1.An Immunohistochemical Study of the Relationships between Estrogen and Progesterone Receptors and Proliferating Cell Nuclear Antigen in Endometrial Hyperplasia and Adenocarcinoma.

Estrogen and progesterone receptors exist in the epithelial and stromal cells of the endometrium. Proliferative disorders of the endometrium may be associated with autocrine and paracrine actions of estrogen and progesterone in epithelial and stromal cells. This study was performed to evaluate the differences estrogen and progesterone receptor(ER/PR) expression in the epithelial and stromal cells of endometrial hyperplasias and adenocarcinomas using immunohistochemical methods. Immunohistochemical analysis of proliferating cell nuclear antigen(PCNA) was done to evaluate a possible correlation between PCNA and hormone receptor expression. Evaluation was based on samples from 31 simple hyperplasias, 30 complex hyperplasias, and 32 adenocarcinomas. The immunohistochemical expression of ER, PR and PCNA in epithelial and stromal cells were examined according to a scoring system based on the percentage of positive cells and the staining intensity. The results were as follows; 1) The expression of ER and PR in epithelial cells showed a graded, significant decreases in simple hyperplasia, complex hyperplasia and endometrial carcinoma, in that order(ER: P=0.008, PR: P= 0.026). 2) PR expression in the stromal cells showed a significant decrease between hyperplasia and adenocarcinoma(P=0.003). The difference in ER expression was not significant. 3) In stromal cells, the decrease in PR expression was more prominent than the decrease in ER expression when complex hyperplasia was compared to simple hyperplasia. 4) The PCNA expression in simple and complex hyperplasia and adenocarcinoma was not higher than the expression of PCNA in nomal proliferative endometrium. There was no significant difference in PCNA expression between simple and complex hyperplasia and adenocarcinoma(P=0.073). 5) A negative correlation between PCNA and ER/PR expression was not demonstrated in simple and complex hyperplasia, or in adenocarcinoma. Endometrial hyperplasia and adenocarcinoma are probably related to a paracrine action of estrogen and progesterone in epithelial and stromal cells. A progressive loss of PR expression in stromal cells may induce abnormal proliferation of endometrium due to a disrupted hormonal balance.

2.The effect of food habits on blood component profile and health condition.

The purpose of this study was to investigate the effect of food habits on blood components level(blood glucose, uric acid, hemoglobin, some lipids levels) and health condition in Taejon. The study was conducted with 250 subjects (male 130, female120) who cisited in a general hospital from May to June in 1997. Questionaires were used for food habits. The data were analyzed by using chi-square-test, t-test, ANOVA and multiple range test. Reuults were summerized s follow ; mean of Broca Index were 110.0+/-13.7% in male, 110.6+/-16.6% in female. Mean values of systolic blood pressure and diastolic blood pressure were 130.9+/-17.6mmHg, 76.2+/-12.6mmHg, respectively. The systolic blood pressure was significantly higher in man than woman, Mean serum total cholesterol, LDL-cholesterol, and HDL-cholesterol and triglyceride levels were 202.8+/-0.2, 121.1+/-5.5, 50.3+/-2.9, 157.1+/-12.6mg/dl, repectively. Triglyceride level was significantly higher in man than woman. But HDL-cholesterol was significantly lower in man than woman. And also, the value of LPH and AI were higher in man. So Man has more risk of atherogenesis than woman. Mean values of blood sugar, uric acid and hemoglobin was 102.7+/-.36mg/dl and 14.1+/-.56g/dl were higher in man than woman except blood sugar. Food habit of woman were better than it of man. The levels of education and income did not affect to food habit score. The most problem of habits were intake od salty food and lack of exercise. The intake of nutrients were higher as food habit score was higher. Intakes of energy, Ca, vitamin A and vitamin B1 were lower htan RDa's of nutrients in bad food habit group. This result did not show that blood components level and health condition were significantly different by food habits. But Broca Index and triglyceride levels were higher in bad food habit group than other groups, but not statistically significant. By this result, it is more needed food habit education to man and yong people. The education of food habits will include the real methods of weight control, low intak of salt and exercise to bad food habit group.

3.A Study of Dietary Intake, Physical Status and Biochemical Status of Children in Taejon.

This study was conducted to investigate the relationship among Rohrer index, and nutritional intake and biochemical status. We examined physical status, dietary intakes, serum cholesterol, blood pressure and other biochemical of children in Taejon. Thin survey was carried out in Octobe, 1995. The subjects were 362 children, aged from 6 to 11 years old. The results were summarized as follows. By Rohrer index, 4.5% of the subjects were lean, 70.2% were normal, 17.0% were overweight and 8.6% were obese. Systolic and diastolic blood pressures of the subjects were 111.73 and 69.88mmHg, respectively. The systolic blood pressure of obese subjects(119.93mmHg) was higher than any other groups. Total cholesterol level was 160.87mg/dl, cholesterol levels in each group were not significantly different. Hemoglobin level of total subjects was 13.10mg/dl, especially hemoglobin levels of 11 year-old girls was lower than that of the same aged boys suggesting that the girls should be supplied with more protein, iron and other nutrients. Energy and protein intakes wee 86.6% and 94.5% of RDA, respectively. The nutrients above the RDA were only two, phosphorus and-vitamin C. The intakes of iron, zinc and vitamin A were lower than 50% of the RDA's. The levels of zinc and niacin were highest in obese group than any other groups. The subjects in overweight and obese groups had eaten more calories from snack and less calories from breakfast than other groups. Rohrer index was correlated with cholesterol, systolic blood pressure and creatinine levels.

