1.The study on Fatigue, Pain, and Coping of Pain in Fibromyalgia and Arthritis patients.

The propose of this study was to identify fatigue, pain and coping of pain and to compare the variables between fibromyalgia and chronic arthritis. The sample consisted of 133 patients who visited H university hospital. Data were collected by questionnaire from May 1 to September 30, 1999. Data was analyzed by descriptive statistics. chi-test. pearson correlation coefficient. and ANOVA. As a results, most of all patients felt fatigue and the mean score of the fatigue was above average. The mean score of rheumatoid arthritis and fibromyalgia patients on pain was higher than Osteoarthritis patients. and there was the statistically significant difference among three groups on pain(F=10.63. p=0.00). There was also the statistical difference among three groups on coping of pain(F=4.74, p=0.01). The mean score of rheumatoid arthritis and fibromyalgia patients on coping of pain was higher than Osteoarthritis patients. Fatigue showed positive relationship with pain(r= .262, p= .002). and pain showed positive relationship with coping of pain(r=.319. p=.000). According to this finding, fibromyalgia patients and rheumatoid arthritis patients felt high fatigue and pain, therefore the development of nursing intervention for relieving fatigue and pain would be needed.

2.The Lack of a Direct Effect of Tumor Necrosis Factor-Alpha on Sperm Motility.

Male genital tract inflammatory conditions may be associated with unexplained infertility. The presence of cytokine such as tumor necrosis factor-alpha (TNF-alpha) was reported in the semen of infertile men. However, the effect of these cytokines on human sperm function is still unclear. The purpose of this study was to investigate the in-vitro effects of TNF-alpha on human sperm motility with computer assisted sperm analysis. Washed sperm from 16 normal men were incubated without and with TNF-alpha(0.1, 10, 1000 ng/ml). The changes of parameters of sperm motility were recorded at different time intervals (0, 5, 24 hour). There was no significant change of parameters of sperm motility in the incubation with TNF-alpha. It is suggested that TNF-alpha alone does not interfere with the sperm motility and more studies are needed.

3.Virulence Factors and Genotyping of Enterotoxigenic Escherichia coli O128 Isolates from Clinical Specimens.

Sixteen strains of LT-producing enterotoxigenic E. coli 0128 which were isolated from diarrheal patient's stool in Pusan University Hospital, were serotyped and analyzed for plasmid DNA profile, MRHA of human blood cells, and also tested for possession of LT, ST, aggA, EAST1 genes by the PCR method and analyzed the RAPD pattern. Screening sensitivity for ETEC by salting out test was 87.5%. These data suggest that hydrophobicity test using salting out is rapid, inexpensive, and simple screening test for ETEC. CFAs were identified in 87.5% of strains; 43.75% the strains harbored CFA/I, 43.75% CFA/II, and 12.5% expressed none of these CFAs. For plasmid profiles, 12 strains had 60 MDa plasmid and several smaller plasmids. The strains showed 5 types of plasmid profiles. By PCR, LT gene but not ST gene was detected from all of the 16 strains EAST1 gene was detected from 14 strains. Ten strains could be differentiated to 3 patterns by chromosomal DNA fingerprint. The chromosomal DNA fingerprinting is considered very useful for the epidemiological study.

4.Adenocarcinoma Arising in Adenomyosis.

Adenocarcinoma in adenomyosis is unusual and it is mostly associated with adenocarcinoma in the endometrial mucosa. In contrast, adenocarcinoma arising in adenomyosis without endometrial adenocarcinoma is extremely rare and it suggests that it arises de novo from adenomyosis. We report a case of adenocarcinoma arising in adenomyosis in 44-year-old woman. The endometrial cavity contained a polypoid lesion with atypical hyperplasia, but no evidence of adenocarcinoma in the endometrial mucosa. Simple, complex and atypical hyperplasia associated with well differentiated adenocarcinoma was also noted in the areas of adenomyosis.

5.A Case of Dyke-Davidoff-Masson Syndrome with Infantile Spasm.

