1.A Study on Quantitative Analysis of Immune Globulin in Sera and Joint Fluids of Chronic Synovitis Patients

In several pathologic conditions including rheumatoid arthritis and degenerative arthritis, the joint swelling is the main manifestation as a local reaction of systemic diseases. A quantitative analysis of Ig G, Ig A, and Ig M in sera and joint fluids of such patients is attempted for the purpose of evaluating the significance of its clinical application. Also, the significance of the change in immune globulin level is evaluated statistically. 1) Joint fluids were aspirated from 74 patients who visited Seoul National University Hospital with a chief complaintt of joint swelling. (from April, 1968 to October, 1970) Sera wera taken from 55 cases. 2) In detalis of joint fluid aspirants, 23 patients were rheumatoid arthritis, 25 were degenerative arthritis and 26 were involved in traumatic arthritis. 3) In sera rheumatoid arthritis patients were 13, degenerative arthritis were 18, and 24 cases were traumatic arthritis. 4) The level of Ig A of jnint fluid is about one third of that of serum and the Ig G and Ig M level of joint fluid are approximately a half of serum respectively. 5) In rheumatoid arthritis cases, Ig A level in serum and Ig G level of joint fluid are increased significantly but, in general, there is a tendecy of increment of the other fractions except the level of Ig M in joint fluid. 6) In degenerative arthritis, the levels of lg 6 in serum and in joint fluid are elevated, and Ig M value in serum is decreased significantly. 7) The serum values of Ig G, Ig A, Ig M of traumatic arthritis cases are almost same as normal cases. 8) In joint fluids of traumatic arthritis cases, Ig G value is 950.6±272.2 mg/100ml, Ig A is 111.8 ±19.7 mg/ml, and IgM is 44.5±8.9 mg/100ml. The levels of immune globulins in joint fluids of traumatic arthnitis cases are used as control data, because of its nortorious difficulty in obtaining joint fluid from healthy persons.

2.Radiological Evaluation of Posterior Cruciate Ligament Injury

The evaluation of posterior cruciate ligament injury of the knee should be objective. Then stress machine, Gonylaxometer, knee ligament arthrometer (K-T 1000) and Genucome were introduced recently, but they have many problems such as soft tissue error, expensiveness and low popularity. Authors modified the radiological assessment of anterior cruciate ligament injury, reported by Hooper(1986), and applied to the evaluation of posterior cruciate ligament injury. The patient was positioned supine with the one side of leg held on a support so that the knee was flexed approximately 45°to 60°. The thigh was unsupported and a weight(0.9 Kg to 4.5 Kg with increment of 0.9 Kg) was placed on the anterior leg (just distal to the patellar tendon) and lateral view had been taken in this stressed position. Same procedure was done on the other side and the posterior displacement of both knees were compaired. The differences of posterior displacement between both knees of 50 normal adults were studied and 9 cases of posterior cruciate ligament injury were studied, too. The following results were obtained. 1. The difference of posterior displacement could be measured objectively and it is useful for the diagnosis of posterior cruciate ligament injury and the objective evaluation after conservative or operative treatment of posterior cruciate ligament injury patient. 2. The differences of posterior displacement between both knees of 50 normal adults were ranged from 0 to 3 mm with a mean of from 0.88 ±0.87mm to 1.14 ±0.94mm. 3. The differences of posterior displacement between injured knee and normal knee of posterior cruciate ligament injury patient were more than 6mm in 9 patients. 4. The radiological method was inexpensive and easy to perform.

4.The Therapeutic Effects of Piroxicam in Acute Musculoskeletal Lesions

Total 76 patients with acute painful musculoskeletal problems were treated with piroxicam, newly developed non steroidal-anti-inflammatory drug. Patients were divided into three groups, i.e. 1) acute low back strain or sprain group, 2) acute bursitis group, and 3) acute muscle sprain or strain group. The results were as following. 1. The effect of piroxicam on the various inflammatory changes was noticed promptly with initial 40 mg dose and pain control action was most prominent. 2. The drug was very well tolerated and side effect were generally mild. Three gastrointestinal symptoms and one skin eruption were developed. Over-all results of the piroxicam on the patient were evaluated as excellent in 51.3%, and good in 38.21%.

