2.A STUDY ON THE HEAT GENERATION OF BONE DRILLING BURS ACCORDING TO IRRIGATION DURING IMPLANTATION OF DENTAL IMPLANT.

There are a lot of elements affecting the success of implant prosthesis. The quality of surgical procedure is considered as one of the key factors. To avoid the excess heat generation is an important element of successful osseointegration and it can be achieved by using a gentle surgical technique with a sharp instrument in bone drilling. This study was performed to measure and analyze comparatively the heat transmitted to surrounding bone at a distance of 0.5mm from the periphery of the drill hole in each drilling stage. The results were as follows. In standard system, the temperature of surrounding bone tissue ranged from 29.2degrees Cto 48.3degrees C with irrigation and from 34.6degrees C to 84.3degrees C without irrigation. And in wide system, the temperature of surrounding bone tissue ranged from 29.5degrees C to 52.5degrees C with irrigation and from 34.8degrees C to 87.8degrees C without irrigation. And the temperature ranges exceeded the threshold without irrigation, while showing less than the threshold by the cooling effect of irrigation. In comparing standard system with wide system, although there was no significant difference, 4.3mm pilot and 4.3mm twist drill of wide system showed high value and wide system showed slightly high elevation of temperature in all depth in fixture installation In the finite element analysis, the calculated value by the Fourier's cooling law were applied to the bone drilling surface. And through analysis using different irrigation temperatures at 28degrees C, 15degrees C and 5degrees C, and according to the time. The result was that the cooling water at least below 15degrees C was required to maintain the temperature of surrounding bone less than threshold in bone drilling, the cooling water below 5degrees C was required to gain more sufficient cooling effect, and cooling over 5 seconds was needed after bone drilling for sufficient effect.

3.The Prevention of Posterior Displacement of Tibia During Treatment of Posterior Cruciate Ligament Injury Using Modified Quengel Hinge

In the treatment of posterior cruciate ligament injury, posterior displacerrient of tibia might be occured because of the direction of gravity due to weight of lower leg. To prevent this problem several methods have been introduced, but not settled yet. Therfore the author tried to solve this problem by applying Quengel brace that was firstly described by Mommsen in 1922 and perfected by Jordan using the correction of flexion contracture of the knee in hemophiliacs. But owing to the development of some problems in original hinge for prevention of posterior displacement of tibia after posterior cruciate ligament injury, we modified the design of the hinge that more effective anterior traction force should be operated on tibia. The authors applied Quengel brace including original and modified hinge to 21 cases of posterior cruciate ligament injury between June 1983 and May 1986. The followings were obtained. 1. By modification of the hinge, more effective anterior traction force operated on tibia without pressure sore or joint narrowing. 2. On clinical application of modified Quengel brace, posterior displacement of tibia were prevented effectively.

7.Locking of the Metacarpophalangeal Joint of the Thumb: Report of Two Cases and an Anatomic Study of the Heads of the First Metacarpals.

Locking of the metacarpophalangeal(MP) joint of thumb in mild hyperextension is relatively uncommon problem. The most frequently recognized cause appears that proximal palmar ligament or volar plate were ruptured tranversely with its distal part riding over the volar prominence of the radial condyle of the first metacarpal head. We experienced two cases of locking of the MP joint of the thumb due to volar plate injury. Closed reduction was failed in both cases and open reduction was done by cutting the constricted ligament hundle over the radial condyle of the first metacarpal head. Our description of this mechanism is supported by expeimental evidence that we obtained from the cadeveric specimens. The height of radial side condyle of first metacarpal head is higher than ulna side of that by 1.76mm in both hands of 16 cadeveric specimen. So we are going to report these cases with a review of the literature, and suggest that the preferable location of surgical incision in open reduction is radial side of metacarpal head.

