1.Clinical Nurses' Job Stress, Emotional labor, Nursing Performance, and Burnout in Comprehensive Nursing Care Service Wards and General Wards.

PURPOSE: This study was done to identify and compare nursing performance including job stress, emotional labor and burnout between nurses who work in comprehensive nursing care service wards and general wards. METHODS: Participants were 187 nurses working in a general hospital in Busan which provides comprehensive nursing care service (comprehensive nursing care service ward: 95, general ward: 92). Data were collected using questionnaires and analyzed using descriptive statistics, χ² test, t-test, ANOVA, and multiple regression with the SPSS/WIN 23.0 program. RESULTS: There was no significant difference in the variables between comprehensive nursing care service ward nurses and general ward nurses. Factors influencing burnout were emotional labor in comprehensive nursing care service ward nurses while job stress and emotional labor were the factors influencing burnout in general ward nurses. CONCLUSION: The findings of this study indicate that comprehensive nursing care service can be a positive delivery system for nurses. Therefore, there is a need for strategies to integrate comprehensive nursing care service systematically.

2.A Case Report of Fatal Vaginal and Anal Fisting.

Fisting is an uncommon sexual activity, and death due to fisting is very rare. In the present report, we describe a case of a woman who died from fisting by her male partner. A 38-year-old woman went to a motel with a man after consuming alcohol, and there they kissed each other. As desired by woman, the man inserted his fingers into her vagina and anus, and later inserted his fist and forearm into her vagina and anus. After 20-30 minutes of fisting, the woman became unconscious and died. The autopsy confirmed the presence of perineal and vaginal lacerations with massive internal pelvic soft tissue injury with uterine artery rupture. A partial tear was noted at the rectosigmoid junction. The cause of death was hypovolemic shock due to uterine artery rupture. Following court trials, the assailant was sentenced to 4 years of imprisonment for the felony of inflicting bodily injury resulting in death.

4.A Case of Left Atrial Myxomoa Presented as Pseudovasculitis.

Left atrial myxoma is well-known mimicker of systemic vasculitis. In general, however, these patients showed some symptoms and signs of cardiovascular disease. We experienced a patient with left atrial myxoma who first presented as vasculitis without any symptoms and signs suggesting cardiovascular disorders. A 45 year-old lady showed tender skin nodules, livedo reticuiaris-like skin lesions, severe constitutional symptoms, multiple cerebral infarctions, and elevated ESR and globulin. Skin biopsy findings were thought to be consistent with polyarteritis nodosa. Echocardiography was performed as a routine test for evaluating suspected vasculitis patients and it revealed left atrial myxoma. However, we were not able to detect any auscultation abnormality, even after echocardiography, After echocardiography, we found the tumor emboli in skin biopy specimen with deeper section. This case demonstrated the importance of suspecting the possibility of left atrial myxoma when performing diagnostic work-up for vasculitis.

5.A Successful Management of Ruptured Duodenal Varix by Means of Endoscopic Ligation with Detachable Snare.

Duodenal varix is a rare site of hemorrhage in patients with portal hypertension, but its rupture is a serious and often fatal event. They can be diagnosed by means of upper gastrointestinal endoscopy, selective superior mesenteric artery angiography, slenoportogram. Especially, upper gastroduodenal endoscopy is the diagnostic procedure of choice in diagnosing duodenal varices. Treatment modalities for bleeding duodenal varices are sclerotherapy, varix ligation, portocaval shunt, and duodenal resection. Endoscopic sclerotherapy has limited success in controlling active duodenal varix. Endoscopic ligation with a detachable snare is a useful therapeutic measure in the treatment of bleeding duodenal varices. Wc report a patient with bleeding duodenal varix, successfully treated by means of endoscopic ligation with a detachable snare. The endoscopic examination showed spurting bleeding from dilated vessel at the third portion of the duodenum. We successfully controlled the bleeding duodenal varix by means of endoscopic ligation with a detachable snare.

