1.Evaluation of Nutritional Status of Inpatients with Medical Health Problems.

PURPOSE: To investigate nutritional status of inpatients by using subjective. and objective evaluation methods and to find the relationship between them. METHOD: The subjects were 101 inpatients with medical health problems at a university hospital. Nutritional status was evaluated by the Subjective Global Assessment (SGA) and physical assessment including percentage of weight loss, serum albumin, hemogloin, and hematocrit. Data were analyzed using frequency, percentage, mean, Kendall's tau. RESULTS: Grouping by the SGA, 61.4% were classified as severe malnutrition group. When applying the objective methods{physical assessment). 1.9~42.6% were diagnosed as malnutrition each item. The percentage of weight loss during previous 1-6month(r=.43~.54. P=.0001), serum albumin(r=-.26, P=.0003), hemoglobin of male(r=-.38, P=.0001), and hematocrit of male(r=-.34, P=.0001) were significantly correlated with SGA score. The coincidence rate of nutrition evaluation between the objective methods and SGA were 27.7 35.6%, 20.8%, 47.5%, 58.4% in percentage of weight loss, albumin, hemoglobin, and hematocrit. CONCLUSION: These findings showed a majority of inpatients were exposed to the risk of malnutrition. We recommend to evaluate inpatients' nutritional status periodically and to develop nursing intervention to solve their nutritional problems.

3.A Case of Diencephalic Syndrome.

Diencephalic syndrome is a rare cause of failure to thrive in infancy and early childhood. The syndrome is characterized by profound emaciation with normal appetite, loss of cutaneous adipose tissue, hyperactivity, euphoria, and nystagmus. It commonly occurs in association with chiasmatic and hypothalamic gliomas. It has also been described in association with other histologic types. There is the marked increase of serum growth hormone, which may exhibit an inappropriate, even paradoxical response in stimulation test. A male infant of 12 months of age, showed markedly elevated growth hormone but he had failure to thrive findings. Evenly enhanced round mass was seen at suprasella area in brain CT. Its histological findings was "Desmoplastic infantile ganglioglioma", very rare histologic type. Here we report a case of diencephalic syndrome presented by failure to thrive in association with hypothalamic tumors.

5.A new method for concentration of proteins in the calcareous corpuscles separated from the spargana of Spirometra erinacei.

Calcareous corpuscles are a characteristic structure found in larval and adult stage cestodes. These corpuscles are known to contain several protein components and to possess protein-binding activity. However, the proteins bound to calcareous corpuscles in situ have not been studied. The present study was undertaken to identify the proteins on calcareous corpuscles. Calcareous corpuscles were purified from the plerocercoids (= spargana) of Spirometra erinacei, and serially dissolved using 0.1 M sulfamic acid solution. Collected supernatants were examined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and silver staining. The results showed that only the fraction remaining after the 19th dissolved fraction contained proteins. A total of 20 protein molecules were detected in gel, with major bands at 56, 53, 46, 40, 35, 29, 28, 24.5, 21, 19, 16, 13, 10 and 8 kDa. In particular, the proteins corresponding to the 21 and 16 kDa bands were most abundant. Our results demonstrated for the first time the protein contents of the calcareous corpuscles of spargana. Further studies on the functions of these proteins are required.

7.Efficacy and Safety of Combined Subacromial and Intravenous Patient-controlled Analgesia after Arthroscopic Rotator Cuff Repair.

BACKGROUND: This study investigated the efficacy and safety of combined subacromial and intravenous patient-controlled analgesia for control of postoperative pain after arthroscopic rotator cuff repair. METHODS: Between May 2012 and August 2014, 60 patients who underwent arthroscopic rotator cuff repair with acromioplasty and received patient-controlled analgesia were studied prospectively. Cases were divided into 2 groups: combined subacromial and intravenous infusion group (group A, 30 cases) and solitary intravenous infusion group (group B, 30 cases). The visual analogue scale was used to record the patient's level of pain every 12 hours during postoperative 72 hours and the following 48 hours after the suspension of patient-controlled analgesia. RESULTS: The mean preoperative visual analogue scale score was 7.8 in group A and 7.6 in group B, and the immediate postoperative visual analogue scale score was 7.9 and 8.1 for each group. At postoperative time (From 12 hours to 72 hours after operation), the scores of combined subacromial and intravenous infusion were significantly lower than those of solitary intravenous infusion. Significant difference in the frequency of supplemental analgesic injections was observed between group A and group B (p=0.008). However, no significant difference in complication rate was observed between the two groups (p=0.562). CONCLUSIONS: Combined subacromial and intravenous patient-controlled analgesia after arthroscopic rotator cuff repair is more effective than solitary intravenous infusion without significantly increasing complications. Therefore, combined subacromial and intravenous patient-controlled analgesia could be a effective pain control method.

