1.Effect of Indomethacin Therpy on Prevention of Intraventricular Hemorrhage in Very.

PURPOSE: Intraventricular hemorrhage(IVH) is a major risk factor for neurodevelop- mental handicap in very low birth weight infant. So we carried out to determine the efficacy of indomethacin in reducing the incidence of intraventricular heorrhage in very low birth weight infant. METHODS: A prospective, random trial was carried out to determine the efficacy of indomethacin in preventing intraventricular hemorrhage. A total of 46 infants of less than 1500g birth weight admitted to NICU of IL Sin Christian Hospital from August 1995 to June 1997 were analyzed. Randomly, 23 infants were given indomethacin and 23 infants were not given as control group. Serial cranial ultrasound examination was performed. RESULTS: There were no differences in the birth weight, gestational age, sex, Apgar score, blood pressure, intake and output within the first 3 days between the indornethacin and control groups. Of 23 infants given indomethacin, four had germinal matrix or intraventricular hemorrhage, in comparison with 10 of 23 control infants(P=0.045). CONCLUSION: Prophylactic indomethacin lowers the incidence of IVH in very low birth weight infants.

4.A Case of Neuronal Heterotopia.

Neuronal migrational disorders of the brain represent abnormalities in the formation of the neocortex caused by faulty migration of the subependymal neuroblasts. The neuroblasts normally migrate between the sixth and 15th gestational week and in doing so form the six-layered neocortex. When the migration does not occur in a normal fashion the resultant brain anomalies include lissencephaly, pachygyria, schizencephaly, hemimegalencephaly, heterotopia, and polymicrogyria. Neuronal heterotopia is a collection of nerve cells in abnormal locations as a result of arrest of their radial migration, improper formation, or destruction of the radial glial fiber. We reported a case of neuronal heterotopia with brief review of related literatures.

5.Three Dimensional Reconstitution of Oral Mucosal Keratinocytes and Its Biologic Characteristics.

The purposes of this study were to develop an in vitro co-culture model of epithelial tissue with dermal equivalent, cultured at an air-liquid interface, and to evaluate the effects of extracellular matrix and concentration of calcium and fetal bovine serum in medium to find optimized culture condition. Oral keratinizing epithelial cells in monolayer culture were grown in Mitomycin-treated 3T3 feeder. Primary cultured oral epithelial cells were reconstituted onto the dermal equivalents consisting of 3T3 fibroblast and type I collagen, and co-culture was grown at the air-liquid interface. The histomorphological development of reconstituted oral epithelium in vitro for 21 days revealed 10~12 layered statified epithelium, closely similar to the parakeratinized gingival epithelium. Neither laminin nor type IV collagen was able to induce keratinocyte differentiation. But a mixture of laminin and type IV collagen induced well-polarized keratinizing tissue with anchoring structure of basal cells. When the reconstituted oral epithelium was incubated in 1.0% and 0.5% serum-containing medium, the granular cell layers with orthokeratinization developed. The reconstituted epidermis generated in serum-free keratinocyte growth medium (KGM)-containing pituitary extract showed features of incomplete differentiation. The present study shows that the dermal equivalents containing fibroblasts will support epidermal morphogenesis and differentiation. And these results suggest that extracellular matrix and calcium concentration are important factors during the reconstitution of keratinizing epithelium in vitro.

The ovarian hyperstimulation syndrome (OHSS) is one of the notorious complications of assisted reproduction procedure. Severe and moderate OHSS associated with a spontaneous pregnancy is rare. We experienced a case of two normal, spontaneously conceived pregnancies with OHSS and her past obstetric history included prior spontaneous OHSS in the first trimester and term vaginal delivery of normal baby.

7.Dietary Risk Factors Related to Bone Mineral Density in the Postmenopausal Women with Low Bone Mineral Density.

