1.Airway Resistance after Exercise Loading Test in Asthmatic Children.

Exercise induced asthma (EIA) is defined as acute, reversible, usually self-limiting airway obstruction which develop after strenous exercise in patients with asthma. EIA is a bigger problem in children than adults due to more active and smaller airways which obstruct more easily when bronchospasm develops. The exercise elicited many change in pulmonary function of asthmatic children. These change could be measured by measurements of PEFR, FEV1, FVC, or SRaw. The propose of this study is to determine the changes and sensitivity of different pulmonary function test in asthmatic children. 37 asthmatic children exercised on free running and pulmonary function tests were performed before and every 5 minutes during 15 minutes after exercise. The results were as follows. 1) In 37 asthmatic children, EIA developed in 23 children (62%) and did not developed in 14 children (38%). The difference in sex, height, weight, total eosinophil count, serum IgF and test between EIA and Non-EIA were not significant (P>0.05). 2) In EIA group, %PEFR was changed from 109.56% to 83.56%, But in Non-EIA group, %PEFR was not changed before and after exercise loading test. 3) In EIA group, %FEV1 was changed from 90.91% to 74.87%. But in Non-EIA group, %FEV1 was not changed before and after exercise loading test. 4) In EIA group, %FVC changed from 91.83% to 75.35%. But in Non-EIA group, %FVC was not change before and after exercise loading test. 5) There was significant difference in pre-exercise specific airway resistance between EIA (6.7+/-1.7) and normal children (5.8+/-2.2)(p<0.05). In EIA group, %SRaw was changed from 115.7% to 161.2%. But in Non-EIA group, %SRaw was not changed before and after exercise loading test. 6) During the post-exercise observation period, increasement of SRaw was greater than changes in any other pulmonary function test. SRaw is the most sensitive test for the ditection of exercise induced airway obstruction is asthmatic children.

2.A Case of Congenital T Cell Lymphoblastic Lymphoma.

T cell lymphoblastic lymphoma is characterized by immature lymphoid cells that are indistinguishable from the lymphoblasts and prolymphocytes of acute lymphoblastic leukemia. Several characteristic clinical features of lymphoblastic lymphoma, with include a high male-to-female ratio, a relatively high incidence in older children and young adults, the frequent presence of mediastinal involvement at the time of diagnosis. Also, this disease is rapidly progressive, and early dissemination to the bone marrow, blood, and central nervous system leads to the evolution of a picture resembling a acute lymphoblastic leukemia. We have experienced a rare case of congenital T-cell lymphoblastic lymphoma in which 1/365 year old female newborn had generalized multiple irregular protruding mass on her body. On bone marrow biopsy, CSF analysis, ultrasonogram and whole body MRI studies. We found metastasized tumor mass to, orbit, abdominal cavity, bulva, skin, and lower extremities. Histopathologically, specimen from mass on the right thigh showed diffuse infiltration of poorly differentiated and immature lymphoid cells in the skelectal muscle and subcutaneous soft tissue. By immunophenotyping studies using anti T cell and B cell monoclonal antibody, these cells were reactive with UCHL-1, MB2, but unreactive with MB1, L26. Based on these findings, the lesion was diagnosed as lymphoblastic lymphoma of the T-cell type, which occurred congenitally. Most T cell lymphoblastic lymphoma are noted, but congenital case was not reported. So we report it with a brief review of literature.

3.The Effects of Trendelenburg Position and Intraabdominal CO2 Gas Insufflation on Cardiopulmonary System during Pelviscopic Surgery under General Anesthesia.

BACKGROUND: Pelviscopic techniques have rapidly increased in therapeutic procedures as well as diagnostic procedures because of the many benefits associated with much smaller incisions than traditional open techniques. But the deliberate pneumoperitoneum with carbon dioxide during pelviscopic surgery may cause some problems-hypercarbia, pneumomentum, subcutaneous or mediastinal emphysema, pneumothorax, hypoxemia, hypotension, cardiovascular collapse and cardiac dysrhythmia. METHOD: We observed the changes of blood pressure (systolic, mean, diastolic), pulse rate, PaCO2, PaO2, peak inspiratory airway pressure and expired tidal volume at 10 minute after induction of general anesthesia (control value), 30 minutes and 60 minutes after insufflation of CO2 and Trendelenburg position. RESULTS: The blood pressure, PaCO2 and peak inspiratory airway pressure were increased significantly than control values (p<0.05). The changes of pulse rate and expired tidal volume were not statistically significant in comparison to control values. The PaO2 was decreased significantly (p<0.05). CONCLUSION: To minimize the risk of CO2 retension and unstability of cardiovascular system during pelviscopy under the Trendelenburg position, we must monitor the vital signs and the arterial blood gas status continuously and carefully.