5.Radiological Evaluation of Posterior Cruciate Ligament Injury at Quadriceps Neutral angle

A method for exact evaluation of ligament integrity of the knee joint would be of clinical importance from the diagnostic point of view as well as from the desire to introduce objective measurement of stability either pre-and postoperatively, or before and after conservative treatment. Assessment of the posterior cruciate injury has been considered by numerous authors over the past decades. Even stress machine, such as Gonylaxometer, knee ligament arthrometer(KT-l000) and Genucome, have many problems such as soft tissue error, expansiveness and low popularity. Generally, orthopedic surgeons use passive tests, in which the displacing force is applied externally, to evaluate the integrity of the ligaments of the knee. Posterior tibial displacement of knee was measured at quadriceps neutral angle using a specially designed frame which minimized the rotational error to negate the quadriceps pulling effect. Posterior displacement of both sides of knee was measured in 15 patients who had posterior cruciate ligament injury and 40 adults who had normal knee. The following results were obtained. l. In the normal knee, the mean score of posterior tibial displacement was 3.0875mm in left, 2.8125mm in right each other and there was a mean of 1.05mm posterior displacement of tibia compared to the contralateral knee(range: 0–2.5mm). 2. More than 6mm displacement of lateral femoral condyle compared to the uninjured knee suggested posterior cruciate ligament injury. 3. This radiological measurement seems very efficient in detecting the posterior cruciate ligament instability of knee because of its simplicity, inexpensiveness and easy reproducibility with minimal error.

7.The Measurement of the Soft Tissue Pressure beneath a Tourniquet in the Arm

The use of a pneumatic tourniquet is potentially associated with injury to underlying muscles, vessels, and nerves if excessive pressure occurs beneath the toumiquet. In order to minimize the risk of soft tissue injury, the lowest tourniquet pressure that maintains a bloodless operative field should be used. A clinical study was undertaken to evaluate the pneumatic tourniquet setting required for adequate hemostasis in upper extremity surgery. From March to September in 1984, the subcutaneous soft tissue pressure of the 20 upper extremities beneath a pneumatic toumiquet in the arm were measured directly and the following results were obtained. 1. The underlying subcutaneous soft tissue pressure was not affected by adult, age, arm circumference, and blood pressure of normal range. 2. The underlying subcutaneous soft tissue pressure showed direct correlation with the tourniquet pressure respectively. 3. A tourniquet preasure of more than 250 mmHg was not rarely required in a normotensive individual.

8.Reconstruction of Anterior Cruciate Ligament with Iliotibial Band Transfer

Ligamentous injury of the knee which could be easily occur in sports or traffic accidents resulted in more and severe disability than that of other joints. Rupture of the anterior cruciate ligament which restrain hyperextension, excessive rotation and anterior motion of tibia on femur is the most frequent and significant injury of the knee. Its treatment and clinical significance is one of the most controversial issue and its incidence tends to increase. From 1983 to 1985, modified Scott operation were performed on 10 patients at the department of orthopedic surgery, Chung Ang university hospital. Following results are obtained. 1. No limitation of range of motion of the knee was developed. 2. Intra-articular fixation by use of pull-out wire was done and special consideration was done for the site of fixation on tibia plateau and method of dilatation of posterior capsule. 3. Of the ten patients, nine had negative anterior drawer test, postoperatively.

The introduction of the strong clover-leafed nail combined with the principle of reaming out the medullary canal and the development of image intensifiers has been extended the indications of the in tramedullary nailing. In this report, an attempt was made to introduce the method and to verify the indications and advantage of retrograde intramedullary nailing of humeral shaft fractures, as compared with the conventionally accepted methods.
