Recent advance in the field of microvacular surgery has popularized the reattachment of severed digit, with increasing success rate and improving functional regain. Reattachment surgery was performed in six digits, two thumbs and four fingers, which had been amputated at or distal to the level of DIP joint. Microvascular anastomosis of one digital artery was done and the venous drainage was reestablished with blood letting from the pulp of each digit. There was one failure in which hypersensitivity to heparin was present, while all other five showed success. Postoperatively considerable sensory recovery was present in all.

5.A STUDY ON THE HEAT GENERATION OF BONE DRILLING BURS ACCORDING TO IRRIGATION DURING IMPLANTATION OF DENTAL IMPLANT.

There are a lot of elements affecting the success of implant prosthesis. The quality of surgical procedure is considered as one of the key factors. To avoid the excess heat generation is an important element of successful osseointegration and it can be achieved by using a gentle surgical technique with a sharp instrument in bone drilling. This study was performed to measure and analyze comparatively the heat transmitted to surrounding bone at a distance of 0.5mm from the periphery of the drill hole in each drilling stage. The results were as follows. In standard system, the temperature of surrounding bone tissue ranged from 29.2degrees Cto 48.3degrees C with irrigation and from 34.6degrees C to 84.3degrees C without irrigation. And in wide system, the temperature of surrounding bone tissue ranged from 29.5degrees C to 52.5degrees C with irrigation and from 34.8degrees C to 87.8degrees C without irrigation. And the temperature ranges exceeded the threshold without irrigation, while showing less than the threshold by the cooling effect of irrigation. In comparing standard system with wide system, although there was no significant difference, 4.3mm pilot and 4.3mm twist drill of wide system showed high value and wide system showed slightly high elevation of temperature in all depth in fixture installation In the finite element analysis, the calculated value by the Fourier's cooling law were applied to the bone drilling surface. And through analysis using different irrigation temperatures at 28degrees C, 15degrees C and 5degrees C, and according to the time. The result was that the cooling water at least below 15degrees C was required to maintain the temperature of surrounding bone less than threshold in bone drilling, the cooling water below 5degrees C was required to gain more sufficient cooling effect, and cooling over 5 seconds was needed after bone drilling for sufficient effect.

6.Percutaneous Decompression in the carpal Tunnel Syndrome.

Carpal tunnel syndrome is the most common peripheral nerve entrapment syndrome due to compression of the median nerve at the wrist. Division of the transverse carpal ligament for median nerve compression in patients with carpal tunnel syndrome is a common and successful procedure. A variety procedures exist to release the transverse carpal ligament the classic open release, limmited open or percutaneous release, and endoscopic release. From February 1994 to January 1996, 39 carpal tunnel releases were performed by percutaneous method in 30 patients (3 males and 27 females) at the Chonnam University Hospital. The patients age ranged from 42 to 67 years (average 54.8 years). The duration of the symptoms ranged from 3 months to 10 years. According to Brown's outcome analysis, results were as follows. 1. Persistent pain and paresthesia were noted in only 3 cases (7.7%) at 24 week postoperatively. 2. Two-Point discrimination on the index finger was improved significantly at 12 week postoperatively (p<0.05). In electromyographic study, conduction delay of motor and sensory fibers was diminished significantly at 4 week postoperatively (p<0.05). 3. Functional outcomes (grip strength, pinch strength, scar tenderness and return to work) were showed good and excellent result. We concluded that percutaneous decompression is one of the less morbid method for decompressing the carpal canal and relief of symptoms for the patient.

7.ADHD and Its Comorbidities in College Students, with a Focus on Depression and Pathological Internet Use

Background:
There is growing interest in adult attention-deficit hyperactivity disorder (ADHD) because many ADHD children have symptoms that last through adulthood, and adult ADHD has a high risk of comorbid mental disorders. This study examined the prevalence and comorbidities of adult ADHD, with a focus on depression and pathological internet use among Korean college students.
Methods:
This study included 1358 college students, who were asked to complete self-reporting questionnaires on their characteristics, ADHD symptoms, and related clinical factors. The study analyzed associations between ADHD and depression, anxiety, pathological internet use, and problematic alcohol use.
Results:
The prevalence rate of ADHD among college students was 5.7%. A significant association was determined between ADHD symptoms and school satisfaction, depression, anxiety, and pathological internet use. Multivariate analysis showed that ADHD symptoms in college students were significantly associated with depression and pathological internet use.
Conclusion
The prevalence of ADHD among college students was similar to that of children and adolescents. Moreover, the risk of depression and pathological internet use was high in the ADHD group. Early screening and intervention for ADHD and comorbidities will be required to prevent the negative consequences of the condition and help students adapt in the future.

9.Two Cases of Segmantal Dilatation of the Intestine in Newborn Infants.

Segmental dilatation of small intestine or colon can induce signs of intestinal obstruction, such as abdominal distension, vomiting and constipation. There are no anatomical gross obstructive lesions, and moreover, ganglion cells are observed in both dilated and undilated distal segments of the intestine. It often accompanied by other congenital anomalies. We reported two cases of segmental dilatation of the intestine in the newborn infants, one in small intestine and the other in colon, with brief review of the related literatures.
