1.Production and Characterization of Antibodies to Propionibacterium acnes from Immunized Chicken Eggs.

BACKGROUND: Treatments inhibiting Propionibacterum acnes as like antibiotics for acne vulgaris can produce complications, and have their limited uses.OBJECTIVE: Antibodies reacting to P. acnes were produced from immunized chicken eggs, and were characterized by aggulutination test and inhibiting abilities to growth of P. acnes. METHODS: P. acnes were cultured, and their lipase were used as immunogen. Antibodies were purified from immunized chicken eggs by diafiltration. Their titers were measured by ELISA. Antigen-antibodies agglutination test was performed, and the ability of inhibitions of P. acnes grown in media was observed. RESULTS: Antibodies were well produced especially when 0.25 to 2 mg of immunogen were injected per one chicken. When antibodies were reacting to P. acnes, pellet formations were not shown because of agglutination above 17.8 pg concentration of antibodies, and acitivities of P. acnes were markedly suppressed on microscopic examination. Growth of P. acnes on media were also inhibited for quite a long time. CONCLUSION: Antibodies reacting to P. acnes were produced, and their abilities to inhibit the growth of P. acnes were confirmed. In future, produced antibodies from immunized chicken eggs would be hoped to be a main component of medication or cosmetics for improving or preventing acne lesions.

2.Epidermal Nevus Syndrome with Various Skin Manifestations.

We report a case of epidermal nevus syndrome showing various skin manifestations improved with CO₂ laser and chemical peeling in a 11-year-old girl. Skin lesions were composed of linear verrucous plaques and numerous papillomatous papules on the face, neck, scalp and trunk, multiple congenital nevocellular nevi on the face, and extensive cafe au lait spots on the trunk. The associated findings of skeletal involvement were gingival hemihypertrophy and benign bone lesion of the 7th rib. CO₂ laser and chemical peeling MCA, 50% TCA) were applied to remove these skin lesions which improved considerably.

Bronchoalveolar lavage (BAL) has emerged as a useful technique for the study of pulmonary interstitial disorders. Several types of information are provided by the evaluation of lavage fluid. Identification of cellular constituents helps to separate inflammatory process. Recently we have studied cellular constituents of BAL from three cases with histologically confirmed pulmonary sarcoidosis, idiopathic pulmonary fibrosis and hypereosinophilic syndrome. Pulmonary sarcoidosis showed a marked increase in lymphocytes, idiopathic pulmonary fibrosis revealed a predominance of neutrophils, and hypereosinophilic syndrome presented a marked increase in eosinophils in the lavage fluids.

6.Volumetric Quantitation of Pulmonary Regurgitation and Right Ventricular Function in Postoperative Tetralogy of Fallot by Echocardiography and Magnetic Resonance Imaging.

PURPOSE: Despite improved surgical techniques, pulmonary regurgitation is a common postoperative sequelae in patients with tetralogy of Fallot. Pulmonary regurgitation may induce deterioration of right ventricular performance, right ventricular dilatation and ventricular arrhythmia. The evaluation of the clinical significance of pulmonary regurgitation in postoperative tetralogy of Fallot has been troublesome since there was no technique that could accurately quantitate volume of pulmonary flow. Magnetic resonance imaging (MRI) with velocity mapping provided data on the volumetric quantitation of pulmonary regurgitation. The purpose of this present study was to assess right ventricular function and the severity of pulmonary regurgitation after surgical correction of tetralogy of Fallot by echocardiography and MRI. METHODS: Echocardiography and MRI were performed on nine patients with total surgical correction of tetralogy of Fallot. Quantitative volumetric assessment of pulmonary reguritation and right ventricular function was done. RESULTS: The pulmonary regurgitant volume was similar between echocardiography and MRI, and the pulmonary velocity was significantly larger by MRI in comparison with echocardiography. Right ventricular end-diastolic volume and stroke volume were similar in both two methods, but end-systolic volume was significantly larger by MRI in comparison to echocardiography. Pulmonary regurgitant volume was positively correlated with right ventricular end-systolic volume, end-diastolic volume and stroke volume. Each parameter (pulmonic velocity, pulmonary regurgitant volume, right ventricular end-diastolic volume, end-systolic volume, stroke volume) was significantly correlated linearly between echocardiography and MRI. CONCLUSION: Cine MRI with velocity mapping is an accurate method for the noninvasive, volumetric quantitation of pulmonary regurgitation after surgical correction of tetralogy of Fallot.

7.Value of Phospholipase C gamma-1, Epidermal Growth Factor Receptor, and Her-2/neu in Human Breast Cancer.

