Clinical observation was performed on 81 cases who were received exchange transfusion at Ilsin Christian Hospital from January, 1986 to Decrmber, 1990. The results obtained were as follows: 1) Among 6,388 cases of neonatal jaundice, the exchange transfusion was performed on 81 cases (1.27%). 2) There was no sex predominance (male 40, female 41). The birth weight of 75 cases(92.6%) ranged from 2,500 gm 4,000 gm. 3) The most predominant delivery type was normal vaginal delivery (66 cases, 81.5%). Instrumental delivery was 9 cases (11.1%) that was higher than control group (4.2%). 4) Analysing the exchanged 81 cases, sepsis accounted for 28 cases (34.6%) of the total exchanged group:ABO incompatibility 25 cases (30.9%), complicated causes 13 cases(16.0%), unknown causes 12 cases (14.8%) and Rh incompatibility 1 cases (1.2%). 5) Repeated exchange transfusion was required in 12 cases (16.0%). 6) In 20 cases (24.7%), exchange transfusion was performed on 7 days or more after birth. In 4 cases (4.9%), including 1 cases of Rh incompatibility, exchange transfusion was done within 24 hours after birth. 7) The most common organism of sepsis, which was the most predominant etiology of exchange transfusion was E. coli in 12 cases (31.5%). 8) After exchange transfusion, serum bilirubin, Hb and reticulocyte count were decreased by 24%, 22% and 36%, respectively.

Purpose: The study is examined the effects of the smoking cessation program of life skills training using flipped learning for male middle school students. Methods: The study was a nonequivalent control group non-synchronized design. The Smoking Cessation Program of life skills training using flipped learning is a Smoking Cessation Program that applied 5 stages of flipped learning such as before class, introductory class, during class, after class, and during work. Participants were 32 male students attending a middle school in D city, with 16 in the control group and 16 in the experiment group. Data collection was conducted from February 26, 2019 to May 14, 2019 for the control group, and from May 28, 2019 to August 19, 2019 for the experiment group. The collected smoking knowledge, smoking attitude, self-efficacy, basic psychological needs, and urine cotinine were analyzed by x2 test, independent t-test, two-way repeated measured ANOVA, and Fishers exact test using the SPSS 25.0 program. Results: The experimental group showed higher smoking knowledge, smoking attitude, self-efficacy, basic psychological need, and urine cotinine negative response than the control group, and there were significant differences. Conclusion These findings indicate that the Smoking Cessation Program of life skill training using flipped learning is an effective intervention for cessation of middle school male students, and it has a positive effect on the increase in urine cotinine, and a physiological indicator of the cessation effect, and is effective in the success of smoking cessation.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.

BACKGROUND: Herpes zoster is characterized by vesicular skin lesions over the unilateral sensory dermatomes being caused by the reactivation of varicella-zoster virus and its incidence seems to be increasing recently. OBJECTIVE: The purpose of this study was to elucidate the epidemiology and the clinical characteristics of herpes zoster. METHODS: The authors observed 877 cases of herpes zoster clinically for annual incidence, age, sex, monthly distribution, predilection site, associated disease, and complications at the Department of Dermatology in Chung-Ang University Hospital during a 10 years period(1982.9-1992.8). RESULTS: 1) The annual incidence averaged over 10 years was 2.26%(877 cases of total 38.717 outpatients) being on the increase recently. 2) The age distribution was in the range of 2-86 years old being most prevalent in the 6th decade and there was equal sex ratio(444:433). 3) There was no statistically significant monthly or seasonal variation in the incidence of herpes zoster. 4) The most common anatomical distribution was thoracic dermatome(51.7%), followed by trigeminal(17.1%), cervical(16.1%), lumbar (10.6%), sacral(1.4%)and multiple deramatomic involvement(3.1%). Left or right side was affected in about the same ratio(439:437), and bilateral involvement was in 1 case. 5) Associated disease of herpes zoster were observed in 210 patients(23.9%) ; hypertension(7.0%), diabetes mellitus(4.1%), tuberculosis(1.8%), postoperative status(0.7%), malignancy(0.7%), gastric ulcer(0.7%), and so on, 6) The most common complication of herpes zoster was postherpetic neuralgia(7.4%), followed by eye complication(2.1%), bacterial infection(1.4%), scar formation(0.7%), neurogenic bladder(0.2%), Ramsay-Hunt syndrome(0.2%), motor paralysis(0.1%), herpes zoster generalisatus(0.1%) 7) The incidence of postherpetic neuralgia increased with age and was highest in trigeminal dermatome. 8) Recurrence of herpes zoster was observed in 4 patients(0.5%). CONCLUSION: According to this study, the annual incidence of herpes zoster showed a tendency to increase recently but there was no significant monthly or seasonal variation. The anatomical distribution was most commom on the thoracic dermatome. The incidenc of postherpetic neuralgia increased with age being most common in trigeminal nerve.

