Background: Umeclidinium/vilanterol (UMEC/VI; ANORO ELLIPTA, GSK) is a commonly used dual bronchodilator. This study evaluated the safety and effectiveness of UMEC/VI in Korean patients with chronic obstructive pulmonary disease (COPD) over a 6-year period. Methods: This was an open-label, multicentre, observational, post-marketing surveillance study. A total of 3,375 patients were enrolled consecutively in 52 hospitals, by 53 physicians, between July 2014 and July 2020. Patients who were administered UMEC/VI (fixed-dose 62.5 μg/25 μg) at least once and were monitored for safety and effectiveness were included in the analysis. Incidence and severity of adverse events (AEs) reported after administrating at least one dose of UMEC/VI were monitored, including unexpected adverse events (UAEs) and adverse drug reactions (ADRs). Effectiveness of UMEC/VI after 24 weeks of administration was also assessed using physician’s evaluation (effective, ineffectiveo change, worsening, indeterminable) and lung function improvement. Results: Of 3,375 patients, 3,086 were included in the safety assessment group (mean age±standard deviation: 69.76±8.80 years; 85.9% male [n=2,652]; 73.1% aged ≥65 years [n=2,255]). The overall incidence of AEs was 28.8% (n=890), of which 2.2% (n=67) were ADRs. Serious AEs and UAEs were reported in 181 (5.9%) and 665 (21.6%) patients, respectively, and two patients (<0.1%) reported unexpected severe ADR. Of the 903/3,086 patients analysed for effectiveness, most (82.8%, n=748) showed overall disease improvement after UMEC/VI treatment. Conclusion This study confirmed UMEC/VI administered to Korean patients according to the prescribing information was well-tolerated and can be considered an effective option for COPD treatment.

BACKGROUND: The purpose of this study was to analyze the relationships between cardiorespiratory fitness and body mass index (BMI), metabolic syndrome risk factors, homeostasis model assessment-insulin resistance (HOMA-IR), and high sensitivity C-reactive protein (hsCRP) in male high school students in Korea. Furthermore, the effects of the cardiorespiratory fitness levels on these four factors were examined. METHODS: The level of cardiorespiratory fitness, BMI, percent body fat, waist circumference (WC), systolic blood pressure, glucose, triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), HOMA-IR, and hsCRP of 227 male high school students were measured. Analyses of multiple correlation and simple regression were applied. RESULTS: The results showed that the level of cardiorespiratory fitness correlated negatively with BMI, percent body fat, WC, TG, and HDL-C, in addition to HOMA-IR and hsCRP, and strongly influenced BMI, percent body fat, WC, TG, HDL-C, HOMA-IR, and hsCRP. Comparative analysis of our subjects showed that higher cardiorespiratory fitness levels had positive effects on BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. CONCLUSIONS: This study found that there are close correlations between cardiorespiratory fitness and BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. It also showed that, compared to lower levels of cardiorespiratory fitness, higher levels had beneficial effects on BMI, metabolic syndrome risk factors, diabetes, and cardiovascular disease risk factors.

PURPOSE: This study aimed to explore the subjective sleep quality of depressed and non-depressed mothers in the late postpartum period and to determine the relationship with their health promoting behaviors, family functioning, parenting stress. METHOD: A non-probability sample of 128 mothers completed a self-administered questionnaires at 4-6weeks postpartum. The Edinburgh postnatal Depression Scale (EPDS) and Pittsburgh Sleep Quality Index were used to measure mother's experiences of depression symptoms and sleep. Related factors of sleep quality were measured by the Korean Family Functioning Scale, Health Promoting Lifestyle Profile, and Parenting Stress Index. The data was analysed using t-test, one-way ANOVA, and the Pearson's correlation coefficients. RESULT: The results indicated that the depressed mothers(EPDS> or =10) had poorer sleep quality than the non-depressed mothers(EPDS<10), reported shortened sleep duration, and experienced more daytime dysfunctions. Depressed mothers who had no job, did not drink coffee, and were primipara tended to report poorer sleep quality. There were significant correlation between poorer sleep quality and lower health promoting behaviors, higher family intimacy and lower family communication, and higher parenting stress among depressed mothers. CONCLUSION: Our findings support the view that depressed mothers' experiences of poor sleep are much higher than non-depressed mothers and multi-faced. Nurse professionals should screen for sleep problems in the depressed mothers with a different biopsychosocial and behavioral aspect from the non-depressed mothers in the late postpartum period.

