1.Effects of Nurses' Incident Reporting Attitude, Perception of Importance on Patient Safety Management, and Patient Safety Culture on the Reporting of Patient Safety Events

Purpose:
This study’s purpose is to examine the effects of nurses' incident reporting attitudes, their perceptions of importance of patient safety management, and patient safety culture on reporting patient safety events.
Methods:
We used a cross-sectional design with a convenience sample of 192 nurses with more than three months clinical experience from five provincial hospitals. The data were collected through an online structured self-report questionnaire from September 25 to October 15, 2022. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression.
Results:
The most important influencing factors for the level of patient safety event reporting were perceptions of the importance of patient safety management (β=.24, p=.005), followed by patient safety culture (β=.23, p=.019), incident reporting attitude (β=.18, p=.016), and near miss reporting experience (β=.14, p=.022). The explanatory power of the model was 33%.
Conclusion
To increase the level of patient safety incident reporting, differentiated education and standardized work procedures are needed. Also, it is necessary to prepare policies that revitalize patient safety reporting systems at medical institutions as measures to prevent patient safety accidents and recurrences.

4.Current and Future Foodservice Management Performance in Child-care Centers.

This study examined foodservice management performance in child-care centers and suggests ways in which meal service quality can be improved. Questionnaires were distributed to 51 child-care facilities. The majority of respondents were facility directors (dietitians) and their facility type was tax-paid (92.2%). The dietitian response rate was 51.0%, and the majority (96.2%) were hired with co-management status, visiting a facility once a week (76.0%). Only 52.1% of the facilities had menu planning by a dietitian, and improvements were needed in terms of planning menus with standardized recipes, especially for infant meals. The monthly food cost per child was 47,394 won, and the labor cost for a co-management dietitian was 3,670 won per child, indicating 21.8% and 1.8% of the tuition fee, respectively. Other necessary improvements included: more reliable food purchasing management, securing additional foodservice equipment, and better sanitation management. In addition, respondents rated the following as requirements to ensure high quality meal service: 'modernized foodservice equipment and facilities', 'government financial support', and 'information on nutrition and foodservice management provided by dietitians'. Based on the study results, the following are recommendations for improving meal service quality in child-care centers: Dietitian placement should be extended to facilities of over 50-capacity in addition to their current placement in facilities of over 100-capacity, and co-management dietitians should have their control span restricted to two facilities instead of five. Finally, nationwide nutrition support plans and nutrition education programs should be developed and implemented by dietitians, and their roles should be extended to foodservice mangers as well as nutrition teachers.

5.Colonic Diverticulosis: Evaluation wth Double Contrast Barium Enema.

PURPOSE: To evaluate the pattern of colonic diverticulosis according to age and sex, and recent trend. MATERIALS AND METHODS: The authors retrospectively reviewed 120 cases of colonic diverticulosis in 1,020 patients who had undergone a double contrast barium enema examination between January 1st, 1993, and December 31st, 1995, and analyzed the frequency, size, multiplicity and anatomical site, according to age and sex. Diverticulum size was classified into one of three groups : less than 5mm, 5-10mm, over 10mm in diameter. RESULTS: The overall incidence of colonic diverticulosis was 120 cases among 1,020 patients(11.8%) with an incidence 5.3 times higher in males than in females. Peak incidence was in the fifth decade, with 19 cases (15.8%) among males, and after the sixth decade, with four cases(3.3%) among females. Mean age was 57.7 years. Diverticulum size of 5-10mm in diameter was predominant 2% of cases) ; average diameter was 5-6mm. The incidence of colonic diverticulosis was 5.1 times more frequent in the right colon(101 cases) than in the left (20 cases). CONCLUSION: The overal overall incidence of colonic diverticulosis has continually increased; in addition it has also recently increased slightly in left-sided colon. This is thought to be due to various factors, both congenital and acquired, including longer life with good health care, constipation, irritable bowel syndrome, stress and the tendency of eating patterns to more closely resemble those of the west.

6.The incidence of ABL Deletion on Derivative 9 Chromosome in Chronic Myelogenous Leukemia by Interphase Fluorescence In Situ Hybridization and its Association with Progression to Blast Crisis.

BACKGROUND: Philadelphia(Ph) chromosome is found in about 95 percent of chronic myelogenous leukemia(CML) patients. Ph chromosome results from a reciprocal translocation between the long arms of chromosomes 9 and 22, and the fusion gene, BCR-ABL contribute to oncogenesis. Three to five years after first diagnosis, CML progresses to the blast crisis, and is accompanied by secondary cytogenetic changes in about 85% of cases. In this study, we investigated the incidence of ABL deletion of derivative 9 chromosome in CML and evaluated the association between this deletion and progression to the blast crisis by interphase fluorescence in situ hybridization(FISH). METHOD: The subjects included in this study were a consecutive series of 58 patients who were diagnosed as CML at Seoul National University Hospital between January 1997 and April 2000. On 90 archival bone marrow aspirate samples from these 58 CML patients, interphase FISH was performed with a commercially available probe. RESULTS: The ABL deletion of derivative 9 chromosome was detected in 17(29.3%) of 58 patients with CML. Eighteen of 58 patients progressed to blast crisis in this period. ABL deletion was found in 7 of 18 patients with blast crisis, and not in 11 remainders. The mean duration from the diagnosis to blast crisis was 37.1 months in 7 patients with the ABL deletion, while the mean duration was 74.2 months in 11 patients without the ABL deletion. The mean duration from the diagnosis to blast crisis in patients with ABL deletion was significantly shorter than in patients without ABL deletion(P=0.043). CONCLUSIONS: We found that 29.3% of patients with CML had the ABL deletion on derivative 9 chromosome. In these patients, the time taken for evolution to blast crisis was significantly shorter than that of the patients without ABL deletion.

