1.Ultrasonographic findings of the pelvic masses

Ultrasonography is most commonly utillized diagnostic tool in obstetric and gynecology for the evaluation ofpatient with a pelvic mass or pregancy. For it is characterized by no radiation hazard, noninvasive examinationand high diagnoastic accuracy. Also it affords an accurate assessment of the presence, size, location and internalconsistency of a pelvic mass. The recent availability and improved resolution of realtime scanning have afforded amore flexible and complete approach to evaluation of normal and abnormal structures in the pelvis. We analyzedultrasonographic findings in 154 pathologically proven cases of pelvic mass examined at Korea General hospitalfrom Jan. 1983 to Apr. 1984. The results were as follows; 1. The age distribution was from 12 years to 66 yearsand the majority of patients were between the ages of 21 and 50 yeasrs (91.4%). 2. The incidence of pelvic masswas 27.9% in uterine leiomyoma, 22.7% in ovarian cyst, 13.0% in addenomyosis and 8.4% in serous cystadenoma. 3. Mild to moderately echogenic nodular uterine enlargement with some cystic change(81.5%) of leiomyoma and multiplesmall vesicular pattern of intrauterine contents with uterine enlargement of Homole were the most common ultrasonographic findings. The location and type of leiomyoma were most common in the fundus and body (95.3%), and intramural myoma(53.5%). The most frequent finding of ovarian teratoma was cystic mass with echogenic focus(41.7%) but the echogenic appearance of the lesions was extremely variable. The ultrasonographic findings ofectopic pregnancy were cystic or complelx adnexal mass(80.0%), with or without fluid in cul-de-sac and deviationof uterus by adenxal mass. 4. Accuracy of ultrasonography in detemining the overall correct diagnosis of thepelvic masses compared with proven diagnosis was approximately 61.7%. The diagnostic accuracy was 90.7% inluterine leiomyoma, 100% in H-mole nad 80.0% in ectopic pregnancy.

3.The Influencing Factors of Influenza Vaccination in the Elderly Participating in Lifetime Transitional Health Examination.

PURPOSE: The purpose of this study was to investigate the influencing factors of influenza vaccination in the elderly participating in lifetime transitional health examination. METHODS: This study was a secondary analysis of data collected from lifetime transitional health examination (for 66-year-old people) conducted by the National Health Insurance Corporation (NHIC) from January 1 to December 31, 2008. Questionnaires were received from NHIC to obtain information regarding gender, chronic diseases, health-related behaviors, and ADL. A total of 255,333 participants who responded all the questions in the questionnaire were included in the analysis. Collected data were analyzed by descriptive statistics, chi2 test, and multiple logistic regression. RESULTS: The influenza vaccine coverage rate in 66-year-old people was 66.1%. The influenza vaccine coverage rate was higher in female elders and those with hypertension, diabetes, heart disease or past smoking, and lower in those with stroke, current smoking, drinking, no-exercise or ADL-dependency. CONCLUSION: Strategies for improving the influenza vaccination coverage rate in the elderly are needed. The strategies should give priority to the elderly with current smoking, drinking, and no-exercise, and home visiting programs are needed for the elderly with stroke and ADL-dependency.

4.The Effect of Onion Peel Extract on Inflammatory Mediators in Korean Overweight and Obese Women.

Quercetin, found abundantly in onion peel, has been known to have antioxidant and anti-obesity effects and improves endothelial function. The purpose of this study was to evaluate the effects of a quercetin-rich onion peel extract (OPE) on the inflammatory mediators in overweight and obese women. This study was a randomized double-blind, placebo-controlled study. Thirty-seven healthy overweight and obese women were randomly assigned to two groups, and one group was given a soft capsuled OPE (100 mg quercetin/day, n = 18) and the other group a same capsuled placebo (n = 19) for 12 weeks. Fat mass was measured by bioimpendance method at baseline and after 12 weeks of intervention. The levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured with colorimetric assay kits. The concentrations of leptin, adiponectin, visfatin, tumor necrosis factor (TNF)-α and interleukin (IL)-4 in plasma were determined by using enzyme-linked immunosorbent assay kits. Baseline characteristics of anthropometric indicators and blood metabolic profiles were not significantly different between placebo and OPE groups. Compared with baseline value, both placebo and OPE supplementation significantly decreased the percent of body fat mass and induced plasma adiponectin levels while ALT and AST activities as well as leptin, visfatin, TNF-α, and IL-4 levels in plasma were not significantly different between two groups after 12 weeks of the supplementation. These findings suggest that 12-week supplementation of OPE do not affect modulators of systemic inflammation in overweight and obese women.

5.Antioxidative Activity of Onion Peel Extract in Obese Women: A Randomized, Double-blind, Placebo Controlled Study.

