PURPOSE: This study was performed to identify the meaning of life experience following suicide attempt among middle-aged men. METHODS: A qualitative research design was adopted using van Manen's hermeneutic phenomenological approach. The participants were six middle-aged men who had attempted suicide at least one time. Data were collected in 2013 through in-depth interviews. Individual interviews were recorded; and literary, art works and phenomenological literature were searched to identify the meaning of the experience. RESULTS: The five essential themes of the life experience of middle-aged men who attempted suicide were 'Bitter reality confronted again', 'Anger buried deep inside', 'Broken family, inescapable fetters', 'Blocked relationships, closed world' and 'A step towards a new life'. CONCLUSION: The meaning of lived experience found in this study provides deep insight into the experience following suicide attempt in middle-aged men and crucial information to give directions to appropriate support and nursing interventions.

PURPOSE: This study aims to explore the relationships between health-related behavioral and psychological factors and cardiovascular and cerebrovascular diseases (CCVD) comorbidities among Korean adults with diabetes mellitus (DM). METHODS: Data included in the Fourth Korean National Health and Nutrition Examination Survey were used. This study compared three groups: those diagnosed with DM only, DM and hypertension, DM, hypertension and CCVD using multinomial logistic regression analyses and the classification and regression tree (CART) model. RESULTS: Weight control (OR = 4.01) and depression (OR = 2.37) are related with increased odds of having hypertension and CCVD comorbidity in those with DM. The CART model suggested that the high prevalence risk groups for hypertension or CCVD comorbidities were diabetic adults aged between 51 and 69 with a body mass index of 25 and above and those aged 70 and above. CONCLUSION: For effective control of CCVD comorbidities among diabetic Korean adults, psychological support for depression and weight control need to be prioritized when managing DM. Weight control intervention needs to be reinforced for DM patients aged between 51 and 69 and that even if BMI is below 25, the occurrence of comorbidities needs to be carefully monitored for DM patients aged 70 or older.

Malignant acanthosis nigricans is very rare disease. The disease is related with internal malignancy, especially gastrointestinal tract malignancy. Reported occurrence rate of malignant acanthosis nigricans associated internal malignancies are 64% of stomach adenocarcinoma, 27% of other organs of abdomen (liver, gall bladder, small intestine, rectum, uterus, ovary), 9% of non-abdominal organs(breast, lung). We report a case of malignant acanthosis nigricans associated with stomach adenocarcinoma from 20-year-old female and discuss review of literature on a acanthosis nigricans briefly.

To compare the features of stroke associated with nonvalvular and valvular atrial fibri!lation, we carried out a retrospective study on cerebral infarcts in 27 patients with nonvalvular atrial fibrillation (AF) and in 33 patients with valvular AF.Iwe analyzed the clinical courses, the underlying heart diseases, and the features of echocardiography and brain cornputed tomograpy, Thefollowing results were obtained; 1) Most strokes in nonvalvular AF occurred after 7th decade and the age of stroke onset in nonvalvular group was younger than that of nonvalvular group(p<.05).; 2) Echocardiographic studiesshowed that the left atriurn of the heart in the nonvalvular AF group was srnaller in size than the valvular group and that the left atrial thrombi were not detectable on conventional echoardiograrn in the nonvalvular irouP.: 3) Both groups showed sirnilar distribution in the sites, number, and nature of the brain lesions.; 4) Stroke recurred with sirnilar frequency in both groups and rnost of them occurred during the first year of the initial stroke.

For lumbar disc herniation, conventional laminectomy and discectomy were used for a long time. Since chemonucleolysis was introduced as a treatment modality of lumbar disc herniation in 1964, other various minimally invasive surgical techniques like Percutaneous Nucleotome, Arthorscopic Microdiscectomy and laser discectomy were developed to minimize postoperative discomfort and morbidity. In conventional surgery with laminectomy, there is no limitation for surgical procedures, and the surgical result is very excellent over 90%. But it causes a postoperative wound pain due to muscle injury and a possibility of nerve root injury. Minimally invasive surgical techniques has advantages of minimal postoperative pain and morbidity. Also it has limitations of narrow indication like soft disc herniation without bony stenosis and comparatively low surgical result about 70%. Recently a new minimal invasive endoscopic discectomy system was developed which has advantages of conventional laminectomy and minimally invasive surgical techniques. It can decompress the nerve root like conventional open surgery and has the benefits of small incision, limited tissue disruption and faster recovery time. We have had 3 cases experiences of MicroEndoscopic Discectomy(MED) system in lumbar disc herniations and got excellent clinical result.
