1.Spontaneous, Positional, Positioning and Post Head Shaking Nystagmus in Unilaterally Labyrinthectomized Cats.

BACKGROUND AND OBJECTIVES: Unilateral vestibular dysfunction evokes many bothersome symptoms such as nausea, vomiting, vertigo, ataxia, and positional change. We attempted to understand the vestibular signs after a loss of unilateral vestibular function. MATERIALS & METHOD: We evaluated the serial changes of eye movements and body posture after unilateral labyrinthectomy which was performed on five cats. Data obtained from three of these cats. The eye movements were measured in vertical, horizontal and torsional planes using magnetic search coils, with eyes open in light and eyes open in darkness. The eye movements were recorded with head in neutral, tilted, hanging, and Dix-Hallpike position, and after head was shaken. RESULTS: 1) Spontaneous nystagmus was present for the first three days. 2) Head tilting positional nystagmus was present only on the first day after the operation. Head hanging position produced nystagmus with high slow phase eye velocity for 4 weeks. 3) Positioning nystagmus was recorded for 4 weeks and it was more prominent when labyrinthectomized ear was undermost. 4) Head shaking nystagmus around the pitch and roll axis was evoked within one week, but in the yaw axis it persisted for 4 weeks. When the cat was tilted to the lesion side, the intensitiy of positional and positioning nystagmus was stronger. CONCLUSION: Unusual positions such as head hanging and Dix-Hallpike position evoked prolonged nystagmus. Head shaking in the yaw axis could be used on a long term basis to understand unilateral vestibular dysfunction.

Paratubal cysts come from paraovarium of the broad ligament between the fallopian tube and ovary. Overall, these cysts constitute 10% of all adnexal masses and are commonly incidental findings upon surgical exploration for other reasons. They more commonly occur in women between 30 and 40 years of age. We report two cases of rapidly growing, huge paratubal cysts in young women, which are presented with brief review of literature.

3.Robot-Assisted Laparoscopic Surgery for Pelvic Organ Prolapse among Peri- and Post-Menopausal Women

For patients with apical compartment pelvic organ prolapse (POP), abdominal sacrocolpopexy has been considered superior to the vaginal approach in terms of less dyspareunia and decreased risk of recurrence. Robot-assisted sacrocolpopexy (RSC) can help overcome difficulties in laparoscopic sacrocolpopexy (LSC) by facilitating deep dissection and suturing. Moreover, RSC is a safe and efficacious option for patients with POP. It has several benefits, such as its high anatomical cure rate, improvement of sexual function, reduction of perioperative complications, and low recurrence rate. In addition, it can be a safe option for elderly patients. RSC has a steep learning curve and numerous other reported advantages compared with LSC; however, insufficient data conclude that the former is universally superior, especially in cost effectiveness. Thus, further studies are needed to support the widespread adoption of robot-assisted surgery for pelvic floor reconstruction.
4.Alteration of Circadian Diurnal Rhythms of Cardiovascular Parameters by Night Shift Work in 3 Shift Nurses.

Background: There are incoherent studies about the effect of night shift work on circadian diurnal rhythms of cardiovascular parameters. Our study aimed to assess the alteration of circadian diurnal rhythms of cardiovascular parameters by night shift work. Methods: We studied 26 healthy nurse volunteers engaged in 3 shift work. From March 16 to April 30, 2006, ABPM (ambulatory blood pressure monitoring) was performed to each nurse two times during the day and night shift. Three nurses were excluded due to pain omdiced by ABPM. Blood pressure and heart rate was monitored every 30 minutes. Paired T-test was used for statistical analysis. Results: The mean arterial pressure, the mean systolic pressure, the mean heart rate and the mean pulse pressure during the overall period were significantly higher in the night shift than the day shift (P-value: <0.05, <0.001, <0.05, and <0.001, respectively). The mean arterial pressure, the mean diastolic pressure and the mean heart rate during in-hospital working period were significantly higher in the day shift (P-value: <0.05, <0.001, and <0.05, respectively). All parameters during the daytime sleep period after the night shift were significantly higher than during the nighttime sleep period after the day shift (P-values: <0.001). Proportional percentile falls of 3 parameters (systolic pressure, diastolic pressure, and heart rate) during the daytime sleep period after the night shift were significantly less than during the nighttime sleep period after the day shift (P-values: <0.001). Conclusion: The night shift work caused significant alteration in homeostasis of autonomic nervous system associated with circadian diurnal rhythms of cardiovascular parameters.

5.Knowledge regarding cervical cancer, human papillomavirus and future acceptance of vaccination among girls in their late teens in Korea.

OBJECTIVE: The aim of this study is as follows; First, to assess knowledge regarding cervical cancer, HPV and the acceptance of HPV vaccination among girls in their late teens in Seoul, Korea. Second, to find out factors affecting the acceptance of HPV vaccination. METHOD: From November to December 2006, we surveyed Ewha Womans University freshmen and third year female students of Keumran High school affiliated with Ewha Womans University. A written questionnaire was sent to 310 students and 275 students responded. The questionnaire was divided into three sections that sought information about the followings : (1) Demographic characteristics; (2) Knowledge regarding cervical Pap smear testing and cervical cancer; (3) Knowledge regarding HPV and HPV vaccines. We scored the questionnaire giving one point for each correct answer. We summed up scores of knowledge on each section and compared the scores according to the acceptance of HPV vaccination in target group. RESULTS: The mean score of knowledge regarding cervical Pap smear testing and cervical cancer, HPV and HPV vaccines were 5.31+/-1.48 and 3.11+/-2.09, respectively. None of the demographic characteristics influenced the acceptance of HPV vaccine. The level of knowledge regarding HPV and HPV vaccines significantly affected the acceptance of HPV vaccine, however, knowledge regarding cervical Pap smear testing and cervical cancer did not. CONCLUSION: This study emphasize the need for further education regarding HPV infection and HPV vaccination for improvement in HPV vaccine acceptance.

