Expression of the pS2 protein in breast carcinoma is a useful guide to evaluate the prognosis and response to tamoxifen. The pS2 protein is an estrogen-regulated 60 amino acid protein which was originally discovered following the screening of cDNA libraries in MCF-7 breast carcinoma cells and is induced through estrogen-dependent transcription of the pS2 gene. The presence of the pS2 protein in breast cancer is considered as valuable as the receptor status, or even more so, in predicting the response to hormonal therapy. We have investigated the pS2 protein expression in 62 cases of primary breast cancer in order to know the relationship between the expression rate of the pS2 protein and hormonal receptor status using immunohistochemical procedures on formalin-fixed and paraffin-embedded tissues. Concomitantly, both the estrogen receptors (ER) and progesterone receptors (PR) were examined using the immunohistochemical technique. Positive staining for the pS2 was seen in forty-nine cases (79%) of the tumors. Forty three cases (88%) of the pS2 positive tumors were ER positive and forty one cases (84%) of the pS2 positive tumors were PR positive ; forty six cases (93%) of pS2 positive tumors were positive for ER and/or PR. The pS2 status correlated significantly with the ER (p<0.0001) and PR (p<0.001). The results reveal a close association between the pS2 protein and either or both the ER and PR status.

Rupture of anterior communicating artery aneurysm is one of the most important casuses of subarachnoid hemorrhage. Subarachnoid hemorrhage due to ruptured anterior communicating artery aneurysm shows characteristic CT findings, such as forntal hematoma or septum pellucidum hematoma. In some patients with the ruptured aneurysm, direct carotid angiography may be needed. particularly when transfemoral approach is not possible. However, not much have been known about the puncture site selection between the two carotid arteries in relation to the location of hematoma in CT scans. To determine the ideal puncture site of the direct carotid angiography, seventy-five cases of anterior communicating artery aneurysm confirmed by operation from May 1988 to April 1992, were analyzed in terms of relationship between location of hematoma on CT, direction of aneurysm and A1 artery on angiograms. Frontal lobar hematoma was more prevalent than septal hematoma, and was more common on the right side. In cases of lobar hematoma, aneurysm was directed to the ipsilateral side of the hematoma (67%) and was most commonly fed by contralateral A1 artery(75%). Septal bematoma consisted 36% of the cases and among there, midline directed aneurysm was most common, frequently fed by left A1 artery. In conclusion, when performing direct carotid angiography in patients with anterior communicating artery aneurysm, if forntal hematoma is observed on CT, it may be beneficial to puncture contralateral carotid artery. Meanwhile, if hemorrhage of septum pellucidum is observed, it may be better to puncture left carotid artery.

PURPOSE: To evaluate morphologic characteristics of the clustered microcalcifications in benign lesions and to compare with that of malignant lesions. MATERIALS AND METHODS: We retrospectively reviewed 33 patients with clustered microcalcifications( five calcifications in number per square centimeter)on mammography. We analyzed mammographic findings with regard to shape, size, number/cm2, irregularity of size and shape, presence of central lucency, and accompanied mass density. RESULTS: Of 12 benign lesions, the most frequent shape was dot or round form (4/12, 33.3%) followed by ring form (3/12, 25%). The size was smaller than 0.5mm in 5 patients (41.7%), 0.5-1 mm in one and more than 1 mm in 6 patients(50%). The number of calcifications per square centimeter were five to ten in 41.7%, more than ten in 58.3%. Of all 21 malignant lesions, the most frequent shape was branching form (8/21, 38.1%). The sizes of calcification were less than 0.5ram in 76.1%(16/21). Central lucency within the calcification was seen only in benign lesions (3/14, 21%). Irregularity ih size and shape of calcifictions was noted in both benign and malignant lesions. Accompanied parenchymal mass density was more commonly associated with malignant lesions(80. 9%) than benign lesions(58.3%). CONCLUSION: Our results suggest that benign clutered microcalcifications on mammography could be differentiated from malignant calcifications with the criteria of larger size, central lucency and infrequent accompanied mass.

