The origin of osteoclast-like giant cell tumor (OGCT) of the salivary gland has been debated because the prototypic cells of osteoclast-like cells and mononuclear stromal cells are largely unexplained in this gland. Bone marrow-derived CD14+ and CD45+ monocyte-derived multipotential cells (CD14+/CD45+ MOMC) may be one of the possible origins of OGCTs of salivary glands, which have never been explored in salivary OGCTs. We present a case of OGCT accompanied with carcinoma ex pleomorphic adenoma in the parotid gland of a 67-year-old Korean female. The tumor presented as a rapidly growing cervical mass comprising a central area of carcinoma ex pleomorphic adenoma and a peripheral circumferential area of OGCT. The immunohistochemical staining pattern was phenotypically consistent with bone marrow-derived CD14+/CD45+ MOMC. This case is the first report of a salivary OGCT in Korea.

3.The Development of a Continuing Nursing Care Program for Cancer Patients after Discharge.

The purpose of this study was to develop a continuing nursing care program for cancer patients and the caregivers staying at their home and to test its effects on patients' symptom distress, caregiver burden, and satisfaction of life. The continuing nursing care program was based on the homecare needs of cancer patients, and was provided by three clinical nurses who took care of the patients while they were hospitalized. The program consisted of discharge education about selfcare using education materials(book and handout) and provisions of direct care. These were counselling and education during which visiting patients' home one week after discharge and telephone counseling two weeks after discharge. Counseling by telephone was always available during the research period. A quasi-experimental research design was used to test the effects of the program. Subjects for this study were 53 cancer patients discharged from the hospital and caregivers. These subjects were assigned to an experimental group (n=23) receiving continuous nursing care, or to a control group (n=30) not receiving continuous nursing care. Data from control group was collected first to protect from contamination. Data collection was done from October of 1998 to February of 2000. The collected data was analysed using mean, t-test, and chi-square test computed by SPSS software. The summary of results was as follows: 1) The symptom distress was a little decreased at posttest, but there were no significant differences between the experimental and the control group in symptom distress. 2) The score of caregiver burden was significantly decreased in experimental group at posttest, but no differences in control group. 3) There were no significant differences between the experimental and control group in the satisfaction of life. 4) The score of satisfaction of continuing nursing care program in experimental group at posttest was 2.321 of 3. In conclusion, even though this study did not obtain evidence of effectiveness of continuing nursing care program on patients, such as. It is still expected to be effective by a more improved program. Therefore we want to give some suggestions for further studies. 1) It is needed to make a communication channel with the patient's doctor to response promptly and appropriately to patient's conditions. 2) The research is necessary on patients in terminal stage or early stage of cancer diagnosis who have many nursing needs. 3) It is needed to readjust the roles and job assignment of clinical nurse to implement effectively as a program provider.

4.MRI Study about the Early Changes of Lumbar Disk Degeneration using Magnetization Transfer Contrast (MTC).

PURPOSE: To obtain magnetization transfer ratio(MTR) of the annulus fibrosus and nucleus pulposus and to assess the feasibility of utilizing the changes of these MTRs as an early indicator of disk degeneration. MATERIALS AND METHODS: MR images of lumbar spine with magnetization transfer(MT) technique in 42 patients were obtained. spin echo techniques (600/14) with same TR/TE with 1KHz off-resonance saturation were employed in 1.0T MR system. MTRs were calculated in two regions, anterior annulus fibrosus and nucleus pulposus, and the results were compared between the normal and degenerative disks, from grade I to IV, on T2-weighted images. RESULTS: MTRs of the nucleus pulposus were 17.6% in the normal disks, and 26.7%, 28.4%, 29.1%, 29.7% in degenerative disks, from grade I to IV, respectively, with a significant difference(P<0.05). On the other hand, MTRs in the annulus fibrosus were 30.2% in the normal disks and 31.5%, 33.2%, 32.1% and 35.6% in degenerative disks, from grade I to IV, respectively, without significant difference. CONCLUSION: Since MTRs are significantly higher in degenerative nucleus pulposus than those of the normal disks, increased MTRs in the nucleus pulposus can be used as an early sign of the degeneration of the nucleus pulposus.

5.Major patterns of inflammatory sinonasal diseases on CT.

Paranasal sinus CT is known as the most effective imaging modality in the evaluation of inflammatory sinonasal diseases and can depict the distribution, causative lesions obstructing main drainge route, and associated findings. Recently, functional endoscopic sinus surgery has been widely used for the evaluation and treatment. Before operation, PNS CT has been routinely used to evaluate the paranasal sinuses and mucociliary drainage route. The authors analyzed the PNS CT findings of 3156 cases in 1578 patients with chronic sinusitis symptoms. Sinonasal inflammatory diseases were categorized into 5 patterns according to the obstruction sites. They were 1) infundibular (10%, 316/3156), 2) ostimeatal unit (41%, 1294/3156), 3) sphenoethmoidal recess (12%, 379/3156), 4) sinonasal polyposis (30%, 946/3156) , and 5) unclassifiable (6%, 190/3156) patterns. The main causes for infundibular obstruction in order of frequency were inflammatory mucosa, enlarged ethmoidal bulla and Haller's cell. With respect to the middle meatus obstruction, is main causes in the order of frequency were polypoid lesion, inflammatory mucosa and medially deviated uncinate process. In particular, sinonasal polyposis showed one or more of the characteristic associated findings of infundibular enlargement, air-fluid level, ethmoidal sinus was bulging and bony deossification or sclerosis as well as sinonasal polypoid change. In conclusion, the inflammatory sinonasal diseases were classified into five patterns, and the causative lesions or anatomic variations were efficiently detected by the PNS CT. Furthermore, it could provide a guidance for proper management of the sinusitis including functional endoscopic sinus surgery.