PURPOSE: The purposes of this study were to (a) explore the prevalence of sleep patterns of middle-aged women; (b) identify the relationship between subjective sleep quality and its related variables; and (c) predict the possible explanations of how the related factors affect subjective sleep quality of middle-aged women. METHODS: A total of 256 women aged 40-60 years were purposely sampled and given self-administered questionnaires in a cross-sectional, correlation coefficient design. The data was analysed using t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. RESULTS: The results were as follows; (a) a total of 40% of the women were poor sleepers as defined by a global PSQI score >5; (b) there were significant differences in quality of sleep related to type of family, number of present illnesses, and use of medication; (c) health promoting behaviors, family functioning, depression, and climacteric symptoms were significantly related to subjective sleep quality; and (d) 34% of variance in subjective sleep quality can be explained by climacteric symptoms, depression, type of family, and spouse. CONCLUSIONS: Our findings suggested a high proportion of poor sleeping in middle-aged women. Poor sleeping appeared to have poor health promoting behaviors, lower family functioning, worse depression, and higher climacteric symptoms. Climacteric symptoms, depression, type of family, and spouse were independent risk factors for poor sleeping. Nurse professionals should screen for sleep problems in middle-aged women with a biopsychosocial and behavioral aspect.

PURPOSE: This study was conducted to identify awareness levels in regard to preconception care and gender differences targeting single undergraduate students in their 20s. METHODS: The study participants were 867 single undergraduate students aged 20~29 (523 women, 344 men). This was a descriptive research, in which awareness levels of preconception care and pregnancy were identified the through questionnaire surveys. RESULTS: Gender differences in the awareness of reproductive care before conception were found. Generally, women's awareness of issues related to preconception care and pregnancy was higher than that of men. Women recognized risk factors in pregnancy (chi2=22.85, p<.001) and positive behaviors (chi2=10.91, p = .012) better than men. Women's awareness of preconception care was significantly higher than that of men (t=8.37, p<.001). The awareness levels of the students who completed a reproduction related class (t=3.16, p = .002) and the students who knew about folic acid (t=-10.78, p<.001) in preconception care were statistically significantly higher. CONCLUSION: The results indicate that the ways to educate and alert students to major risk factors before pregnancy and other reproductive care content before conception is to provide education both single women and single men.

The inhibitory effect of rice(Oryza sartiva L., illpumbyeo) against mutagenicity induced by tryptophan pyrolysates were investigated using Salmonella typhimurium reversion assay. Both methanol extracts of obtained from brown and white rice were found to possess strong activites of inhibiting the mutagenicities of 3-amino-1,4-dimethyl-5H-pyriod[4,3-b]indol(Trp-P-1) and 3-amino-1-methyl-5H-pyrido[4,3-b]indol(Trp-P-2) on Salmonella typhimurium reversion assay. As the concentration of methanol extract increased, inhibitory effect on mutagenicity increased but reached at steady state as inhibition rate of 90% when the concentration was above 10mg/plate. There was no significant difference(p>0.05) in inhibitory effect of methanol extracts between brown and white rice against tryptophan pyrolysates.

OBJECTIVES: We sought to describe the perioperative and postoperative adverse events associated with sacral colpopexy and evaluate the surgical outcome, complications, and benefits of laparoscopic sacral fixation for patients with pelvic prolapse. METHODS: Ninety-two women with uterine prolapse underwent sacral colpopexy between January 2011 and September 2016 at Chosun University Hospital. Patients' electronic medical records were investigated for demographic, intraoperative, and postoperative data. Strict definitions were used for all clinically relevant adverse events. Patients' outcomes were documented with 1 self-administered quality of life questionnaires: the Pelvic Floor Distress Inventory-20 focused on symptom distress. The primary analysis looking at perioperative and postoperative adverse events was descriptive and statistics were reported for all groups as n/N (%) with 95% confidence intervals for categorical variables and as mean ± standard deviation and mean (range) for all continuous variables. RESULTS: Their mean age was 69 ± 8.1 years, mean follow-up duration was 12 months, and mean operating time was 61 minutes. There were seven conversions due to anesthetic or surgical difficulties. Follow-up was performed using a telephone questionnaire and physical examination at 12 months. There were three cases of sacral pain with strong analgesics, one of vaginal erosion, two of transient urinary retentions, one of spondylitis, and two of mesh infection. Of the patients, 98.9% were satisfied with the surgical results, while none complained of sexual dysfunction or problems performing her usual activities. CONCLUSIONS: Laparoscopic sacral colpopexy is a feasible and highly effective technique that offers good long-term results with complication rates similar to those of open surgery with the added benefit of being minimally invasive.