PURPOSE: To evaluate the value of fast field echo(FE) technique in accesing temporOmandibular joint(TMJ) dynamics. MATERIALS AND METHODS: A series of 120 TMJ MRI examinations of 60 symptomatic patients was performed. The fast FE technique was used to provide dynamic images of the TMJs in various phase of mouth opening with a step-bite-block. RESULTS: Among 120 cases, there were 30 cases which were normal, 21 cases of anterior displacement with reduction, 66 cases of anterior displacement without reduction, 11 cases of deformity of menisci, 23 cases of bony deformity, and 3 cases of medial displacement. The menisci and condyles were clearly delineated and the motions of the menisci during jaw opening were well evaluated in both normal and pathologic conditions. CONCLUSION: The dynamic TMJ MRI scan using fast FE technique permits both fast scanning and study of joint dynamics as well as TMJ anatomy.

T cell lymphoblastic lymphoma is characterized by immature lymphoid cells that are indistinguishable from the lymphoblasts and prolymphocytes of acute lymphoblastic leukemia. Several characteristic clinical features of lymphoblastic lymphoma, with include a high male-to-female ratio, a relatively high incidence in older children and young adults, the frequent presence of mediastinal involvement at the time of diagnosis. Also, this disease is rapidly progressive, and early dissemination to the bone marrow, blood, and central nervous system leads to the evolution of a picture resembling a acute lymphoblastic leukemia. We have experienced a rare case of congenital T-cell lymphoblastic lymphoma in which 1/365 year old female newborn had generalized multiple irregular protruding mass on her body. On bone marrow biopsy, CSF analysis, ultrasonogram and whole body MRI studies. We found metastasized tumor mass to, orbit, abdominal cavity, bulva, skin, and lower extremities. Histopathologically, specimen from mass on the right thigh showed diffuse infiltration of poorly differentiated and immature lymphoid cells in the skelectal muscle and subcutaneous soft tissue. By immunophenotyping studies using anti T cell and B cell monoclonal antibody, these cells were reactive with UCHL-1, MB2, but unreactive with MB1, L26. Based on these findings, the lesion was diagnosed as lymphoblastic lymphoma of the T-cell type, which occurred congenitally. Most T cell lymphoblastic lymphoma are noted, but congenital case was not reported. So we report it with a brief review of literature.

A gene p53, which is a tumor suppressor gene located on chromosome 17p, regulates cell growth and proliferation, and inactivation of this gene appears to be a common abnormality in many malignant tumors in human. To investigate the prevalence and patterns of p53 protein overexpression and the significance of the p53 protein overexpression as a marker for poor prognosis in transitional cell carcinoma of the bladder which were treated with radical cystectomy, fifty deparaffinized archival specimens from radical cystectomy for transitional cell carcinoma of the bladder were analyzed immunohistochemically to detect overexpression of p53 protein using the mouse monoclonal antibody DO-7 with a standard avidin-biotin immunoperoxidase method. The incidence of p53 expression was 17 of50 (34 per cent) specimens. The p53 expression was associated with histopathologic grade but not with T stage and the status of lymph node involvement. There was no evidence of high recurrence rate, metastasis rate or low survival rate in patients with p53 positive group compared to p53 negative group. But the patients with p53 positive group showed higher T stage and shorter mean survival time than those with p53 negative group. It was not statistically significant. Among many variables evaluated with multivariate analysis, T stage was a significant factor in terms of metastasis, while the status of lymph node involvement was a significant one in terms of survival. In conclusion, the incidence of p53 gene mutation appeared to be higher in invasive and high grade bladder cancer than in superficial and low grade ones. The p53 expression failed to show an unfavorable prognostic factor in transitional cell carcinoma of the bladder treated with radical cystectomy. Prospective studies of large cohorts of specimens are needed to evaluate nuclear overexpression of p53 protein as a prognostic marker in bladder cancer.