PURPOSE: Oncogen or growth factor receptor such as phospholipase C isoenzyme gamma-1 (PLC gamma-1), epidermal growth factor receptor (EGFR), and Her-2/neu which related with tyrosin kinasemay and then regulating vell proliferation may have a role as prognostic factors for breast cancer. MATERIAL AND METHODS: With assumption that expression of PLC gamma-1, EGFR and Her-2/neu oncogene has close relationship with prognosis of breast cancer, 59 breast cancer patients who were operated upon at Korea University Hospital during a period of 6 years starting June 1988 to May 1994 were selected for this study. This study was carried out by comparing between expression of PLC gamma-1, EGFR and Her-2/neu oncogene and patient's survival rate. These expression were also compared with TNM system, estrogen and progesterone receptor and at same time these expressions were compared with each other to see whether there are any relationship among these expression. RESULTS: Expression of PLC gamma-1, EGFR and Her-2/neu were present in 42% (25/59), 46% (27/59) and 20% (12/59). The expression of PLC gamma-1 was closely related with the expression of EGFR (p<0.05) and Her-2/neu (p<0.05), but there were no relationship between the expression of PLC gamma-1 and hormonal receptors and TNM stage (p>0.05). The expression of EGFR was closely related with the expression of Her-2/neu (p<0.05) and hormone receptors (p<0.05), but there were no relationship between the expression of EGFR and pathologic TNM stage (p>0.05). The expression of Her-2/neu was not closely related with hormone receptors and TNM stage except axillary lymph node metastasis. There were close relationship between overall and disease free survival and PLC gamma-1 and Her-2/neu. But EGFR had only related with disease free survival rate. CONCLUSION: In conclusion, the expression of PLC gamma-1, EGFR and Her-2/neu oncogene in human breast cancer may be useful prognostic factors independently and it may potentiated its individual value as a prognostic factors if use them together.

8.Development of the Quality Indicators in Long Term Care Service.

PURPOSE: This study was designed to develop quality outcome indicators for nursing homes and community-based home care that would contribute to an appropriate evaluation and improvement of quality of long term care in Korea. METHODS: The preliminary quality indicators of long term care were developed from a literature review and clinical expert panel. A content validity testing was done using a panel of experts who were selected from academic and clinical field of long-term care. The final quality indicators were confirmed after application in four nursing homes and four home care agencies to test clinical validity. RESULTS: The preliminary quality indicators consisted of 3 domains and 19 indicators. The final quality indicators were composed of 4 domains and 17 indicators. CONCLUSION: This study demonstrated the feasibility of outcome quality indicators in long term care. These quality indicators can be effectively used to evaluate the quality of nursing home and home care and to improve the quality of care in the Korean long-term care system.

9.Spontaneous Rupture of Renal Artery Aneurysm in a Patient with Untreated Hypertension.

Spontaneous rupture of renal artery aneurysm is a rare, but life threatening complication with high mortality. The etiology of non-traumatic renal artery aneurysm is fibromuscular dysplasia, atherosclerosis, vasculitis, and pregnancy. We here report a case of spontaneous rupture of renal artery aneurysm in a patient with untreated hypertension. A 39-year-old non-pregnant woman complained of sudden onset of right flank and lower quadrant abdominal pain. Ultrasonography showed large fluid collection in right lower abdomen. Emergency laparotomy demonstrated huge retroperitoneal hematoma due to spontaneous rupture of right renal artery aneurysm. Microscopic examination of the aneurysmal wall revealed intimal hyperplasia without atherosclerotic change.

10.Comparison of Meibomian Gland Loss between Demodex-infested Group and Dry Eye Patients without Demodex Infestation

Purpose:
To compare the degree of meibomian gland drop-out associated with Demodex infestation using non-contact meibography.
Methods:
We performed noncontact meibography using an infrared digital camera on 24 adult subjects infested by Demodex and 40 without infestation. The right upper and lower lids were scored based on the loss of meibomian glands, and the scores of the upper and lower eyelids were summed to obtain the total meiboscore for each subject. Meiboscores were evaluated according to Demodex infestation and gender in each group.
Results:
Sixty-four eyes of 64 people were enrolled in this study. The study subjects had an average age of 67.9 years (range, 40-79 years; men, 65.2 years; women, 69.1 years). There was a significant association between Demodex infestation and total meiboscore and upper and lower eyelid meiboscores (p = 0.000, p = 0.002, p = 0.004, respectively). There was no significant difference in the meiboscores between men and women. Age and meiboscore suggested a weak positive correlation; however, the R2 value was low, due to the concentrated age distribution of the study population.
Conclusions
Based on noncontact meibography, the authors concluded that the prevalence of changes in the meibomian glands was significantly related to Demodex infestation; however, there was no significant difference between men and women.
