4.Diagnostic Value of Stable Microbubble Rating Test and Shake Test for the Early Detection of Respiratory Distress Syndrome in Prematurity.

Respiratory distress syndrome (RDS) of newborn is a disease revealed high morbidity and mortality rate, especially in premature infant. To evaluate the predictive value of Stable Microbubble Rating (SMR) and Shake test on RDS in premature infant, the anthors carried out the gastric aspirates Shake test and SMR test at birth, 3 and 6 hours after birth respectively on 124 premature infants who were born at the department of Pediatrics, Dong-San Hospital, Keimyung University during the period of 6 months form June 1993 to November 1993. The following results were obtained: 1) Among the 124 premature infants, RDS was developed on 23 (18.5%). 2) The birth weight, gestational age and Apgar score were significantly lowered in RDS cases than non-RDS cases (p<0.005). 3) The sensitivity, specificty, positive-and negative-predictive values of Shake test were 87.0% 79. 2% 48.8% and 96.4%, and those of SMR test were 100% 93.1% 76.7% 100% at birth, 95.7% 88.1% 64.7% 98.9% at 3rd-hour of life, and 95.7% 72.3% 44% 98.6% at 6th-hour of life. With the results of this study we concluded that SMR test at 6th-hour of life.

5.Differential Regulation of Antioxidant Enzymes during Monocyte Differentiation.

Antioxidant enzymes levels were determined in monocytes during phorbol myristate acetate (PMA)-induced differentiation. PMA induced the differentiation of a human monocytic leukemia cell line THP-1 into macrophage-like cells as indicated by activity of acid phosphatase and morphological changes. The level of Mn-superoxide dismutase (SOD) was selectively increased in PMA-treated THP-1 cells after one day of culture, while the levels of Cu/Zn-SOD and catalase were progressively decreased by Western blot analysis. In contrast, levels of Cu/Zn-SOD and catalase protein and enzyme activitiy remained unchanged in THP-1 cells after transforming growth factor-p, treatment. Cu/Zn-SOD is oxidatively inactivated by exposure to H,O, which is produced by PMA-treated THP-1 cells, and then the inactivated enzyme undergoes proteolysis and fragmentation as analyzed by radiolabeled method. Thus monocytes have a coordinated system for synthesis and degradation of antioxidant enzymes during PMA-induced differentiation.

7.Effects of Holistic Hospice Nursing Intervention Program on Self Esteem and Spiritual Well-being for Inpatients of Hospice Palliative Care Unit.

PURPOSE: This study was to testify the effects on self esteem and spiritual well-being of holistic hospice nursing intervention program ("Rainbow program") for inpatients of hospice palliative care unit. This was designed as a preliminary experimental study with one-group pre-post test. METHODS: A total of 27 patients who were over 18 years old, and admitted in hospice palliative care unit of S hospital in P city, submitted informed consent for this study, participated in holistic hospice nursing intervention program(total 10 sessions and 1,200 minutes for 2 weeks) from April 6, 2004 to April 20, 2005. To test the effects of this intervention, Self Esteem Questionnaire (SEQ) and Spiritual Well-being Questionnaire were used. The collected data were analyzed by Paired t-test with SPSS/WIN 12.0 program. RESULTS: (1) Hypothesis No. 1 "The experimental group which received Rainbow program will have a higher degree of self esteem than before" was supported (t=11.554, P<0.001). (2) Hypothesis No. 2 "The experimental group which received Rainbow program will have a higher degree of spiritual well-being than before" was also supported (t=6.387, P<0.001). CONCLUSION: This Holistic Hospice Nursing Intervention Program was effective in increasing self-esteem and spiritual well-being of patients in hospice palliative care unit. Therefore, it can actively be used and also applied to hospice palliative care practice, research, and education as a useful model of interdisciplinary team approach by hospice professionals.

9.Tendonitis of the Flexor Hallucis Longus with Triggering of the Great Toe: A Case Report.

We present an unusual case of 31 year old lady with a longitudinal tear of flexor hallucis longus(FHL) caused by trauma. Tendonitis of the FHL, well known as dancers tendonitis, can be characterized by triggering of the great toe. As the foot is brought into plantar flexion with a forcible active contraction of the FHL, a snap is noted in the region of the posteromedial aspect of the ankle. Magnetic resonance imaging may be helpful in establishing diagnosis. The patient has obtained longterm satisfactory relief of her symptoms with surgical treatment. The literature on tendonitis of the FHL is reviewed with a reported case.

10.In Vitro Pharmacokinetics of Vancomycin Release from Locally Implantable Materials

Local deposition of antibiotics has become increasing popular in the management of open fractures or osteomyelitis, and several substances have been employed as the vehicle for delivery. Although the elution characteristics of some substances have been documented, a comparative study of the characteristics of the commonly used substances were performed in order to establish the clinical indications for particular vehicles. Carriers were prepared, which were human iliac cancellous bone, bovine cancellous bone matrix(Lubboc?), absorbable gelatin sponge(Gelfoam?), fibrin glue(Beriplast? P) and polymethylmethacrylate(CMW?) for elution characteristics of vancomycin. The each carriers were immersed on the 20 ml of PBS and then obtained the samples for analysis of concentration of vancomycin at first, second, third, fourth, fifth, sixth, fourteenth, twenty-first, twenty-eighth day after immersion. The assay technique was fluorescent polarization immunoassay(Abbott, Dallas, Texas, U.S.A.). Nearly 50% of vancomycin was released from human iliac cancellous bone, Lubboc? and fibrin glue during the first 3 days, and negligible after first week. Gelatin sponge produced high local concentration of vancomycin during the first week. PMMA eluted the only 6% of vancomycin during the first day and trace amount detected as long as 4 weeks. There are significant statistical difference among carriers at second and fourth week(ANOVA test, P < 0.05). The authors considered that human iliac cancellous bone, Lubboc? ,Gelfoam? and fibrin glue may be best employed when brief antibiotic coverage is required, whereas PMMA may be better suited for long-term coverage.
