BACKGROUND: Urography(IVU) is considered the best first investigation in patient with suspected urinary calculi, but recently ultrasonography(USG), combined with a plain film of the abdomen, has been suggested as an alternative. METHODS: We undertaken study to see if this approch can be used in emergency patients and outpatients by radiologists with different amounts of ultrasound experence. Some 192 patients with suspected urinary colic presenting to Koo Hospital Emergency Department and Youngnam Universith Hospital outward Department(IM, URO, FM) over 12-month period were studied. They had a plain abdominal film(KUB) and USG examination of the kidney, ureter, bladder following hydration and subsequently underwent IVU. Of these, 22 patients passed a stone before their IVU. The data analysis was performed on the remaining 170 patients. Urography was used as the gold standard. RESULTS: Some 170 patients subsequently underwent IVU at a mean interval of 1.8 days after the ultrasound examination. In 91 of 170 patients the IVU was positive. In 97 of 170 patients the combination of JUB plus USG was positive, leaving three false negative KUB plus USG. Thus the sensitivity of KUB plus USG was 97%, specificity was 89%, positive predictive value was 91%, and negative predictive value was 96%. CONCLUSIONS: Our findings in this study suggest that in the hydrated patient the combination of KUB plus US is a very sensitive and relative specific screening test. Because of the high negative predictive value of KUB plus US, urography is not likely to be helpful when KUB plus US are negative. Urography is indicated only if KUB plus US findings are equivocal or if intervention is necessa.ry.

PURPOSE: The purpose of this study was to identify the attributes, antecedents, and consequences of nurse's work interruptions. METHODS: Walker and Avant's concept analysis method was used to analyze this concept. Relevant articles published before August 2018 were searched through MEDLINE, CINAHL, EMBASE, KISS, and RISS databases using “interruption,” “work or task,” and “nurse” as keywords. RESULTS: The attributes of work interruption by nurses were as follows: 1) new tasks to do; 2) cognitive transition of work priorities; 3) loss of work continuity; 4) tasks to be resumed. The antecedents of work interruption were intrusion of unplanned events, internal and external factors that result in nurses forgetting their original intentions, an unpredictable work environment, and cultural climate where interruptions are considered as a part of the work process. The consequences of work interruption were decline in job satisfaction, trigger of work errors, lengthening of work completion time, decline in work productivity, increase in work stress, and delay of transferring needed information in a timely manner. CONCLUSION: The results of this study provide basic data to reduce the negative consequences of nurses' work interruptions, and contribute to expanding the knowledge necessary for improving patients' safety and nurses' performance.

PURPOSE: Today's clinical nurses deal with complex problems that need accurate evidence for practice and decision making. In this study the effectiveness of an EBP education program was examined. METHODS: A pre-posttest design was used for this study and participants were 46 nurses working at a tertiary hospital located in Suwon, Korea. Date collection was done before and after the education program, from July 27 to October 2, 2015. Data were analyzed using paired t-test and ANCOVA with SPSS 21.0. RESULTS: There were significant differences in scores before and after the EBP education program for EBP readiness: belief (t=-5.65, p<.001), implementation(t=-2.89, p=.006), competence (t=-4.21, p<.001), and for evidence-based decision making (t=-16.25, p<.001) by the nurses. CONCLUSION: The findings indicate that the EBP education program has positive effects on EBP belief, implementation, competence and evidence-based decision making. In the future, it is necessary to reinforce the content of the program in the clinical workplace and to provide continuous education for clinical nurses.

Hemodialysis(HD)-associated hypotension is a frequent complication, but it is difficult to manage. Until now, several maneuvers have been tried to prevent the HD-associated hypotension. Of these, the sodium content of dialysate was regarded as an important factor for maintaining blood pressure during HD. In this study, we evaluated the effect of hypertonic dialysate on blood pressure, interdialytic weight gain and the incidence of thirst. The study was done for 6 weeks successively with 3 different groups. Each patient was dialysed with 3 different dialysates for 2 weeks: Group I(Conventional HD: sodium concentration: 137 mEq/L), Group II(Hypertonic HD: 147 mEq/L) and Group III (Sequential HD: from 147 to 140 mEq/L). Hemodynamic parameters(blood pressure, pulse rate and ultrafiltration rate), biochemical parameters(hematocrits, blood urea nitrogen, creatinine, osmolality, sodium, potassium, chloride, fasting blood sugar) and complications (interdialytic weight gain &thirsty sensation) were compared among 3 groups. The results were as follows: 1. Decline of systolic blood pressure and diastolic blood pressure at the time of a 3 hour check during hemodialysis was lower in the Group II than Group I and III (p=0.002; p=0.012). and decline of diastolic blood pressure at the time of a 4 hour check during hemodialysis was lower in the Group II and III than Group I(p=0.04). 2. ncidence of hypotensive episodes during dialysis was significantly lower in Group II than group I (p=0.0287). 3. The ultrafiltration in Group III at the time of 1 hour, 2 hour and 3 hour check during hemodialysis was higher than that in Group I and II at the time of 1 hour, 2 hour and 3 hour check during emodialysis respectively (p=0.0001; p=0.0001; p=0.0004). 4. Interdialytic weight gain was higher in Group II(3.1+/-0.8) than GroupI(2.8+/-0.8) and III (2.9+/-0.9) (p=0.0422). 5. Hematologic and biochemical results were not significantly different among 3 Groups. 6. Frequency of thirst was different in Group I, II and III, 0.05+/-0.12, 0.41+/-0.24 and 0.22+/-0.29 respectively (p=0.0259). The results suggest that hypertonic HD was effective in preventing HD-associated hypotension but interdialytic weight gain and thirst sensations were increased as compared with a conventional method. In this situation, sequential HD seems to be an alternative method to minimizes the side effect of hypertonic HD.