8.Clinical values of CT and dynamic CT in brain infarction

With the advent of faster scan time and new computer program, a scanning technique called “dynamic computedtomography” has become possible. Dynamic CT consists of performing multiple rappid sequence scans after injectionof contrast material. The authors have evaluated the clinical usefulness of CT and dynamic CT of 93 patients withbrain infarction and/or ischemia during the period of 17 months from April 1983 to Aug. 1983 to Aug. 1984 inDepartment of Radiology, Chonbuk National University Hospital. The results were as follows; 1. The agedistribution ranged from 18 years to 78 years. Among them the most common age group was between 50 years and 59years(40.9%). 2. The sites of brain infarction were cerebral lobes(63 cases,68), basal ganglia(15 cases, 16.1%)and mlultiple sites(6 cases, 6.4%). The common affected site was middle cerebral artery territories. 3. Thecontrast enhancement of acute infarction was noted in 14 cases(17.5%) which occured commonly between 3 days and 2weeks from ictus. 4. The patterns of time-density curve in brain infarction and/or ischemia were as follow: a .Depression of slow wash-in phase was 20 cases(59%). b. Lower peak concentration was 17 cases(50%), c. Lower anddelayed peak concentration was 7 cases(21%), d. No definite peak concentration was 6 cases(18%). First threepatterns of time-density curve were thought as relatively characteristic curve of brain infarction and/orischemia. 5. Two cases that showed negative findings on precontrast CT scan appeared to be positive findings ashypodensity on postcontrast CT scan and were confirmed as brain infarction by dynamic CT. 6. The diagnostic entityof dynamic CT scan were as follows: a. large artery thrombotic infarction were 23 cases (58%). b. lacunarinfarction were 6 cases (15%). c. ischemia were 5 cases (13%), d. normal were 5 cases(13%), In six cases oflacunar infarction which was doubtful hypodensity on pre-and postcontrast CT scan had a marked difference in CT#(HU) on absolute scale graph of dynamic CT, so diagnosis of lacunar infarction could be made easily. 7. Theclinical values of dynamic CT consist in not only diagnosis of lacunar infarction but also evaluation ofeffectiveness of medical or surgical treatment.

9.A case of primary obstructive megaureter with CT demonstration

Megaureter is a relatively rare disease and its first description was done by Caulk in 1923. It is characterized by marked ureteral dilatation without obvious cause, and occurs in lower ureter especially. Authers report a case of primary obstructive megarureter with CT demonstration in 19 year-old female. To our knowledage,this is the first case report of primary obstructive megaureter in Korea.

10.Prediction of Failure to Survive Following In-hospital Cardiopulmonary Resuscitation.

BACKGROUND AND PURPOSE: The purpose of this study is to compare two clinical predictive rules, the pre-arrestmorbidity(PAM) index and the prognosis-after-resuscitation(PAR) score, which predict failure to survive following in-hospital cardiopulmonary resuscitation(CPR). METHOD: The study population consisted of 162 consecutive adult patients who underwent CPR at Wonju Christian Hospital over a year period. The PAM index and PAR score were calculated from the most recent data available for each variable prior to cardiac arrest. Each predictive tool was compared between the group of discharge alive and the group of in-hospital mortality. Performance of the predictive scores was also compared by receiver-operating characteristic(ROC) curves where appropriate. RESULTS: PAM index of study population was 4.39+/-2.69 and PAR score was 2.99+/-3.36. PAM index in the group of discharge alive was 1.87+/-2.79, and PAM index in the group of ih-hospital mortality was 4.51+/-2.62. PAR score in the group of discharge alive was 0.75+/-1.75, and PAR score in the group of in-hospital mortality was 3.1+/-3.4. The PAM index identified 15 patients with a score>8, while the PAR score identified 39 patients with a score>4, none of whom survived. The sensitivity of the PAR score for the prediction of failure to survive was 25%, while that of the PAM index was 10%; neither index incorrectly identified a patient as a non-survivor who eventually survived. Both of predictive methods were not significantly different in the ROC curve. CONCLUSION: Although further confirmation is necessary, PAM index and PAR score may provide useful prognostic information to physicians and patients involved with decisions about do-no- resuscitate orders.