Dyke-Davidoff-Masson syndrome is a rare clinical syndrome consisting of hemiatrophy of cerebral hemisphere, enlargement of ventricles and prominent pneumatization of petrous pyramid of the temporal bone. Its clinical manifestations are hemiparesis, several types of seizure, mental retardation and other neurologic deficits. We experienced a case of Dyke-Davidoff-Masson syndrome in a four months old boy who visited to our department due to right side upper extremity weakness and infantile spasm. The brain magnetic resonance imaging showed marked hemiatrophy of left cerebral hemisphere and enlargement of left lateral ventricle. So with the review of literatures, we report a case of Dyke-Davidoff-Masson syndrome with infantile spasm.

7.A Case of Septo-Optic Dysplasia.

Septo-optic dysplasia, as first described by de Morsier, is a rare developmental anomaly of absent or hypoplastic septum pellucidum, a primitive optic vesicle, and dysplasia of the optic nerve, chiasm, and optic tracts. It is a common cause of hypopituitarism in children and has variable hypothalamic-pituitary dysfunction. We experienced a case of septo-optic dysplasia in 5 month old female baby who presented with developmental delay and seizure. A magnetic resonance imaging of the brain showed absence of the septum pellucidum and the fundoscopic examination disclosed the absence of left optic disc and atrophy of right optic disc.

8.Blood Folate Level Determined by a Microplate Reader and Folate Intake Measured by a Weighted Food Record.

Microbiological method using a 96-well microplate reader for folate assay was established, and folate intake and blood folate concentrations of 23 female college students were assessed. To evaluate folate intake, dietary data were collected by a 3-day weight food record, and serum and RBC folate concentrations were measured by the new method. The coefficient of variation for the new method was less than 10%. Mean daily folate intake of the subjects was 126.7ug which is only 50.7% of the RDA. The mean concentrations of serum and RBC folate were 7.46ng/ml and 294.4ng/ml, respectively, which were within the normal range. These results indicate that folate intake seems to be underestimated due to incomplete food composition database. Therefore, folate database should be appropriately in order to asses folate intake accurately.

9.A Comparison Study on Fatigue and Pain in Rheumatoid Patients - centered on AS, FM, and SLE patients.

This study was designed to offer descriptive data for nursing intervention for relief of fatigue and pain, and to distinguish by the characteristic difference and the symptoms such as fatigue and pain on Ankylosing Spondylitis (AS), Fibromyalgia(FM), and Systemic Lupus Erythematosus(SLE) patients. The sample consisted of 92 patients(AS 29; FM 30; SLE 33) who visited H-University Rheumatism Hospital in Seoul. The data were collected by a structured questionnaire from May 1, 1999 to April 30, 2000. The results were as follows: Patients of 95% experienced fatigue in the last week and a fatigue score of three disease groups were above average. The fatigue score of FM patients was highest in the other disease, but which was not a statistically significant difference(F=1.417, p=.248). The mean score of AS and FM patients in pain was higher than the SLE patients, and there was the statistical significance among the three groups on pain (F=8.239, p=.001). There wasn't a statistical difference among three groups on coping wtih pain(F=1.451, p=.240). There wasn't any correlation between fatigue and pain in each disease (AS: r=.008, p=.966; FM: r=.328, p=.077; SLE: r=.237,p=.185). Therefore, morning stiffness and pain management during sleeping is needed through good body alignment in the AS patients. Adequate rest for fatigue and multiple coping strategies for pain maybe basic nursing intervention in FM and SLE. According to their fatigue rhythm, a regular exercise program is needed for rheumatic disease because they complained of fatigue above average and their fatigue was repeated better and worse only during the one week.

10.Cytomegalovirus Retinopathy in Aequired Immunodeficieney Syndrome.

Cytomegalovirus retinopathy(CMV)is a necrotizing retinitis which can occur as a late manifestation of the Acquired immunodeficiency syndromde(AIDS), which rapidly increases in number after original description of the disease in 1981 by Gottliebet al. We experienced a patient, who complained of decreased visual acuity of the left eye only we initially diagnosed the patient as CMV retinitis, and during the course of treatment, finally diagnosed the patient as CMV retinitis with AIDS, and the patient was subdivided into Group IV, Subgroup C by center for disease control classification system To our knowledge, for about 10% of the patient of the CMV retinopathy, AIDS was diagnosed like same manner as we did, but we couldn't find any report in korea, so we report this case with a review of literatures.
