OBJECTIVES: The aim of this study was to identify the relationship between serum gamma-glutamyltransferase (GGT) and bone mineral density (BMD) in postmenopausal women. METHODS: We evaluated 200 postmenopausal women who were visiting a health promotion center at a university hospital from January 2009 to December 2011. Their current medical diseases and medication history were collected through medical records. Basic physical examinations and laboratory tests were performed on all subjects. RESULTS: The levels of serum GGT within their normal range were positively correlated with waist circumference (P = 0.01), triglycerides (P <0.001), alkaline phosphatase (P = 0.009), and uric acid (P = 0.01). The serum GGT within their normal range were negatively associated with the femur neck BMD (P = 0.002). In adjusted analysis including age and body mass index, the BMD of the femur neck was more strongly associated with a high-normal serum GGT level among the postmenopausal women as compared with those with a low-normal serum GGT level (P = 0.02). CONCLUSIONS: Serum GGT within its normal range is negatively correlated with the BMD in the femur neck among postmenopausal women. It can be useful for selecting a group that is at high risk for the bone fracture regardless of the underlying mechanism.

Pulmonary histiocytosis X is a rare granulomatous disorder of unknown etiology that alters the interstitium of the lung. When confined to the lung, it is known as primary pulmonary histiocytosis X or eosinophilic granuloma of the lung. The chest radiograph shows characteristic nodular, reticular, and cystic abnormalities, which are most apparent in the upper and middle lung zones, but spare the costophrenic angles, The CT demonstrates innumerable small cysts with thin walls, and fine nodules. Recently we experienced pathologically proven primary pulmonary histiocystosis X in 35 years old male patients who had recurrent pneumothorax.

The time course of recovery in vestibular neuritis varies between individuals. The aim of this study was to identify the predictors for the early or late recovery of vestibular neuritis. The inclusion criteria were patients 1) who had an acute onset of vertigo lasting at least 24 hours, 2) with a horizontal-torsional unidirectional spontaneous nystagmus, and 3) with a canal paresis of 20% or more on the bithermal caloric tests. The primary endpoint for this study was an early or late recovery of vestibular neuritis as a dependent variable. A functional level scale was used to define the late recovery (5 or more points) at seven days after the symptom onset. The secondary endpoint was the duration of hospitalization. One hundred twenty eight patients met the inclusion criteria for this study, and among them, 71 patients had an early recovery. Multiple logistic regression analysis showed that diabetes mellitus was the only independent significant variable for the prediction of a late recovery of vestibular neuritis. In addition, the diabetes mellitus was a predicting variable for long duration of hospitalization. Diabetes mellitus was a predictor for a late recovery of vestibular neuritis.

PURPOSE: To compare the clinical characteristics and laboratory finding between adenoviral and group A streptococcal (GAS) pharyngitis. METHODS: A retrospective review of medical records was performed in the patients with adenovirus infection among those who were admitted for febrile respiratory disease from January 2011 to July 2013 and GAS pharyngitis among those who visited for symptoms of scarlet fever from August 2006 to July 2013. RESULTS: 179 patients (AV1 group) were diagnosed with adenoviral pharyngitis and 37 (AV2 group) of these patients had adenovirus single infection. 26 patients (GAS group) were diagnosed with scarlet fever. Adenoviral infection (AV2 group) developed in younger patients compared to GAS group (2.8+/-2.1 years vs. 5.4+/-1.8 years, P=0.000). Total durations of fever and admission were longer in AV2 (6.3+/-2.6 days vs. 3.3+/-1.9 days, P=0.000; 4.1+/-1.2 days vs. 1.9+/-1.8 days, P=0.000, respectively). WBC counts were higher in AV2 (11,449+/-5,680 cells/mm2 vs. 6,722+/-6,941 cells/mm2, P=0.000). CRP was not significantly different between AV2 and GAS group (3.8+/-3.2 mg/dL vs. 5.2+/-5.1 mg/dL, P=0.368). No difference was found between two groups in the percentage of antibiotics use (91.9% vs. 100%, P=0.261). CONCLUSION: Clinical characteristics and measures of inflammation in the laboratory findings were similar between adenoviral and GAS pharyngitis group. It is necessary to conduct the test for respiratory virus and bacteria in early stage to differentiate in the pharyngitis patients with leukocytosis and elevation of CRP level.