BACKGROUND: Quercetin, found abundantly in onion peel, has been known to have anticholesterol, antithrombotic and insulin-sensitizing properties. Here, we investigated the effect of quercetin-rich onion peel extract (OPE) on reactive oxygen species (ROS) production and antioxidative defense in obese woman. METHODS: This study was randomized, double-blind, placebo controlled study. Thirty-seven healthy obese participants were randomly assigned that eighteen subjects received red soft capsuled OPE (100 mg/d, 50 mg bis in die), while the other nineteen subjects received same capsuled placebo for 12 weeks. ROS production and superoxide dismutase (SOD) activity in plasma were determined by using ROS and SOD assay kits, respectively. RESULTS: Baseline characteristics of anthropometric indicators and blood metabolic profiles were not significantly different between the two groups. Compared with baseline values, OPE consumption significantly reduced waist and hip circumference. Plasma ROS level and SOD activity were decreased in both placebo and OPE groups compared with baseline values. However, plasma ROS level in OPE group was significantly lower than in placebo group while plasma SOD activity in OPE group was significantly higher than in placebo group after 12 weeks of consumption. CONCLUSIONS: These findings indicate that OPE consumption may exert antioxidative effect by preventing the decrease of SOD activity as well as the production of ROS in obese women.

7.A Comparative Clinicopathological Study of Inverted Follicular Keratosis and Well-Differentiated Squamous Cell Carcinoma

Background:
Inverted follicular keratosis (IFK) is a benign tumor that occurs mainly as a single lesion in the head and neck. Histologically, the tumor lobules show endophytic or exophytic growth and are characterized by squamous eddies. IFK should be differentiated from seborrheic keratosis, verruca vulgaris, and squamous cell carcinoma (SCC).In particular, differentiating from well-differentiated SCC can be difficult when downward growth is observed along with some mitotic figures and inflammatory cell infiltration.
Objective:
To evaluate and compare the clinical and histopathological characteristics of IFK and well-differentiated SCC.
Methods:
We retrospectively reviewed the clinicopathological records of 21 patients diagnosed with IFK and 21 randomly assigned patients diagnosed with well-differentiated SCC between 2000 and 2022 at the Dermatology Department of the Yeungnam University Medical Center.
Results:
IFK occurs frequently on the head and neck of middle aged and older adults, and its average size is less than 1 cm. Acantholysis was observed in varying degrees in IFK; however, well-differentiated SCC was mostly absent (17 cases) or mild (three cases) showing a statistically significant difference. Squamous eddies were observed in 21 cases of IFK and eight of well-differentiated SCC. The average number of dyskeratotic cells and mitotic counts did not differ significantly between IFK and well-differentiated SCC.
Conclusion
We suggested some evidence for the irritant origin of IFK. We also compared the clinicohistological findings of IFK with those of well-differentiated SCC and concluded that excluding atypical cells, abnormal mitotic figures, and irregular invasive borders is important for differential diagnosis.
8.Impact of early-life weight status on urinary tract infections in children: a nationwide population-based study in Korea

We aimed to evaluate the association between early-life weight status and urinary tract infection (UTI) risk in children.
METHODS:
A nationwide study was conducted using Korean National Health Screening (NHS) data and National Health Insurance Service data. A sample cohort was selected using data from the 2014 and 2015 NHS for infants and children (4-71 months) and followed up until the end of 2017. Participants were divided into 4 groups (underweight, normal weight, overweight, and obese) based on the weight-for-age (< 2 years) or body mass index (≥ 2 years). Hazard ratios (HRs) with 95% confidence intervals (CIs) for developing UTIs, cystitis, and acute pyelonephritis (APN) were calculated using a Cox proportional hazard model.
RESULTS:
Of 1,653,106 enrolled children, 120,142 (7.3%) developed UTIs, cystitis, and APN during follow-up. The underweight, overweight, and obese groups had higher risks of UTIs than the reference group after adjusting for age, sex, birth weight, and preterm birth. Between 2 years and 6 years of age, boys with underweight had a high risk of UTI and APN, while girls with overweight and obesity revealed elevated risks of UTIs, cystitis, and APN. The HRs for APN in boys with underweight and in girls with obesity were 1.46 (95% CI, 1.03 to 2.07) and 1.41 (95% CI, 1.13 to 1.75), respectively, after adjusting for age, sex, birth weight, and preterm birth. The incidence of APN did not decrease with age in underweight and obese children aged 2-6 years.
CONCLUSIONS
Children with underweight, overweight, and obesity may be at high risk for UTIs.

10.Effect of glass-infiltration treatments on the shear bond strength between zirconia and ultra low-fusing porcelain veneer.

PURPOSE: This study examined the effects of glass infiltration treatments on the shear bond strength (SBS) between zirconia core and ultra low-fusing porcelain veneer. MATERIALS AND METHODS: The zirconia specimens were classified into 4 groups (n = 12): Untreated zirconia (group Z), zirconia coated ZirLiner (group ZL), glass-infiltrated zirconia (group ZG), glass-infiltrated and sandblasted zirconia (group ZGS). A cylinder of ultra low-fusing veneer porcelain was build up on each disk (6 mm × 3 mm). SBS was measured using a universal testing machine. Scanning electron microscope and Energy Dispersive X-ray spectroscopy were used to evaluate the surface of zirconia and failure pattern after SBS. RESULTS: SBS value of group ZGS was significantly lower than that of other groups (P < .05). No significant differences were detected among group ZL, group Z and group ZG. CONCLUSION: Glass infiltration is not effective to the bond strength between zirconia and ultra low-fusing porcelain veneer. Sandblasting also dramatically decreased the bonding strength.
