1.Reconstruction of a Skin Defect Following Excision of Basal Cell Carcinoma in the Nose by Labial - alar Transposition Flap.

We report a case of nasal BCC in which the skin defect is reconstructed by a local labial-alar transposition flap. In nasal reconstruction, the local flap is the preferred methods. This flap has the advantage of a more acceptable scar, is easy to make, and less edematous than a superiorly based flap. However, for defects larger than 1cm, this flap is not advisable and the defect has to be located adjacent to th nsolabial fold.

2.Effect of Glycolic Acid on Collagen Gene Expression in Cultured Human Skin Fibroblasts.

BACKGROUND: Glycolic acid is currently reported to have beneficial effects on the photoaged skin. These effects may be through the modulatory action of glycolic acid in the production of dermal extracellular matrix proteins of fibroblast. OBJECTIVE: The aim of this study was to investigate the specific effect of glycolic acid on the collagen and elastic fiber gene expression in vitro. METHODS: Human skin fibroblasts were cultured from normal skins of excised specimens and glycolic acid (75 µg/ml) was treated. After 24 hours of treatment, six each cell lines were processed for extraction of RNA and subjected to Northern analysis. RESULTS: Type I collagen mRNA expression was significantly increased in the glycolic acid-treated fibroblasts. Elastin mRNA expression did not show any significant change. CONCLUSION: These results suggested that glycolic acid induced the increase of type I collagen mRNA expression and had the specific biologic effect on fibroblast.

3.Two Cases of Cutaneous Protothecosis : Unique Histopathological Findings with Crystal Violet Staining and the Therapeutic Effect of Itraconazole.

Cutaneous protothecosis sometimes poses diagnostic and therapeutic problems. Isolation of the causative organism may not be successful and spores may be mistaken for other skin diseases unless the characteristic sporangia are detected in tissue sections. Because there are few cases, the optimal therapy is still being debated. On Liebs crystal violet staining we found charateristic purplish dots in Prototheca spores; these correspond to the amyloplasts or dense bodies found under electron microscopy. The isolated organisms were inhibited in vitro by itraconazole, amphotericin B, ketoconazole, and amorolfine and we were able to successfully treat two patients with itraconazole.

4.A Case of Hybrid Cyst showing Composite Features of an Eruptive Vellus Hair Cyst and Steatocystoma Multiplex.

Both eruptive vellus hair cyst and steatocystoma multiplex are uncommon conditions. There are clinical similarities between these two entities, but histological features are distinctive. We describe a patient with multiple subcutaneous nodules on the chest and both axilla, showing combined histological features of both an eruptive vellus hair cyst and steatocystoma multiplex. This case suggests that eruptive vellus hair cyst and steatocystoma multiplex are variants of one disorder which originate in the pilosebaceous duct.

6.Comparative Study of Glycolic Acid vs. Jessner's Solution in Patients with Facial Acne.

BACKGROUND: Glycolic acid has become an important and popular agent for chemical peel. Even though many clinicians perform superficial chemical peel to treat facial acne with glycolic acid, as yet there has been no well-controlled clinical trinls for the comparison of this new peeling agent and Jessner's solution. OBJECTIVE: To compare these two chemical peelI agents, our study was focused on three aspects : 1) treatment effects (acne improvement), 2) side effects (erythema, exfoliation), 3) patients preferences of the treatments. METHOD: Sixteen patients with facial acne vulgaris were treated with 70% glycolic acid and Jessner's solution on each side of the face simultaneously. Treatments were repeated biweekly with acne grading. Patient's subjective preferences were asked after the treatment.

7.Transabdominal Selective Fetal Reduction in Multifetal Pregnancy.