4.Effects of Soy Isoflavones Supplementation and Exercise on Urinary Calcium, Magnecium, Copper and Zinc Excretion in Postmenopausal Women.

This study assessed the effects of soy isoflavones supplementation with exercise on urinary mineral (calcium, magnesium, copper, zinc) excretion as an index of bone resorption rates in 67 postmenopausal women. A total subjects were assigned to Isoflavone (90 mg/day) or placebo groups. These groups were further divided into groups that undergone a regular exercise or a rather sedentary state performing daily activity only. We conducted study eight week period. Result showed urinary zinc excretion was more significantly decreased in the isoflavone-sedentary group (-180.76 +/- 171.30 ug/day) than in the placebo-sedentary group (-31.23 +/- 146.60 ug/day), placebo-exercise group (40.93 +/- 193.44 ug/day) and isoflavione-exercise group (-1.21 +/- 160.61 ug/day) (p < 0.05), but no significant changes in the differences between the values of the pre and post study values in urinary calcium, magnesium and copper excretion. These results suggest that Isoflavone supplementation decrease urinary zinc excretion rate in postmenopausal subjects.

5.The Assessment of Foodservice Management Practices in Elementary School Foodservices.

The aim of this study was to evaluate the foodservice management practices in elementary school in order to provide basic information for an efficient foodservice management. A self completed questionnaire survey of 912 schools in Korea was undertaken. Survey questionnaires consisted of 4 parts including general background, foodservice practices, purchasing practices and the status of cooking facilities and equipments. The main results of the empirical survey were as follows. 1. Elementary schools categorized by location were in urban areas (38.2%), provincial areas (48.0%) and in isolated areas (13.8%). 2. Conventional schools were 72.4% of the total followed by commissary management schools(23.5%) and commissary schools(4.1%). 3. The major factors considered for menu planning were nutritional adequacy(77.4%), preference of children(50.8%), food cost(46.0%), and cooking facilities & equipments in order. 4. The average cost of school lunch in provincial areas was 1494.8+/-224.9 won followed by in isolated areas (1337.0+/-793.3won) and in large cities (1271.4+/-308.1won). The average food cost of lunch was approximately 950won with no significant difference by location of schools. 5. 60% processed food were purchased through retailers in schools located in provincial and isolated areas, whereas 6.6% in schools located in urban areas. Food groups such as meat and its processed food, poultries, fish & shellfish, fruits and vegetables were more purchased through wholesalers in schools located in urban areas than in provincial and isolated areas. 6. Cereals including rice were regularily purchased once a month, whereas of other food items were found to be daily purchased in school located in urban and provincial areas.

6.Estrogen Pretreatment of Organotypic Hippocampal Slices Protects Neurons against Oxygen-Glucose Deprivation with Akt Activation.

In several experimental models, estrogens protect neurons against ischemic insults. However, the recent clinical studies of hormone replacement showed negative results to prevent stroke. Therefore, optimal models to study estrogen replacement for neuroprotection are needed before its clinical application. Organotypic hippocampal slice under oxygen-glucose deprivation (OGD) has been established as a model of cerebral ischemia and has advantages to study drug effects. We investigated whether estrogen protected CA1 neurons and affected activation of Akt (pAkt) in CA1 region under OGD. Thus, rat hippocampal slices on day 7 of culture were treated with 17-beta estradiol (E, 1 nM) for 7 days before 30 min OGD, and cell death of CA1 neurons was quantified by propidium iodide (PI) staining and expression of pAkt was studied by Western blot and immunofluorescence. PI intensity in slices treated with E was significantly reduced 72 hour after OGD compared to that of non-treated slices (p<0.05). E pretreatment also increased the expression of pAkt 72 hour after OGD compared to that of no treatment (p<0.01). These data suggest that estrogen pretreatment may rescue neurons from ischemic insults through the activation of Akt and also indicate that our model would be a useful alternative method to study the mechanisms and effects of estrogen replacement treatment for neuroprotection.

