We report a case of giant cell tumor of the tendon sheath involving the foot, in a 21-year-old female patient, who presented with an asymptomatic tumor on the dorsum of her right foot. Histopathologically the tumor is surrounded by thin fibrous connective tissue. The characteristic findings of the excised specimen revealed a typical mixture of abundunt round or polygonal histocyte-like cells with varying portions of spindle-shaped fibroblast-like cells and multinucleated giant cells. Variable amounts of hyalinized fibrous stromal tissue were also present. After excision, local recurrence has not been observed for 3 months.

Bcl-2 oncoprotein is being localized to mitochondria and interfering with programmed cell death (apoptosis) independent of promoting cell division in the lymphoid and nonlymphoid cells. The bcl-2 oncoprotein expression has been reported in follicular lymphomas as well as in diffuse non-Hodgkin's lymphoma, leukemia and a variable propotion of Hodgkin's lymphoma cases. Recent evidence suggests that some lymphomas protected from apoptosis is conferred through expression of Epstein-Barr virus(EBV) latent membrane protein which turn to cause upregulation of bcl-2. To define the role of the bcl-2 oncoprotein and EBV in lymphoid malignancy, we tried immunohistochemical studies with anti-bcl-2 antibody and In situ hybridization (ISH) with EBV-encoded small nuclear RNAs(EBER) in the paraffin embedded sections of 46 non-Hodgkin's lymphoma (NHL) cases and 20 Hodgkin's lymphoma (HL) cases. Bcl-2 oncoprotein expression was found in 37 of 46 cases (80%) of NHL with relatively strong cytoplasmic staining, and in 14 of 20 cases (70%) of HL with weak cytoplasmic staining in limited small numbers of RS, Hodgkin and lacunar cells. The widespread presence of bcl-2 oncogene in many different types of both NHL and HL supports that the extended cell survival through overexpression of bcl-2 gene protein may be a growth advantage of neoplastic lymphoid cells. In the ISH analysis for EBV, the presence of EBV was detected in 17 of 20 cases (85%) of HL, compared to 6 of 44 cases(13.6%) of NHL. It appears to be no direct correlation between overexpression of bcl-2 oncoprotein by neoplastic lymphoid cells and the presence of EBV in NHL but it seems to be a definite association between EBV and HL.

OBJECTIVES: Despite the accumulating evidence of the effectiveness of positive airway pressure (PAP) therapy in obstructive sleep apnea (OSA) syndrome, adherence to PAP therapy is not high. Several factors reportedly affect PAP adherence; however, it remains unclear whether patients' symptoms were detrimental to adherence rate. This study is aimed at investigating the relationship between insomnia symptoms and adherence. METHODS: Retrospective analyses were performed in 359 patients with OSA (mean age 58.4±13.2 years; females, n=80). Logistic regression analyses were performed between PAP adherence with clinical factors and questionnaires, such as Epworth Sleepiness Scale, Insomnia Severity Index, and Beck Depression Inventory (BDI). RESULTS: PAP adherence was defined as the use of PAP for ≥4 h per night on 70% of nights during 30 consecutive days. The median follow-up time was 55 days (interquartile range, 30–119 days), and 54.3% showed poor adherence. Non-adherent patients showed more severe sleep onset insomnia, higher BDI, and higher nadir oxygen saturation (SaO₂). Patients with good adherence had higher apnea–hypopnea index, oxygen desaturation index, and respiratory arousal to total arousal ratio. Sleep onset insomnia [odds ratio (OR)=1.792, p=0.012], BDI (OR = 1.055, p=0.026), and nadir SaO₂ (OR=1.043, p=0.040) were independently associated with PAP non-adherence. CONCLUSIONS: Not the severity of insomnia but sleep onset insomnia was associated with PAP adherence, as well as depressive mood. It suggests that different interventions for reducing insomnia and depressive mood are needed to increase PAP adherence in patients with OSA.

OBJECTIVES: Night shift workers suffer from sleep and daytime disturbances due to circadian misalignment. To investigate the role of environmental light in daytime sleep following 12 h-night shift work. METHODS: We enrolled 12 h-shift female nurses working at one university-affiliated hospital (n=10, mean age 26.6 years, shift work duration 3.8 years). This is a cross-over study to compare sleep between under light exposure (30 lux) and in the dark (<5 lux) following 12 h-night duty. Two sessions of experiments were underwent and the interval between sessions was about a month. Psychomotor vigilance test (PVT) had performed on awakening from sleep at each session and sleep-wake pattern had been monitored by actigraphy throughout the study period. Daytime sleep was also compared with night sleep of age-and gender matched daytime workers (n=10). RESULTS: Sleep parameters and PVT scores were not different between two light conditions. Activities during sleep seemed to be more abundant under 30 lux condition than in the dark, which was not significant. Compared to night sleep, daytime sleep of shift workers was different in terms of rapid eye movement (REM) sleep. Three shift workers showed sleep onset REM sleep and first REM sleep period was the longest during daytime sleep. CONCLUSIONS: Unexpectedly, daytime sleep of 12 h night shift workers was well-maintained regardless of light exposure. Early occurrence of REM sleep and shorter sleep latency during daytime sleep suggest that shift workers meet with misalignment of circadian rhythm as well as increased homeostatic sleep pressure drive.
