1.Two Cases of Sex-linked Ichthyosis Improved by an Oral Aromatic Retinoid (Ro 10 - 9359).

We present the therapeutic effects of an aromatic retinoid (Ro 10-9359) in two cases of sex-linked ichthyosis. Case 1, Uncle of Case 2, was a 24-year-old male who had had dark brown scales on the trunk and upper extremities for 15 years. Case 2 was a 10-year-old boy who had had dirty brown scales on the trunk and lower extremities since 3 years of age. The diagnosis of sex-linked ichthyosis was made by characteristic scales and histopathologic findings. Initially, both patients received an oral aromatic retinoid (Ro 10-9359) with a daily dose of 1.5mg/kg body weight for the first two weeks. Thereafter the drug was tapered gradually to the dose of 0.5mg/kg body weight according to the therapeutic response. There was near complete clearing of the skin lesions in both patients after 10 weeks treatment. Side effects were of mild intensity and included dryness of mouth and nose, chelitis, paronychia and arthralgia of knee joint.

2.Aggressive Angiomyxoma of the Scrotum: A case report.

Aggressive angiomyxoma is a rare, locally aggressive tumor and occurs exclusively in the pelvic and perineal regions of women. But this tumor can occur in men, and the ratio of man to woman is about 1:6. We report a case of aggressive angiomyxoma of the scrotum in a 75-year-old man. The resected mass measured 23 21 10 cm and weighed 1,150 g, and was covered with thick but intact scrotal skin. The cut surface of the mass showed pale yellow to grayish white loose and edematous fibrous tissue with focal cystic degeneration. Microscopically, the mass was composed of loosely arranged bland-looking spindle or stellate cells and variable sized blood vessels with thickened and hyalinized wall. The spindle cells were strong positive for vimentin, focal, weak positive for desmin and smooth muscle actin but negative for S-100 protein. The spindle cells were also positive for estrogen and progesterone receptors.

3.A Study on Photopatch Test in Photodermatoses.

It is believed that the photopatch test is a valuable screening procedure for the determination of responsible antigenic substances in photoallergic contact dermatitis. But the technigues used for this procedure are cumbersome and require expensive equipment. In the present study, we adapted an easy and inexpensive photopatch test met- hod to our need. We report the results of this test method which was performed. in 21 patients with photodermatitis and 9 patients with polymorphic light eruption. 1. The UVB sensitivity determined by minimal erytherna dose (mean+-standard deviation) was I1618mW sec/cm in photodermatitis group, 108+ROmW sec/cm in polymorphic light eruption group and 126-+32mW-sec/cm in control group. But there was no significant difference among them (p>0. 05). 2. The reaction to UVA were negative in all groups. R. The photopatch test using IO potential photosensitizers revealed 12 positive responses in 9 patients; including 7 patients in photodermatitis group and 2 patients in polymorphic light eruption group. And the number of positive photopatch responses obtaied with each photosensitizer in RO patients with photodermatoses was 5 in chlorhexidine, 2 in paraaminobenzoic acid, musk ambrette and bithionol and 1 in chlorpromazine, respectively.

The developement of squamous cell carcinoma from the draining sinus of chronic osteomyelitis has long been recognized as a rare and late complication. The mode of developement of carcinoma at the site of chronic osteomyelitis is not well understood. This, however, is chiefly a disease of middle aged men, and tibia is the most common site. There are two types in this carcinoma; superficial and deep types. In the superficial type obvious presence of fungaiing growth makes diagnosis simple and easily confirmed by biopsy. In the deep type the diagnosis is difficult clinically, but the features most frequently described are an increase in pain and discharge with swelling and hemorrhage. Amputation at the adequate level is the treatment of choice. Three cases of the disease involving one left femur, and two left tibiae are reported with review of literature.