7.Narrative Exploration of Infertile Nurses' Experience of Natural Abortion.

PURPOSE: This study was done to explore deeply the essence and meaning of abortion related experience of nurses. METHODS: Data were collected through in-depth informal interviews, observation, and field records with two nurses from July 1 to July 30, 2015. Data were analyzed using narrative inquiry methodology. RESULTS: Three fundamental themes were derived from data analysis: 'Considering women with abortion-experience as women to be nursed', 'to be absolutely understood', and 'to make us to think how to help'. CONCLUSION: It is significant that this study was carried out holistically on the experience and change in physical, emotional, social, and spiritual aspects of nurses with abortion-experience, simultaneously interpreting from the prospective of nurses. Therefore the basis of nursing interventions are suggested; holistic aspects as education, counseling, group meetings, and program assistance for the women, the families, and those concerned with abortion- experience.

8.Intraductal Variant of Peripheral Cholangiocarcinoma of the Liver A report of three cases.

Intraductal variant of peripheral cholangiocarcinoma is extremely rare. This variant shows intraductal growth and intraluminal extension without any infiltrative growth. The mode of intraductal growth is not known. The prognosis of this variant is better than that of usual cholangiocarcinoma. We report three cases, one of which is associated with Clonorchis sinensis (CS) infection. The tumors were entirely confined within the dilated peripheral tributaries of the intrahepatic bile duct. Microscopically, the tumors were well to moderately well differentiated, with a papillary or a micropapillary growth pattern. Focal clear cytoplasmic change and mucin production were noted. The tumors showed intraductal spreading without any invasion to the liver parenchyme. Mucosal hyperplasia and dysplasia were noted in the adjacent ducts. The authors assume that intraductal cholangiocarcinoma is a distinct subtype, and persistent irritation, such as, CS infection may undergo a malignant transformation through mucosal dysplasia.

9.Prevalence Rates of and Related Factors to Constipation in Elementary School Students.

BACKGROUND: This study was conducted to explore the prevalence of and the factors affecting constipation in elementary school students in urban and rural areas. METHODS: Study subjects were 585 grade 5 and 6 students. We conducted self-administered questionnaires during October, 2009. Outcome measure was the prevalence of constipation evaluated by the Rome III Diagnostic criteria for functional constipation. Independent variables were gender, grade, residence, mother's employment status, self-rated health status, number of family members, bowel movement habit, and health behavior (nutrition, exercise, and stress). RESULTS: The multiple logistic regression analysis showed that females, grade 5 students, those with working mothers, and those physically inactive reported having constipation significantly more often. CONCLUSIONS: Our study revealed that the female gender, increasing age, mother's employment status, and physical inactivity were risk factors for constipation in elementary school students. To improve bowel function in children, health education programs and school health promotion programs targeting healthy life habits should be implemented.

10.Dermatophagoides farinae specific IgG-subclass antibody in allergic children.

For determining the distribution of specific IgE and IgG-subclass antibodies and their role in allergic disorders, we measured the serum levels of specific IgG-subclass and IgE antibodies to Dermatophagoides farinae (Df) on 60 atopic children, aged between 8 to 12. All of them had positive skin test to Df, and was grouped into three: children with asthma only (group 1), with rhinitis (group 2), and with them together (group 3). Every data were compared with that of 47 nomal children. ELISA (for IgG and IgG-subclass) and RIA (for IgE) were used. The results are as following. 1) Specific IgE-antibody is significantly higher in all three allergic groups, compared with normal control group. But there was no significant difference between three allergy groups. 2) Specific IgG, IgG1, IgG2 IgG4-antibodies were higher than those in normal control group, but IgG3 was not significant. IgG1 and IgG2 were elevated particularly in patients with asthma and rhintis, but IgG4 was elevated in asthma group. 3) The linear correlation between specific IgE and IgG1-antibody was significant (r=0.286), but IgG4 did not have this correlation with IgE. In conclusion, the levels of specific IgE is essential for the diagnosis of allergies, and IgG1 and IgG4 seem to participate in allergic reactions with a different mechanism.