8.Efficacy of Diet Therapy in Korea Hypercholesterolemic Patients.

BACKGROUND: Although diet therapy is the standard initial treatment for hypercholesterolemia, the efficacy of diet therapy in Korean hypercholesterolemic patients never been investigated . This study was designed to evaluate the efficacy of diet therapy for hypercholesterolemia. METHODS: One hundred seventeen outpatients with hypercholesterolemia were educated individualized diet therapy which is low in fat, cholesterol and high fiber. After three and six month, we assessed the changes of plasma lipids, apolipoproteins, body weight and the adherence to diet therapy. RESULTS: After 3 months of the diet therapy, there were significant reductions in total cholesterol(-11.8%), LDL-C(-14.5%), HDL-C(-3.9%), Apo(-5.4%), TC/HDL-C(-7.3%), Apo B /Apo A1(-7.7%) and weight(-3.1%). After 6 months of the diet therapy, there were significant reductions in total cholesterol(-10.7%), LDL-C(-14.0%), TC/HDL-C(-9.1%), Apo B/Apo A1(-7.7%) and weight(-2.3%). In this study 77% of the subjects had experienced improvement in their plasma LDL-C. Nonrespondents were older and had more female patients than respondents significantly. CONCLUSION: Diet therapy in Korean hypercholesterolemic patients is effective for reductions of total cholesterol and LDL-C.

9.Diagnosis of psychiatric disorders of the eldery.

Korea is rapidly aging society and psychiatric disorders of the elderly increasing Korea is rapidly becoming an aging society and psychiatric disorders of the elderly are increasing significantly in importance. Interviewing the elderly requires special skills because of their physical weakness and cognitive impairments such as difficulty concentrating. There are various diagnostic tools for them, for more accurate diagnosis. Representative scales include the Mini-Mental Status Examination for dementia, the Geriatric Depression Scale for depression, and the Confusional Assessment Method for delirium. Differential diagnosis of psychiatric disorders from cerebral and physical disorders is important.

10.Pelvic insufficiency fracture after radiotherapy in patients with cervical cancer in the era of PET/CT.

PURPOSE: To determine the incidence, risk factors, and clinical characteristics of pelvic insufficiency fracture (PIF) in patients with cervical cancer. MATERIALS AND METHODS: Between July 2004 and August 2009, 235 patients with non-metastatic cervical cancer were treated with definitive chemoradiation or postoperative radiotherapy. Among 235 patients, 117 (49.8%) underwent the first positron emission tomography/computed tomography (PET/CT) within 1 year after radiotherapy. The median radiation dose was 55 Gy (range, 45 to 60 Gy). Medical charts and imaging studies, including PET/CT, magnetic resonance imaging (MRI), CT, bone scintigraphy were reviewed to evaluate the patients with PIF. RESULTS: Among 235 patients, 16 developed PIF. The 5-year detection rate of PIF was 9.5%. The 5-year detection rate of PIF in patients who underwent the first PET/CT within a year was 15.6%. The median time to development of PIF was 12.5 months (range, 5 to 30 months). The sites of fracture included 12 sacroiliac joints, 3 pubic rami, 3 iliac bones, and 1 femoral neck. Eleven of 16 patients having PIF complained of hip pain requiring medications. One patient required hospitalization for pain control. The significant risk factors of PIF were old age, body mass index less than 23, bone mineral density less than -3.5 SD, and the first PET/CT within a year after radiotherapy. Radiation dose and concurrent chemotherapy had no impact on PIF rate. CONCLUSION: PIFs were not rare after pelvic radiotherapy in cervical cancer patients in the era of PET/CT. Timely diagnosis and management of PIF can improve quality of life in patients with cervical cancer, in addition to reducing unnecessary medical expenses.
