1.Treatment of Delayed or Nonunion of Humeral Shaft Fractures

For the nonunion of humeral shaft, there have been many methods of treatment. We are aimed to analyse the causes of nonunion of humeral shaft fracture and present the direction of treatment. We reviewed total 21 cases which were diagnosed as delayed or nonunion of humeral shaft and analysed the causes. l. Initial method of treatment was surgical in 19 out of 21 cases. Among these 19 cases, 14 cases were operated with internal fixation with plate and screw. 2. The most common cause of nonunion was inadequate internal fixation in 26 cases(81%) in which were unstable fixation in 13 cases, choice of inadequate internal fixator in 11 cases, and failure of operative technique in 2 cases. Other causes were distraction between fracture fragments in 4 cases(13%) and open comminuted fracture in 2 cases. 3. Eighteen cases of established nonunion due to inadequate internal fixation were treated by rigid fixation with longer and broader plate and bone graft, and 1 case interlocking IM nailing, 1 case Ender nailing and 1 case bone graft only. 4. Union was obtained in all cases at least in 5 months. And there were no specific complications. In conclusion, surgeons should contemplate the operative indication and principles in primary treatment. In treatment of nonunion, surgeons should treat by more longer and broder internal fixator and additional bone graft.

2.In vitro and In vivo Osteogenic Induction of Mesenchymal Stem Cells from Rabbit Bone Marrow.

PURPOSE: The aim of this study was to analyze the osteogenic effect of cultured rabbit mesenchymal stem cells(MSCs). MATERIALS AND METHODS: MSCs were obtained from rabbit femur and were cultured in a Dulbecco's Modified Eagle's Medium(DMEM) with beta-glycerophosphatate, L-ascorbic acid, and dexamethasone to proliferate and differentiate into osteoprogenitor cells until 12 weeks. The expression of osteogenic markers was detected by reverse transcription-polymerase chain reaction(RT-PCR) and the release of osteocalcin was measured by Enzyme-linked Immunosorbent Assay(ELISA). MSCs that were cultured on the porous poly-L-lactic-co-glycolic acid(PLGA) scaffold were implanted into athymic nude mouse to observe the osteogenic activity. RESULTS: As the time, we observed osteoblastic-like cells on the culture flask. Mineralized nodules were observed at 3-4 weeks. Osteogenic markers such as osteopontin, osteonectin, type I collagen, and alkaline phophatase were all identified at 2 weeks. But, expression of osteocalcin was only detected after cells differentiation. The amount of osteocalcin which is a specific protein in osteoblast, increased gradually from 2 weeks until 7 weeks. Scanning electron microscopy revealed that the MSCs were well adhered and proliferated within the PLGA scaffold. Immature bone was identified after 10 weeks in the histological examination of transplanted cell-scaffold composit. CONCLUSION: Our results demonstrate gradual differentiation of MSCs into osteoblastic cells. The adhesion and proliferation of the cells within the biodegradable scaffold represents the possibility of bone formation using cell-scaffold composites.

4.A Case of Hyperlipoproteinemia Type II b with Diabetes Mellitus.

A 32-year-old male patient with uncontrolled diabetes mellitus was affected with hyperlipoproteinemia type II His skin lesion showed yellowish papulsr tuberoruptive xanthoma on forearm, shoulder, elbow and knee. Besides skin eruption, he showed abnormal liver function test and right bundle branch block on EKG. The laboratory examinations revealed increase of serum cholesterol, triglycerides. and fasting blood sugar, and slight turbid color of fasting blood serum which had been kept standing at 4C for 24 hrs. On the agarose electrophoresis, bands of different densities of LDL and VLDL in beta and pre-beta position Were noted. Authors discussed here about laboratory characteristics, clinical manifestations and differential diagnosis of hyperlipoproteinemia type II and III.

5.A Statistical Study of New Outpatients During the Rescent 5 Years (1976~1980).