6.Ribes fasciculatum var. chinense Attenuated Allergic Inflammation In Vivo and In Vitro.

Ribes fasciculatum var. chinense MAX. (R. fasciculatum) has traditionally been used in Korea to treat inflammatory diseases. However, the exact mechanism that accounts for the anti-inflammatory effect of R. fasciculatum is not completely understood. We aimed to ascertain the pharmacological effects of R. fasciculatum on both compound 48/80- or histamine-induced scratching behaviors and 2, 4-dinitrochlorobenzene (DNCB)-induced atopic dermatitis (AD) in mice. Additionally, to find a possible explanation for the anti-inflammatory effects of R. fasciculatum, we evaluated the effects of R. fasciculatum on the production of inflammatory mediators in LPS-stimulated macrophage cells. Treatment of R. fasciculatum significantly reduced compound 48/80- or histamine-induced the pruritus in mice. R. fasciculatum attenuated the AD symptoms such as eczematous, erythema and dryness and serum IgE levels in AD model. Additionally, R. fasciculatum inhibited the production of tumor necrosis factor-alpha (TNF-alpha) and interleukin-6 (IL-6). The maximal rates of TNF-alpha and IL-6 inhibition by R. fasciculatum (1 mg/ml) were approximately 32.12% and 46.24%, respectively. We also showed that R. fasciculatum inhibited the activation of nuclear factor-kappa B in LPS-stimulated macrophages. Collectively, the findings of this study provide us with novel insights into the pharmacological actions of R. fasciculatum as a potential molecule for use in the treatment of allergic inflammatory diseases.

7.Effects of Nursing Competence on Job Satisfaction across Career Stages

Purpose:
This study examined the variations in nursing competence and job satisfaction across different career stages. Additionally, it explored the effect of nursing competence on job satisfaction within each career stages.
Methods:
Participants were 468 nurses working at a university hospital in B city. Data collection was conducted using self-report questionnaires from April 1 to 15, 2020. The data were analyzed descriptive statistics, one-way ANOVA, Scheffé test, and multiple regression analysis employing SPSS 23.0.
Results:
General nursing competence improved significantly after the proficiency stage. Care execution and critical thinking were improved as nurses progressed from the novice to the advanced beginner stage. Additionally, communication showed progress in the competent stage. Furthermore, responsibility and self-development were improved after the proficiency stage.Among the sub-competencies examined, self-development, care execution, and communication significantly influenced the job satisfaction of a nurse’s career. Moreover, self-development was influential at all stages.
Conclusion
This study shows that there was no substantial improvement in nursing competencies as they progressed through various career stages. Self-development was the most crucial sub-competence impacting job satisfaction, with the effects of the sub-competencies varying across career stages. These results will aid in planning clinical continuing education and formulating policies to enhance nurse competency.
8.The Effects of Soy-Based Infant Formula on Growth and Development, and the Isoflavone Concentration of Plasma and Urine in Full-Term Infants.

Soy-based formula has been used for centuries in Korea. Soybeans contain phytochemicals with a biochemically active component, isoflavone. Isoflavone is a kind of phytoestrogens, structurally and functionally similar to estrogen. This study was conducted to investigate the effects of soy-based infant formula on growth, development, and isoflavone concentration in the plasma and urine. Thirty-nine healthy infants who were delivered at K university medical center were recruited. Experimental groups were the breast milk group (n = 15, BM) who were fed breast milk, soy-based formula group (n = 10, SBF) who were fed soy-based infant formula, and the casein-based formula group (n = 14, CBF) who were fed casein-based infant formula for 4 months. HPLC analysis was used to measure the concentration of isoflavones. The measurements of infant weight, length, head circumference and chest circumference were all in the normal growth range and were similar among the experimental groups. No significant differences were found in the scores of total mean of infant development test (Development Quotient, DQ) among the experimental groups. The isoflavone content of soy-based formula was significantly higher than. that of breast milk and casein-based formula. Plasma concentration of daidzein and genistein in the infants fed soy-based formula (daidzein : 264.1 ng/ml, genistein : 392.1 ng/ml) was significantly higher (p<.0001) than that in infants fed breast milk (daidzein : 3.4 ng/ml, genistein : 3.8 ng/ml) and casein-based formula (daidzein: 8.1 ng/ml, genistein: 9.3 ng/ml). Also, urinary daidzein and genistein concentrations in infants fed soy-based formula (daidzein: 19.82 microgram/ml, genistein : 17.89 microgram/ml) were significantly higher (p<.001) than those in infants fed breast milk (daidzein: 0.28 microgram/ml, genistein : 0.22 microgram/ml) and casein-based formula (daidzein : 0.45 microgram/ml, genistein : 0.33 microgram/ml).

This review presents guidelines for the diagnosis and management of endometriosis with emphasis on: it's role in infertility, treatment of recurrence, asymptomatic women, endometriosis in adolescents and menopausal women, and possible association of endometriosis with cancer.

Among the bronchogenic carcinomas, especially squamous cell carcinoma and large cell carcinoma frequently present with cavitation, which may result from tumor necrosis. Cavitary lesions of the tumor are occasionally associated with infection and misdiagnosed as benign lung abscess owing to the partial responsiveness to antibiotics. It is very difficult to distinguish the carcinomatous abscess from the benign lung abscess, because of their similar clinical and radiologic features. Delay in diagnosis of underlying lung cancer may result in poor outcome. Therefore, clinicians should remember that the patients with highly suspicious carcinoma of the lung should undergo further precise examinations to find out malignant cells.