6.A Study on the Burden, Health Status and Well-being in Spouse Caregivers of Patients with Stroke.

PURPOSE: The purpose of the study is to investigate the correlation between burden, health status and well-being in spouse caregivers of patients with stroke. METHOD: The subjects of this study were 160 spouse caregivers registered at general hospital in Seoul and Seongnam. The data were collected from July 2th to October 30th, 2003. The Suh & Oh's burden scale, Yang's health status scale and Park's well-being scale were employed to measure burden, health status and wellbeing respectively. Collected data were analyzed using SPSS 12 version to obtain summary statistics for the descriptive analysis, Pearson Correlation, and Stepwise Multiple Regression. RESULTS: 1. The mean score of the degree of burden, health status and well-being were 3.52, 5.92 and 3.64 points. 2. The burden of the subjects were significantly negative correlated with psycho- logical health status(r=-.482. p<.01) and wellbeing(r=-.455, p<.01). 3. With the result of stepwise multiple regression, psychological health status and wellbeing were the main variables which could explain burden by 27.1%. CONCLUSION: The findings of this study will provide practical guidelines for developing emotional nursing interventions for the spouses taking care of CVA patients who would experience a heavy burden and distress.

8.Effect of Caregiver's Role Improvement Program on the Uncertainty, Stress, and Role Performance of Caregivers with Hospitalized Children.

PURPOSE: In this study a caregiver's role improvement program was developed and tested to identify the effect on uncertainty, stress, and role performance of caregivers with hospitalized children. METHODS: The design of this study was a quasi-experimental study with a nonequivalent control group and a non-synchronized design. Thirty-three caregivers were assigned to the experimental group and 33 to the control group. Data were collected from March 5 2016 to April 10 2016. For the experimental treatment, each individual was given on-site education with situated learning (given 30 minutes each, for 2 sessions), and self-repetition learning activities were performed from the e-book. Data were analyzed using t-test, χ2-test, Fisher's exact test, paired t-test, and independent t-test. RESULTS: The level of uncertainty and stress decreased, and role performance level improved for these caregivers with hospitalized children. CONCLUSION: The findings of this study show that using on-site education through situated learning and self-repetition learning with an e-book as in the caregiver's role improvement program is an effective intervention. Therefore, utilizing the caregiver role improvement program developed in this study is recommended as an effective intervention for caregivers of hospitalized children.

9.Development and Effects of a Child Health Management Program Based on Cooperative Learning for Mothers of Preschool Children.

PURPOSE: This study was conducted to develop and test effects of a child health management program based on cooperative learning for mothers of preschool children. METHODS: For this quasi-experimental study, a nonequivalent control group pretest-posttest design was used. Outcome measures were level of child health management self-efficacy, child health management practices, child's health behavior. Participants were 55 mothers (experimental group 28, control group 27). The program was composed of 6 categories of home health management for preschool children, and provided for three 120-minute sessions. Data were collected from September to October, 2015, at 3 times: baseline, 3 and 6 weeks after beginning the intervention. Data were analyzed using SPSS/WIN 21.0 program. RESULTS: After 3 and 6 weeks, there were significant differences between the two groups in child health management self-efficacy (F=18.33, p<.001), child health management practices (F=8.91, p<.001), and child's health behavior (F=9.91, p<.001). CONCLUSION: Study findings indicate that this child health management program based on cooperative learning is effective and can be recommended as an intervention for mothers of preschool children. It will contribute to enhanced child health management self-efficacy and health management practices and improved child's health behavior.

10.Nursing Needs and Nursing Performance as Perceived by Caregivers with Hospitalized Children.

PURPOSE: The purpose of this study was to identify differences between the degree of nursing need and nursing performance as perceived by caregivers of hospitalized children. METHODS: Participants included 200 main caregivers from two pediatric hospitals where their children had been hospitalized for at least 2 days. Data were collected from October 30 to December 10, 2014. RESULTS: The degree (2.64+/-0.40) of nursing performance perceived by caregivers with hospitalized children was found to be significantly lower than that (3.39+/-0.21) of nursing needs of caregivers. Of nursing needs, direct nursing had the highest score at 3.59, and nursing assessment, the lowest at 3.23. For nursing performance, nursing assessment had the highest score at 2.76, and education and counseling, the lowest at 2.35. CONCLUSION: These results suggest that performance of nursing activities by nurses caring for hospitalized children do not meet the nursing needs of the caregivers. Therefore it is necessary to develop nursing strategies to reduce the gap between nursing needs and nursing performance.