BACKGROUND: The Korean Shoulder Scoring System (KSS) is a reliable and valid procedure for discriminative assessment of the clinical status of patients with rotator cuff tears. This study evaluates the correlation between the preoperative KSS and factors in patients with rotator cuff tears. METHODS: From November 2009 to June 2016, 970 patients who underwent arthroscopic rotator cuff repair were retrospectively evaluated. A total of 490 patients met the study criteria. Preoperative factors included age, sex, symptom duration, mediolateral (ML) and anteroposterior (AP) tear size, acromiohumeral distance (AHD), tangent sign, tendon involvement (type I, supraspinatus; type II, supraspinatus and subscapularis; type III, supraspinatus and infraspinatus; type IV, all 3 tendons), fatty infiltration of rotator cuff muscles (group I, Goutallier stages 0 and 1; group II, Goutallier stages 2, 3, and 4), and KSS. RESULTS: Old age, ML tear size, and AP tear size negatively correlated with the preoperative KSS (p < 0.001). AHD showed a positive correlation with the preoperative KSS (p < 0.001). A significantly inferior preoperative KSS was found in females and type III tendon involvement (p < 0.001). For supraspinatus and infraspinatus, the preoperative KSS of group II fatty infiltration showed a significantly lower score than group I fatty infiltration (p < 0.05). CONCLUSIONS: A relatively lower preoperative KSS was associated with old age, large tear size, narrow AHD, female, type III tendon involvement, and group II fatty infiltration of the supraspinatus and infraspinatus. Our study indicates that preoperative KSS can be a good measurement for the preoperative status of patients with rotator cuff tears.

BACKGROUND: The most of typical seborrheic keratoses (SKs) can be clinically differentiated with ease from other dermatoses. However, its differential diagnosis can sometimes be difficult and recently reported cases clinically suggest that SKs with the skin biopsy result being some other diseases can bring on the need for further evaluations. OBJECTIVE: The aim of this study is to examine the diagnostic yield in submitting clinically diagnosed SKs which showed single discoid lesions of the face for routine microscopic examinations. METHODS: We conducted a retrospective review of cases for histological examinations with a clinical diagnosis of SKs or with a differential diagnosis that includes SKs which shows single discoid lesion of the face. A total of 94 pathology reports containing a diagnosis of SKs in the clinical information field were identified. The accuracy of the clinical diagnosis and clinic histopathological correlations were studied. RESULTS: In this study, 45.7% of patients was clinically assumed to have SKs but turned out to be some other diseases according to the histopathology test results - a total of 19 cases (44.2%) with some considered to be precancerous (actinic keratosis 25.6%, benign lichenoid keratosis 16.3% or cancerous (basal cell carcinoma 9.3%, squamous cell carcinoma 4.7%). CONCLUSION: Lesions suggested to be SKs need more examinations and lesions of atypical clinical appearances which cannot completely exclude the possibility of precancerous or malignant lesions are strongly recommended for skin biopsies.

Blue nevus is a neoplasm composed of pigmented dendritic dermal melanocytic cells in the reticular dermis. The lesion clinically presents as a well dermacated cerulean-blue or bluish black colored papule or plaque that usually measure less than 1 cm in diameter. Pilar sheath acanthoma is a rare, benign follicular hamartoma commonly affecting middle-aged and elderly individuals. It is characterized by a small, solitary, skin colored papule situated on the head and neck, particularly around the upper lip. We report a rare case of a blue nevus associated with a pilar sheath acanthoma in a 26 year-old man presented with a bluish papule on the neck.

Lichen nitidus (LN) is an uncommon, usually asymptomatic cutaneous eruption characterized by the presence of multiple, small, flesh-colored papules. The epidemiologic and pathophysiologic characteristics of LN have not yet been defined. Furthermore, LN has rarely been described in association with other cutaneous diseases. We herein report 3 cases of LN associated with various cutaneous diseases, including lichen striatus, oral lichen planus, and psoriasis vulgaris.
