1.Infantile Digital Fibromatosis: A Case Report.

We herein report a 4-year-old boy with infantile digital fibrornatosis developing on the distal & middle phalanx portion of the right index & ring fingers, and the middle phalanx portion of the right middle finger, which started at the age of 5 months after birth. Biopsy specimen taken from the right middle finger showed the proliferating collagen bundles and fibroblasts in the dermis and eosinophilic cytoplasmic inclusions within the fibroblasts. Although we attempted to treat tbe patient with intralesional injection of triamcinolone acetonide suspension and cryotherapy 5 times biweekly, there was no signifit effect.

4.Follow up Study of Second Look Laparotomy in Ovarian Cancer patients.

A study was to evaluate the efficacy and clinical utility of second look operation in the managements of malignant ovarian tumor at the Department of Obstetrics and Gynecology, Kosin medical colleage from January 1986 to September 1993. Of 220 patients diagnosed as ovarian cancer, 48 patients who had underwent primariy laparotomy were performed second look operation. Thirty-seven patients were found to have no evidence of disease. The result revealed that negative second look operation rate was noted 96.3% at stage I, 66.6% at stage II, 50.0% at stage III, and 42.9% at stage IV and total negative second look operation rate was 77.1%. The 2 year survival rate for stage I, II, III, IV were 85.2%, 66.6%, 50.0% and 42.9% respectively. The survival rate of negative second look operation was 86.5%, that of positive second look operation was 18.1%. In conclusion, second look operation in early stage ovarian cancer need to be justified.

7.The Effect of Acute Normovolemic Hemodilution on Cardiopulmonary Bypass in Coronary Artery Bypass Grafting Surgery.

BACKGROUND: Systemic arterial hypotension is relatively common following initiation of cardiopulmonary bypass (CPB). Decreased blood viscosity is induced by acute normovolemic hemodilution (ANH) and by the use of crystalloid oxygenator prime. The purpose of this paper is to study the effect of ANH on mean arterial pressure, perfusion flow index and PaO2/FiO2 upon initiation of CPB, and on homologous blood usage during CPB in coronary artery bypass grafting (CABG) surgery. METHODS: We reviewed 30 patients constituting an ANH group, and 30 patients in a control group who had undergone CABG surgery within the past 2 years. In the ANH group, 1 or 2 units of fresh autologous whole blood were sequestrated following induction of anesthesia. We compared mean arterial pressure, perfusion flow index, PaO2/FiO2, and hematocrit on the initiation of CPB, and homologous blood usage during and post CPB periods between the groups. RESULTS: Upon initiation of CPB, hematocrit in the ANH group was significantly less than in the control group, but there was no significant difference in mean arterial pressure, perfusion flow index or PaO2/FiO2 between the groups. The use of homologous blood during CPB in the ANH group was not significantly higher than in the control group. CONCLUSIONS: Acute intraoperative normovolemic hemodilution in CABG surgery was safely performed without significant hypotension and increased homologous blood usage during CPB.

8.Analysis of Urinary Stone Components during the Last Two Decades.

PURPOSE: The accurate analysis of urinary stone components is fundamental for studying of the etiology of stone formation and it is essential for the treatment of urinary stone and its prevention. We compared the analysis of urinary stone components during the last two decades. MATERIALS AND METHODS: Stone analysis was performed by Louis C. Herring and Company. The urinary stones were aobtained from January, 1986 to December, 2005. We compared the stone components of the first decade (Group A, 301 cases) with that of the second decade(Group B, 158 cases). RESULTS: The mean age was 47.2+/-14.5 in Group A and 46.0+/-10.2 in Group B(p=0.658). The ratio of males to females was 2.04:1 in Group A and 1.98:1 in Group B(p=0.888). Ureteral stones were the most common stones in both groups. Among all the components analyzed in Group A, calcium oxalate made up 73.1% of the total. Other components found in the stones were uric acid 12.3%, calcium phosphate 8.3%, magnesium ammonium 5.3%, ammonium acid urate 0.7% and cystine 0.3%. In Group B, calcium oxalate was only 56.3% of the total and uric acid was 22.2%, calcium phosphate was 11.4%, magnesium ammonium phosphate was 8.2%, ammonium acid urate was 1.3% and cystine was 0.6%. On comparison of the stones of the two groups, the incidence of calcium oxalate was decreased in Group A(73.1% vs 56.3%, p<0.001). However, the incidence of uric acid in Group B was increased(12.3% vs 22.2%, respectively, p=0.006). There were no notable statistical increases in the frequency of the other components(p>0.05). CONCLUSIONS: In this study, the incidence of calcium oxalate was decreased and that of the uric acid was increased in the recent decade. These changes of stone components probably resulted from the increased animal protein diet.

9.Cutaneous Plasmacytosis with Multiple Nodular Eruptions and Polyclonal Hypergammaglobulinemia.

We report two patients with multiple peculiar skin eruptions and polyclonal hypergammaglobulinemia. Both patients visited our hospital for the evaluation of asymptomatic multiple nodular eruptions on almost their entire body except for the lower extremities. Histologic examinations disclosed prominent infiltration of plasma cells and lymphoid follicular hyperplasia in the dermis but these plasma cells showed neither a mitotic figure nor atypicalities. Laboratory examinations showed polyclonal hypergammaglobulinemias and increased erythrocyte sedimentation rates. In spite of various investigations, the cause of the hypergammaglobulinemia remained obscure.

10.Diffusion Metrics as a Potential Prognostic Biomarker in Cervical Myelopathy

Diffusion tensor imaging (DTI) has a diagnostic value in cervical myelopathy. The usefulness of diffusion metrics as a potential prognostic biomarker was assessed in this review. Various diffusion metrics, such as fractional anisotropy, mean diffusivity, apparent diffusion coefficient, and radial diffusivity, were significant in correlation with functional recovery after surgery. Changes in diffusion metrics not only at the compression level, but also above and below the compression level were confirmed. However, it was difficult to confirm the results owing to the diversity of studies. Further efforts to make a consensus on spinal cord DTI are needed.