Epidermolytic keratosis palmaris is a rare disease which shows clinical findings of Unna Thost keratoderma and histopathologic of epidermolytic hyperkeratosis. We report herein a case of epidermolytic keratosis palmaris et plantaris in a 16-month-old female baby. Light microscopy shows marked hyperkeratosis, large irregular keratohyalin granules, and large clear spaces in the granular and upper spinous layers. Eletron microscopic findings shows that the clear spaces are areas of cytoplasm filled with a fibrillar material and cellular organelles. Abnormal clumping of tonofilament and keratohyalin is also present.

With IT improvement, u-health system has been developed to link home and medical service systems by arranging medical data, decision making and medical guidelines for various situations. It can promote the therapeutic compliance for the patients with chronic diseases. And also for the medical service providers, it can provide useful data to make an accurate medical decision. As we know, the chronic disease is caused by inactivity and the management of chronic disease requires the modification of inactive life style. U-health medical fitness system can not only prescribe proper activity to control the chronic disease but also it can monitor the patients' physical activity in everyday life. By the successful management of chronic diseases based on u-health system, we want to achieve the expansion of disease-free and disability-free life expectancy and a solution to reduce the burden of medical expenses in modern society eventually.

A clinical observation was performed on the 80 cases of neonates born from diabetic mothers among the total of 34,936 neonates delivered at II Sin Christian Hospital from January 1985 to December 1989. The results were as follows: 1) The incidence of infants of diabetic mothers was 0.23%, among whom gestational diabetic mothers were 63 cases (78.7%) and overt diabetic mothers were 17 cases (21.3%). 2) Among the complications of pregnancy and delivery, preeclampsia was most frequent as 33.8%, following by urinary tract infection (13.8%), polyhydramnios (1.3%), diabetic ketoacidosis (1.3%). 3) According to the gestational age, prematurity was 11.3%, full term 78.7% and post term 10%. And according to the growth state, small for date infants were 11.3%, appropriate for date infants 53.7% and large for date infants 35%. 4) The incidnece of giant babies (over 4,000 gm) was 30.0% and sex ratio of male to female was 1.1:1 in infants of diabetic mothers. 5) The most frequent clinical manifestation was tachypnea (45.3%), following by poor feeding (20.0%), cyanosis (18.7%). 6) According to the laboratory findings, 53.7% showed hypoglycemia, 36.3% hyperbilirubinemia, 25% hypocalcemia, 15% polycythemia. 7) On the chest X-ray, there were pulmonary infiltration and hyaline membrane disease in 3.2% each other and pulmonary congestion and wet lung in 1.6% each other. 8) The incidence of congenital anomaly was 13.8% in 80 cases of infants of diabetic mothers, and congenital heart disease was most frequent as 3 cases.

PURPOSE: Benign Convulsions with mild Gastroenteritis(CwG) were first reported in Japan by Morooka in 1982. The condition is recognized as a distinct clinical entity. It is characterized by previously healthy infants and young children aged 6 months to 3 years having afebrile brief generalized tonic-clonic convulsions between the first and the fifth sick days of viral gastroenteritis usually in winter. The patients are less than 5% dehydrated and the seizures tend to occur repetitively over several days with no eleptic discharges in the interictal electroencephalograms. The tests for rotavirus antigens in stool are frequently positive and other laboratory results are normal including cerebrospinal fluid, serum electrolytes and blood glucose. Also, they are known to have a good prognosis. The study intends to compare the clinical symptoms of CwG with normal body temperature and with high body temperature because mild gastroenteritis can induce fever. METHODS: The subjects were 42 patients diagnosed as GwG, who were admitted to Soonchunhyang University Hospital from May 2003 to March 2006. A Cohort study was performed and their characteristics were examined in terms of frequency, sex, age, monthly distribution, body temperature, days in hospital, seizure tups and duration, number of convulsions in one gastroenteritis period, family history, past history, and other symptoms of gastroenteritis RESULTS: The Total number of patients with gastroenteritis in the period were 635 and only 42(6.6%) patients were diagnosed as CwG. The mean age was 17.1+/-4.7 months, and it commonly occurs in winter. the typical symptoms of gastroenteritis were vomiting and diarrhea with 34(81.0%) patients having both symptoms simultaneously. Less vomiting and more diarrhea were noticed after seizure. Rotavirus antigens were positive in 18(51.4%) patients, and the number of seizures was 2.0+/-1.3 times during one gastroenteritis period. There were found no statistical differences in age, sex, days in hospital, seizure types and duration in those groups with and without fever and of pisitive and negative rotavirus antigens. CONCLUSION: CwG was defined as nonfebrile convulsions with gastroenteritis until nowadays, but there are no statistical differences between febrile and nonfebrile CwG. Therefore, febr ile seizures with gastroenteritis can be included in the same category of CwG.