This study was conducted to determine the dietary and lifestyle factors related to bone the mineral density (BMD) of postmenopausal women with decreased BMD. The subjects (N = 163) were recruited from women who visited a orthopedic clinic for BMD check up. A trained dietition interviewed subjects individually to obtain data about dietary behavior, consumption frequency of foods known as main dietary source of calcium and potassium, and clinical symptoms. The risk factors were identified by correlation and multiple regression analysis of variables. The mean age of the subjects was 66.8 years. Most of them showed low levels of education and monthly income. The mean age of menarche and menopause were 17.2 and 48 year, respectively. The mean menopause duration was 18.7 year. Most of the consumption of calcium was centered to vegetable foods. Education level were positively correlated (r = 0.272, p < 0.05) with BMD whereas age, menarch age, menopause duration, number of children were negatively correlated (r = -0.355, r = -0.240, r = -0.283, r = -0.193, respectively, p < 0.05) with BMD. The consumption of soybean, radish were positively correlated (r = 0.187, r = 0.158, respectively, p < 0.05) with BMD. Potassium intake with rice showed significantly negative correlation with BMD (r = -0.189, p < 0.05), but calcium intake with brown seaweeds, bean sprouts were positively correlated (r = 0.247, r = 0.254 respectively, p < 0.05) with BMD. Protein intake with roasted pork was also positively correlated (r = 0.216, p < 0.05) with BMD. Multiple regression analysis showed that the most prominent negative predictor influencing the BMD was age. Minor negative factors influencing the BMI were age of menarche, potassium intake from rice. But the significantly positively factors influencing the BMD were consumption of radish and soybean intake, education, and protein intake with roasted pork. In conclusion brown seaweeds, radish or soybeans can be promoted as cheap foods replacing milk and milk products for menopausal women with low income.

8.Immunohistochemical Study of Immune Cells, with a Special Emphasis on Macrophage Subpopulations in the Rat Spleen after Cyclophosphamide Treatment.

This study was undertaken to investigate the in vivo effects of cyclophosphamide (CY) on subpopulations of macrophages and other types of immune cells including dendritic cells (DCs) as well as on ICAM-1 expression in the spleen of rats. After a single dose of CY (150 mg/kg) was administered to Sprague-Dawley rats by intraperitoneal injection, the rats were sacrificed at 1, 3, 7, 14 and 28 days. The immunocytochemical characterization of the tissues were carried out using the monoclonal antibodies W3/25, OX8, HIS24, 8A2, OX6, OX62, ED1, ED2, ED3, and TLD-4C9 for analysis of macrophage subpopulations, DC(s), CD4(+) and CD8(+) T cells, B cells and ICAM-1 expression in cryostat-cut sections. CY exhibited a profound immunosuppressive effect on CD4(+) and CD8(+) T cells as well as B cells as was expected. However, it was found that CY induced an increase in number of certain subpopulations of macrophages, including ED1(+), ED2(+) and ED3(+) macrophages. Contrarily, CY elicited a decrease in number of DCs. CY induced a conspi-cuous upregulation of ICAM-1 on certain populations of leukocytes. This increased expression of ICAM-1 after CY treatment appears to be related with the recruitment of certain populations of leukocytes. Most of these features began to appear from the first day and reached the maximun on the third and especially, the seventh day, but two weeks after CY administration, these phenomena declined. In conclusion, the present study provided a new insight into the differential effects of CY on various populations and subpopulations of immune cells in the rat spleen.

10.Comparison of Gait Parameters during Forward Walking under Different Visual Conditions Using Inertial Motion Sensors

Purpose:
Gait evaluation in patients with dizziness is essential during both initial evaluation and vestibular rehabilitation. Inertial measurement unit (IMU)-based gait analysis systems are clinically applicable in patients with dizziness. Since dizzy patients can utilize visual inputs to compensate for vestibular deficits, it is more difficult for them to walk with their eyes closed (EC). In this study, we compared gait characteristics during forward walking with both eyes open (EO) and EC between healthy subjects and dizzy patients.
Materials and Methods:
Forty-nine healthy controls (mean age 37.18±10.71 years) and 23 patients with dizziness (mean age 49.25± 15.16 years) were subjected to vestibular and gait analyses. Medical histories, physical examinations, and vestibular function tests ruled out possible vestibular deficits in the controls. Subjects were instructed to walk at a comfortable pace for 10 m under two conditions (EO or EC). Spatiotemporal parameters, kinematics, and simulated kinetics of each gait recording were recorded using a shoe-type IMU system and analyzed.
Results:
Although gait speeds were slower, stride lengths were smaller, and double support times were increased under the EC, compared to the EO condition, in both healthy subjects and dizzy patients, the difference was more prominent in dizzy patients.Phase coordination index values did not differ significantly in either group. Gait asymmetry (GA) increased significantly under the EC condition, compared to the EO condition, in dizzy patients.
Conclusion
GA during forward walking was greater in dizzy patients under an EC condition than under an EO condition.