7.Clinical Type and Treatment Response in Patients with Digital Mucous Cyst: A Retrospective Study

Background:
Digital mucous cysts (DMCs) are benign, occurring mainly on the fingers but rarely on the toes.Although the exact cause of DMC has not been identified, it is classified into two different types based on its pathogenesis and location. Surgical and nonsurgical procedures, including aspiration, sclerotherapy, steroid injection, cryotherapy, and CO 2 laser, can be used to treat DMC.
Objective:
This study aimed to compare treatment response based on DMC type and evaluate its efficacy in surgical and nonsurgical (sclerotherapy) treatments.
Methods:
We retrospectively reviewed the electronic medical records of patients with DMC who visited our dermatology department between January 2010 and November 2020.
Results:
This study enrolled 39 patients having a mean age of 59.2 years. The proportion of female patients (21/39, 53.8%) was more than male (18/39, 46.2%). The DMC occurrence was more frequent in the fingers (32/39, 82.1%), especially in the index and middle fingers than in the toes (7/39, 17.9%). In sclerotherapy, DMC in the distal interphalangeal joint (DIPJ) required more treatment than that in the proximal nail fold (PNF) (p<0.05). The number of treatments using sclerotherapy was higher compared to that of surgical treatments (p<0.05).
Conclusion
The DIPJ type of DMC patients was more difficult to treat using sclerotherapy than the PNF type.Moreover, surgical treatment has a better response than sclerotherapy.
8.Usefulness of Ultrasonography in the Diagnosis of Peptic Ulcer Disease in Children.

PURPOSE: This study was performed to assess the clinical usefulness of transabdominal ultrasonography (TUS) in detecting peptic ulcer disease (PUD) in children. METHODS: Twenty-four patients (19 boys, 5 girls; mean age, 10.6±4.5 years [range, 3.0–17.9 years]) who were admitted to the hospital for acute abdomen or gastrointestinal bleeding and diagnosed with PUD by endoscopy and who underwent TUS were included. Clinical data were retrospectively collected by reviewing patient medical records. Gastric ulcer (GU) was suspected when the gastric wall exceeded 8 mm in thickness and had lost its five-layer structure on TUS. Duodenal ulcer (DU) was suspected if the duodenal wall thickness exceeded 5 mm. RESULTS: Sensitivity of TUS in diagnosing PUD was 66.7% for GU and 38.9% for DU. Mean age and body weight of the 11 patients suspected with PUD on TUS were 10.9±4.4 years and 38.1±17.2 kg, respectively. For 13 patients without suspected PUD, they were 12.1±4.1 years and 39.6±17.0 kg, respectively. There was a significant difference in height, weight, and body mass index between patients who were suspected to have PUD and those who were not suspected on TUS (p=0.014, 0.008, and 0.005, respectively). A significant difference in the sensitivity of TUS in diagnosing PUD was found between patients under 30 kg and those over 30 kg (88.9% and 20.0%, respectively; p=0.003). CONCLUSION: TUS investigation of the stomach and duodenum is an efficient method for PUD detection in children with low body weight. TUS can be used in preliminary diagnostic work-up before further invasive tests.

9.Early Developed Candida Endophthalmitis Following Intracapsular Cataract Extraction.

Candida albicans endophthalmitis following cataract surgery is fortunately rare complication, but once the infection is established, its course is unrelenting and the prognosis for a good visual result is usually very guarded. The authors experienced a case of culture-proved Candida albicans endophthalmitis following intracapsular cataract extraction in a 74-year old man. Endophthalmitis was resolved after treatment with intravitreal injection of amphotericin B, intravenous amphotericin B, oral Ketoconazole and 2% of topical Ketoconazole.

10.Facial Cutaneous Mycosis by Paecilomyces lilacinus.

A 19-year-old man noticed a very slowly spreading erythematous macular lesion on his cheek when he was 5 years old. This asymptomatic and well circumscribed patch was covered with fine scales. He sought first medical attention at the age of 12 years and diagnosis was cutaneous tuberculosis was made by skin biopsy. Seven years after the second biopsy was made and it revealed a few spores within giant cells in the upper dermis. By mycologic study, it was identified as Paecilomyces lilacinus. The patient was put on the treatment of topical and oral antifungal agents. The skin lesion showed signs of improvement after 12 weeks of treatment.