PURPOSE: The purpose of this study was to construct and test a structural equation modeling on the reproductive health behavior of single women with sexual experiences. This study employed Ajzen's Theory of Planned Behavior (TPB). METHODS: The data were collected after receipt of consent from 250 single women with sexual experiences, and analyzed using SPSS 18.0 and AMOS 18.0. RESULTS: Model fit indices for the hypothetical model were suitable for the recommended level: x²=362.407, RMR=0.065, RMSEA=0.070, GFI=0.867. TLI=0.927, CFI=0.938, IFI=0.939, and x²/dF=2.237. Intention showed direct effect with the biggest effect being on reproductive health behavior. Attitude, subjective norm and perceived behavioral control were found to have a direct effect on intention. Among them, perceived behavioral control revealed the largest influence. CONCLUSION: This study suggests that the TPB is a suitable model in explaining the reproductive health behavior of single women with sexual experience. Strategic plans for educational and intervention programs should be aimed to encourage single women to engage in reproductive health behavior.

PURPOSE: The purpose of this study was to develop a tool for measuring the intra-operative caring as perceived by regional anesthesia patients. METHODS: A preliminary instrument with 54 questions, based on a literature review and semi-structured interviews with 11 regional anesthesia patients, was developed. A group of experts revised individual questions, and 34 questions were finally selected. This tool was tested with 137 regional anesthesia patients admitted to D university hospital in B city from August to October 2008. RESULTS: The validity and reliability of the tool were tested using factor analysis. After item analysis, one question with a correlation coefficient under .30 was discarded After performing factor analysis on the final 33 questions, 7 factors were identified; holistic needs fulfillment, consideration, protective environment, cautiousness, concern, information, physical comfort. The total variance shown in the test was 73.5%. A Cronbach alpha of 0.96 showed the reliability of the instrument. CONCLUSION: Results of this study indicate that the tool is statistically reliable and valid to measure the intraoperative caring perceived by regional anesthesia patients. This tool can be utilized usefully in assessing the effects of nursing interventions for regional anesthesia patients.

Purposes: This study was designed to explore the stage distribution of subjects according to stage of change for calcium intake and for exercise, and to identify factors that could discriminate among subjects in various stages. METHODS: The sample consisted of 142 subjects who had taken bone mineral densitometry tests. The instruments used in this study were the Stage Placement Instrument for Calcium Intake and Exercise, the Osteoporosis Health Belief Scale and the Osteoporosis Knowledge Test, and the Osteoporosis Self Efficacy Scale. Data were analyzed using chi square, ANOVA, and discriminant analysis by using the SPSS 12.0 program. RESULTS: For calcium stages, economic level, calcium knowledge, positive social norms for calcium intake, & educational level showed high standardized canonical discriminant function coefficients. For exercise stages, exercise efficacy, susceptibility, exercise benefit, educational level, positive social norm to exercise, educational level, and exercise barrier showed high standardized canonical discriminant function coefficients. CONCLUSION: This study implies that bone mass promoting program incorporating a stages of change model can be applied as useful nursing intervention.

PURPOSE: The purpose of this study was to assess the characteristics of the user of complementary?alternative therapies(CAT) and to identify the important predictive factors associated with them. METHOD: This study included 142 patients attending outpatient rheumatology clinics of D Hospital in Busan between July and August in 2001. The multiple logistic regression model was developed to estimate the likelihood of user or nonuser of CAT. RESULT: The duration of illness and chance score of health locus of control were found to be significant factors through the estimated coefficients of using CAT. Duration of illness is longer and chance score of health locus of control is higher in patients who have used CAT in past than that of nonuser. When the model performance was evaluated by comparing the observed outcome with predicted outcome, the model correctly identified 95% of user of CAT and 31% of nonuser. CONCLUSION: In this survey, duration of illness and chance score of health locus of control are found to be significant factors in predicting utilization of CAT. Nurses who care for rheumatoid arthritis patients should take consideration into health locus of control in planning health education programs.

Colorectal cancer (CRC) is 2nd to 3rd common cancer and the annual incidence of CRC is increasing in Korea. The liver is the most frequent metastatic site of colorectal cancer and approximately one forth of patients presents with liver metastasis at initial diagnosis. Without treatment, patients with colorectal liver metastasis (CRLM) have a poor prognosis; however, long-term survival benefits and even cure have been reported in a portion of patients undergoing surgical resection of liver metastasis. In addition, advances in chemotherapeutic agents, imaging, and surgical techniques can increase the number of patients who are eligible for curative resection. In the past 10 years, combination chemotherapy with 5-fluorouracil (5-FU)/leucovorin (LV) and oxaliplatin or irinotecan was becoming the standard chemotherapy of treatment for metastatic CRC including CRLM. Furthermore, the combined use of targeted agents, such as cetuximab and bevacizumab, plus standard chemotherapeutics revealed more improvements in response rates and survival. Even in patients with resectable CRLM, perioperative chemotherapy with surgical resection could improve in progression free survival. However, the curable portion of patients with CRLM was still less than 20~30%, more detail management based on multidisciplinary team approaches should be needed.