Over the past 30 years, there has been an increase in the incidence of multifetal pregna-ncies, primarily because of the introduction of ovarian stimulants for ovulation induction and assisted reproductive technology ( ART ) in infertile patients. It is well established that multifetal pregnancies are associated with an increased frequency of the maternal complications and gre-ater perinatal morbidity and mortyality. The adverse outcome of multifetal pregnancies is dire-ctly proportional to the number of fetuses, primarily as an consequence of prterm delivery. Re-duction in the number of fetuses in multifetal pregnancies has been proposed as a way to impr-ove the perinatal outcome in this situation. Therefore, selective fetal reduction ( SFR ) is sugges-ted as a therapeutic option for continuation of pregnancy with fetuses mature enough to survi-ve. In this paper, we report our infertility clinic experiences with 6 patients who carried mult- ifetal pregnancies including 1 quintuplet, 1 quadruplet, and 4 triplets. from January, 1991 to May, 1996, transabdominal SFR was accomplished by fetal intrathoracic KCl injection at 9~10 weeks of gestation. After the prcedure, 4 patients remained as twin pregnancies, and 2 patients as single pregnancy. There have been 3 sets of twin deliveries and the 2 sets of single delivery. One case was aborted. Two patients were delivered after 37 weeks of gestation, 2 patients were at 35 weeks, and 1 patient at 24 weeks. All babies have been healthy after birth in patients after 35 weeks gestation. There was no fetal anomaly related to the procedure in the 6 cases. We concluded that transabdominal SFR is a rather safe and useful procedure that may improve the outcome of multifetal pregnancies.

9.A clinical study on the incomptent internal os of the cervix.

OBJECTIVE: This study was performed to evaluate the effect of cervical cerclage and the clinical characteristics in incompetent internal os of the cervix (IIOC). METHODS: A study was conducted on 170 patients with IIOC, 199 cases of IIOC were admitted and treated with McDonald operation or modified Shirodkar operation at department of Obstetrics and Gynecology Il Sin Christian Hospital from January 1. 1994 to December 31. 1998, of this 14 cases were follow up lost, so 185 cases were analyzed. RESULTS: 1) The incidence of IIOC was 0.43%, 1 in 233 deliveries. 2) The mean age of IIOC patients was 30.54yrs old and the most frequent age group was in 30-34yrs old group (43.78%). 3) The average number of gravida and parity before operation were 4.71 and 1.62. & the success rate of operation was low at high gravida & parity. 4) The predisposing factor was previous history of dilation & curettage (43.75%), Cx. laceration after delivery (6.49%), midtrimester termination (4.32%) etc. 5) The operation methods were McDonald operation (91.35%) and modified Shirodkar operation (8.65%) & the success rate of McDonald operation and modified Shirodkar operation were 85.80% and 87.50%. 6) The successful fetal salvage rate was 85.95% and the highest success rate was 87.50% in 14-16 weeks of gestation group. 7) The more cervix dilate, the more failure occurred. 8) The causes of operation failure were premature rupture of membrane (50.00%), preterm labor (34.62%), FDIU, anomaly, APH etc. 9) The delivery methods after operation were vaginal delivery (65.54%), cesarean delivery (34.46%). CONCLUSION: The 14-16th weeks of gestation group & no cervical dilatation have higher success rate indicating that early diagnosis and appropriate timing of operation is associated with a greater operation success rate.

10.Tinea pedis in sewerage workers.

Tinea pedis is frequently found in those people with poor hygine and in hot and humid environments. The authors investigated the clinical, epidemiologicol a id mycological characteristics of tinea pedis in 138 sewerage workers attending a sewerage plant in Seoul. Tinea pedis was found in 82, with a prevalence of 59.4%. The prevalence of tinea pedis increased with age and the period working at sewerage plant, however, there was no statistical significance. Also there was no difference in the prevalence of tinea pedis between the clerical workers and the field workers. Positive rate for KOH smear was 73.2%, and culture positive rates were 42.7%, producing 35 strains of dermatophytes. Twertyeight strains of Trichophyton Rubrum and 7 strains of Trichophyton mentagrophytes were isolated. Twentyseven yeast-form colonies were isolated, and Trichosporon beigelii was foungl in 19 samples. Most of the yeast forms were found mixed with dermatophytes and moulcis, However, 6 were isolated from direct smear positive cases and yieIded pure colonies of yeast. These included 4 cases of T. beigelii, 1 case of Candida parapsilosis, and 1 case of Candida hormicola. In view of the recent report of these fungi as pathogenic organism, these isolates, rspecially T. beigelii, were considered as a causative agent of tinea pedis in certain groups like sewerage workers.