4.A Study on Nutrients Intake, Physical and Biochemical Status of Elementary School Children in Taejon.

The purpose of this study was to investigate nutrients intake, Rohrer index, serum cholesterol level, blood pressure and others for clinical and nutritional study on school children in Taejon. This survey was carried out in October, 1995. The subjects were 362 children, 6 to 11 years old. The results were summarized as follows. By Rohrer index, 4.6% of the children was lean, 69.6% was normal, 17.4% was overweight and 8.6% was obese group. Systolic blood pressure, diastolic blood pressure and skin fold thickness of all subjects were 111.7+/-15.7mmHg, 69.8+/-14.9mmHg and 14.5+/-7.3mm respectively. Systolic blood pressure and skin fold thickness were increased with Rohrer index. Total cholesterol, urea nitrogen and creatinine levels in blood were 160.8+/-23.9mm/dl, 11.5+/-3.5mm/dl and 0.71+/-0.12mm/dl respectively but not significantly different among groups. These levels have a slight tendency to increase in obese group but not significantly different among groups. Hematocrit and hemoglobin levels were 41.3+/-4.7%, 13.1+/-1.6g/dl. These levels of 11 years old girls were lower than same aged boys. Composition ratio of total energy intakes were 65% of CHO, 13% of protein and 22% of fat. Energy and most nutrients intakes were insufficient except for P and vitamin C. Especially the amount of vitamin A, Fe, Zn and folate intakes were lower than halves of RDA'S. Most nutrients intake were not significantly different among groups. Pearson' correlation in Rohrer index with nutrients intakes were all negative correlation significantly different of K, Na and Thiamin.

8.A Case of Vibrio alginolyticus Isolated from Otorrhea of Chronic Otitis Media.

Vibrio alginolyticus is a microorganism of marine environment that occasionally occurs as a human pathogen. We isolated V. alginolyticus from a patient with otitis media. A 37-year-old man had been exposed to seawater one month before admission. The isolate showed typical biochemical and characteristics of this organism such as positive Voges-Proskauer reaction, fermentation of sucrose, growth on 10% sodium chloride media. In vitro susceptibility test shows the isolate was resistant to ampicillin and carbenicillin, but was susceptible to other antimicrobial agents. The patient improved with ciprofloxacin and ofloxacin therapy.

9.Pontine Hemorrhage after Surgical Evacuation of Nontraumatic Subdural Hematoma.

Duret hemorrhage is characterized by an upper brainstem hemorrhage due to increased intracranial pressure by mass effect such as subdural hematoma or a brain tumor. The anteroposterior elongation and downward displacement of the upper brainstem by transtentorial herniation results in the compression and tearing of the paramedian perforating vessels that feed the upper brainstem tegmentum. The consequent hemorrhage that involves the upper brainstem renders recovery to be almost impossible. Following a tricuspid valve replacement surgery, a 56-year-old woman developed a left fronto-temporo-parietal nontraumatic subdural hematoma resulting in transtentorial herniation of the left mesial temporal lobe. A successful surgical evacuation of the hematoma was done with clinical improvement. Two days later, she was referred to neurology with an aggravated right side weakness, dysarthria, and a newly developed extraocular movement disturbance. A brain CT and MRI showed a pontine hemorrhage. We report a case of pontine hemorrhage, a Duret hemorrhage, after the surgical evacuation of nontraumatic subdural hematoma.

10.Relations Among Weight Control Behaviors, Health-related Lifestyles, and Diet Behaviors in Middle Aged Koreans.

In this study, we compared demographic anthropometric characteristic, health-related lifestyle and diet behavior among weight control behaviors of 1187 (555 male, 632 female) aged 40~69yrs in Ganghwa country. All the data were analyzed by chi-square test, trend test, student t-test using SPSS 12.0 version at p < 0.05. 'Attempting weight control (loss)' was more in women than that was found in men (36.6% vs 20.7%), and women attempting weight loss most were 40-50 yrs. The reasons of weight loss were 'health problem' and 'health promotion'. Physical activity and diet restriction were commonly employed as weight control methods. Both genders attempting weight loss had a higher education level, BMI, percentage of body fat, waist circumference and physical activity than those not attempting weight control (p < 0.05). In dietary habits like 'meal regularity', 'slow eating' and 'over eating', women attempting weight loss were superior than those who not attempting weight control group (p < 0.05). Eating pattern changes like 'decrease of fats and fatty foods intake', 'vegetable oil usage', 'increase of fruit and vegetables intake', 'decrease of sugar and salt intake' showed significant differences (p < 0.001) between the attempted weight control groups and nonattempted weight control groups. Salt taste was a preference in male non-attempted weight control group, while sour, hot and spicy taste were preference in female attempted weight control group (p < 0.05). Preference for processed foods, fried foods and snack were significant differences (p < 0.05) in women attempted weight control group. Those attempting weight loss tried to improve their eating patterns. However, those attempting weight loss were poorer than the others in health-related lifestyle and eating habit. Therefore, it is necessary to make an effort that improve healthrelated lifestyle and diet behavior in middle aged group.