6.Evaluation of Participation and Management on the Cyber Hospice Specialist Program.

PUPPOSE: The research purposes analyzed the study participation and lecture evaluation of applicants in the cyber hospice specialist education course to find problems of nurse education application on the web. METHOD: Study participants were 125 nurses for participation and 68nurses for lecture evaluation. The data was analysed by descriptive statistics. RESULT: The results obtained from this study were as follows 1) The residence distribution of study participants was spread out across the nation. Equal distribution of education was accomplished without a difference among provinces. 2) The average study duration in the study participation was about one hour and a quarter minutes a week, and number of access to lecture notes was 65.8 times. But in a discussion room and a cooperative room, the system using rate was very low, so we considered the idea to come up with a more effective application way. 3) The participant's lecture evaluation of cyber education were generally satisfied about the quality of lecture, time, contents etc. CONCLUSION: This study shows the possible implication for nursing fields using a web-based learning program for reeducation in a variety of fields, so nursing cyber application can be considered to come up with this more effective method.

7.A Study of Development and Evaluation of Tutorial Management Strategy for Web-based Nursing Education.

PURPOSE: This study was attempted and completed in order to settle down and qualitatively improve web-based nursing education by evaluating effect and managing strategy of tutor. METHOD: The development of tutor's managing strategy was based on "The Self-regulated Learning" and "The supportive Learning", then it was applied to 79 learners in one of the cyber-learning centers. After applying the tutor's managing strategy, self-regulated learning scale, attitude for school, preference for computer and academic achievement were evaluated. The development of tutor's managing strategy for web-based nursing education are consisted of participation promotion, psychological support and motivation, recognition and promotion strategy of learning activity, management strategy of evaluating stage. RESULT: The levels of learner's self-regulated learning, recognition, behavior, attitude on the school and learning achievement were meaningfully increased in statistics after applying for the managing strategy of tutor. The motivation level and learning participation kept high scores from the beginning with no significant statistical changes. CONCLUSION: It is required to develop an educational program for cultivating well-educated tutors in order to help the effective learning process of nurses based on understanding characteristics of learners.

8.Esthetic restorations of maxillary anterior teeth with orthodontic treatment and porcelain laminate veneers: a case report.

If orthodontists and restorative dentists establish the interdisciplinary approach to esthetic dentistry, the esthetic and functional outcome of their combined efforts will be greatly enhanced. This article describes satisfying esthetic results obtained by the distribution of space for restoration by orthodontic treatment and porcelain laminate veneers in uneven space between maxillary anterior teeth. It is proposed that the use of orthodontic treatment for re-distribution of the space and the use of porcelain laminate veneers to alter crown anatomy provide maximum esthetic and functional correction for patients with irregular interdental spacing.

10.Effect of Hand Massage on Nausea, Vomiting and Anxiety in Childhood with Leukemia on Chemotherapy.

PURPOSE: To determine the effects of hand massage on nausea and vomiting, and anxiety in children with lymphocytic leukemia receiving high dose chemotherapy. METHOD: The children were assigned to an experimental group(15) or a control group(15). All of the children were diagnosed with acute lymphocytic leukemia and admitted for high dose chemotherapy at C University Medical Center in Seoul. The hand massage was performed for 10 minutes twice a day over three days. To measure the effects of hand massage, the Index of Nausea and Vomiting by Rhodes et al. and the State-Trait Anxiety Inventory for children by Spielberger were used. The level of anxiety was measured by systolic blood pressure, diastolic blood pressure, and pulse rate. RESULTS: The score for nausea and vomiting decreased in the experimental group. State anxiety for the experimental group was significantly more positive than for the control group at the 2nd measurement. There was a significant difference in systolic blood pressure between the two groups. The level of diastolic blood pressure in the two groups decreased significantly over time. CONCLUSION: Hand massage could be effective in decreasing nausea and vomiting, state anxiety, pulse rate and blood pressure in children with acute leukemia receiving high dose chemotherapy.