The statistical study of new outpatients of skin problems seen in dermatologic department of Seoul Red Cross Hospital from l976 to 1980 was done and the 10 most common dermatoses were analysed in detail, The results were summarized as follows: 1) Of 19,679 outpatients, males were 10,530(53.5%) and females were 9,149(46.5%). The sex ratio of male to female was 1. 15: l. 2) The most common dermatoses were atopic dermatitis/eczema(16. 9%), fungal infection(16. 2%), contact dermatitis(15. 3%), scabies(10. 7%), urticaria(10. 6%), acne(9. 5%), pyoderma(8. 3%), insect bite(5. 6%), syphilis(4. 1%) and psoriasia (2. 8%). 3) Of 10 most common dermatoses, contact dermatitis, acne, scabies and urticaria were revealed to have a tendency to increase year by year, syphilis and psoriasis were constant pattern and other common dermatoses showed irregular pattern in their incidence. 4) The sex ratio of male to female of l0 most common dermatoses was l. 23: l. Fungal infection, scabies, pyoderma, insect bite, syphilis and psoriasis wereoutnumbered in male and atopic dermatitis/eczema, contact dermatitis, urticaria and acne were outnumbered in female. 5) Atopic dermatitis/eczema & scabies were prevalent in winter, acne, fungal infection, urticaria and pyoderma in summer, and contact dermatitis in spring and summer. 6) The most frequent age groups of atopic dermatitis/eczema were 1 to 10 years, acne and fungal infection, 11 to 30 years, and contact dermatitis and fungal infection, 31 to 50 years.

7.Renal Anomalies in Neonates with Isolated Preauricular Tags.

PURPOSE: The aim of this study was to determine the role of renal ultrasonography in neonates with isolated preauricular tags. METHODS: We performed a retrospective study of 10,997 newborn infants delivered from January 1995 to June 2003 in Wonkwang University Hospital. Nineteen newborns born with isolated preauricular tags were assessed for renal anomalies by performing renal ultrasonography within one week after birth. The study group was compared with a control group of 25 healthy neonates without preauricular tags during the same period, also using renal ultrasonography. RESULTS: Preauricular tags were detected in 19 of 10,997(0.17%) neonates; 57.9% were found on the right side. Renal anomalies in neonates with isolated preauricular tags were detected in four infants (21.1%); this incidence was higher, but was not significant, compared with the control group. Types of anomalies were mild hydronephrosis of grade 1(3 cases) and 2(1 case) which were normalized within one year. CONCLUSION: Routine renal ultrasonography is not recommended for newborn infant with isolated preauricular tags.

9.Human Papilloma Virus Type 16 E7 Oncoprotein Stabilizes p53 Protein but not Induced p53-mediated Apoptosis in HepG2 Cells after gamma-irradiation under Hypoxia.

Human papilloma virus 16 E6 and E7 oncoproteins are well known to change cell functions, especially through p53 and pRb expression, so we studied their effects on molecular mechanisms and on the cell death associated with hypoxia and ionizing radiation. These treatments both caused cell death and increased p53 protein expression in HepG2 cells. This increased p53 expression by gamma-irradiation under hypoxia induced G1 cell cycle arrest and led to apoptosis even though HepG2 cells have a relatively reduced ability to induce p21 and pRb expression levels. Ablation of p53 expression by the HPV 16 E6 gene induced E2F-1 expression, which plays a role in cellular survival, especially under hypoxia or gamma-irradiation. The steady-state level of p53 action produced by HPV 16 E7 did not induce apoptotic cell death or the production of the apoptotic regulators, the bcl-2 family and caspase-3, so it did not appear to participate in apoptotic signaling in response to hypoxia and ionizing radiation. Thus, the HPV 16 E7 oncoprotein did not increase the rate of cell death induced by p53, although p53 might play a role in apoptosis in HepG2 cells.

10.Cytotoxicity of Diamide and the Protective Effect of Thioredoxin on Diamide-Induced Vasculotoxicity in Vascular Endothelial Cells.

BACKGROUND AND OBJECTIVES: This study was designed to examine the effects of diamide and thioredoxin (TRX) on vascular endothelial cells in order to clarify the mechanism by which vascular damage is mediated by oxygen free radicals. MATERIALS AND METHODS: The pulmonary artery endothelial cell (PAEC) line derived from bovine serum was cultured for 8 hours in media supplemented with various concentrations of diamide and TRX. The XTT assay, MTS assay, SRB assay, LDH activity and lipid peroxidation tests were perfomed. RESULTS: In XTT and MTS assays, diamide significantly decreased the cell viability of cultured PAEC in a dose- and time-dependent manner. Diamide showed a decrease in the amount of total protein, although it showed an increase of lipid peroxidation and LDH activity in cultured PAEC. In regards to the protective effect of TRX on diamide-induced cytotoxicity, this showed an increase of total protein, however it showed a decrease of lipid peroxidation and LDH activity. CONCLUSION: Our results suggest that diamide has a vasculotoxic effect on cultured bovine PAEC and that TRX is very effective in the protection of diamide-induced cytotoxicity by duye to the increase of total protein and the decrease of lipid peroxidation and LDH activity in these cultures.
