obliterans organizing pneumonia (BOOP) is a pulmonary disorder that exhibits various radiographic findings. It often shows bilateral, patched, or ground glass interstitial infiltrate, but a solitary nodular pattern rarely appears. We report a case of an 8-year-old boy suffering from BOOP that showed a single nodular pattern of the lung in the chest radiography. We conclude that when there is a solitary nodule discovered in the lungs of children or adolescence, the differential diagnosis must include BOOP.

10.Clinical Review of Pulmonary Tuberculosis in Teenagers According to the Involved Lung.

PURPOSE: The purpose of this study was to investigate clinical features and culture-positive rates according to the involved lung in adolescent pulmonary tuberculosis (TB). METHODS: We retrospectively reviewed the medical records of adolescents who ranged in age from 10 to 20 years and who had been hospitalized with a diagnosis of TB at Kwangju Christian Hospital from 2000 to 2008. RESULTS: Sixty-six patients were identified with pulmonary TB: median age 16.82 years; 48.5% males. Among them, 90.9% of patients were between 15 and 20 years of age. Most patients presented with multiple symptoms, and the most common included cough (74.2%), sputum (60.6%), fever (39.5%), and night sweating (18.2%). Sputum samples were smear-positive in 28 (42.4%), culture-positive in 40 (60.6%), and PCR-positive in 46 (69.7%). The most common radiological patterns included cavitation in 18 (27.3%), pleural effusion in 18 (27.3%), lymphadenopathy in 10 (15.2%), and tuberculoma in 5 (7.6%). The prevalence of smear, culture, and PCR positive rates increased as the number of involved lobes increased (P<0.05, P<0.01, P<0.05). The median treatment duration was 7 months. Twelve patients (18.2%) had lower lung field TB (Group A) and forty-four patients (66.7%) had other areas involving TB, except for Group A (Group B), and ten patients (15.1%) had only TB pleurisy (Group C). The difference of clinical characteristics and culture rates between group A and group B was not significant. CONCLUSION: Pulmonary TB toward late adolescence is increasing. We need to pay more attention to lower lung field TB, which is difficult to detect with specific radiographic findings.