7.The Size of the Main Pulmonary Artery in Patients with IPF: Relationship with the Extent of Honeycombing Lesion.

PURPOSE: To predict pulmonary arterial hypertension by comparing the extent of honeycombing lesion with the diameter of main pulmonary artery (MPA). MATERIALS AND METHODS: We retrospectively reviewed 21 patients (20 men, one woman) who showed honeycombing lesion on HRCT, and also analysized a control group of 30 (29men, one woman) who had no cardiopulmonary symptom and no abnormality on chest CT. On Chest HRCT, we scanned at four levels [1)aortic arch, 2) bifurcation of bronchus, 3) mid-portion of left ventricle, 4) dome of right diaphragm], and two radiologists then evaluated the proportion of honeycombing lesion within the entire lung field. According to the extent of this, we divided the patients into five groups [<20% (I), 20-39% (II), 40-59% (III), 60-79% (IV), > or = 80% (V)], and then divided Group I into two subgroups [<10 % (I|1), 10-19% (I-2)]. Using contrast-enhanced routine CT, the main pulmonary artery (MPA) was measured at its the widest diameter perpendicular to the long axisat the level of its bifurcation. RESULT: In group I (n=11) MPA was 28.4+/-2.6mm ; in group II (n=6), it was31.7+/-2.0mm ; in group III (n=2), 33.5mm ; in group IV (n=1), 33mm ; and in group V (n = 1), 39mm. In subgroup I-1MPA was 26.1+/-1.9mm, and in subgroup I-2, it was 29.8+/-1.6mm. In the control group, MPA was 25.1+/- 2.0mm. CONCLUSION: The extent of honeycombing lesion is closely related to the change in diameter of the MPA. Honeycombing lesion extending over more than 10% of the lung field may lead to pulmonary arterial hypertension.

8.Nutritional Assessment and the Effectiveness of Dietary Counseling in Infants and Young Children with Iron Deficiency Anemia.

PURPOSE: Iron deficiency is still the most common nutrient deficient disorder despite the improvement in general health and nutrition. This study is designed to evaluate the dietary history of infants and young children with iron deficiency anemia(IDA) and the effects of nutritional counseling. METHODS: This study was conducted on 120 children from 6 to 36 months of age with IDA. Their parents completed a questionnaire and took counsel for nutrition. IDA was defined as Hb <11.0 g/dL, ferritin <10 ng.mL or transferrin saturation <15%, or Hb increase >1 g/dL after iron preparation. The questionnaire consisted of their feeding patterns, weaning time and kinds of food. RESULTS: In the 120 infants and young children aged from 6 to 36 months, the parents of 82 cases was counseled about nutrition. Fifty six infants among 82 cases have started weaning and the main foods of weaning were rice and/or rice gruel. Nutritional problems in weaning were that some children over one year of age were using a bottle, and parents restricted weaning food at will because of allergic disease or chronic disease. Most parents were satisfied with the nutritional counseling given from a clinical dietitian and showed good compliance. CONCLUSION: Many infants and young children with IDA were provided with non iron-fortified foods and made an inadequate wean. Most parents were satisfied with the nutritional counseling and showed good compliance. The need of dietary counseling was required for prevention and treatment of iron deficiency anemia because of inadequate weaning.

9.Intraoperative management of liver transplant recipients having severe renal dysfunction: results of 42 cases.

PURPOSE: Whereas continuous renal replacement therapy (CRRT) has been utilized during liver transplantation (LT), there was a lack of evidence to support this practice. We investigated the adverse events at the perioperative periods in recipients of LT who received preoperative CRRT without intraoperative CRRT. METHODS: We retrospectively reviewed medical records of adult patients (age ≥ 18 years) who received LT between December 2009 and May 2015. Perioperative data were collected from the recipients, who received preoperative CRRT until immediately before LT, because of refractory renal dysfunction. RESULTS: Of 706 recipients, 42 recipients received preoperative CRRT. The mean (standard deviation) Model for end-stage liver disease score were 49.6 (13.4). Twenty-six point two percent (26.2%) of recipients experienced the serum potassium > 4.5 mEq/L before reperfusion and treated with regular insulin. Thirty-eight point one percent (38.1%) of recipients were managed with sodium bicarbonate because of acidosis (base excess <−10 mEq/L throughout LT). All patients finished their operations without medically uncontrolled complications such as severe hyperkalemia (serum potassium > 5.5 mEq/L), refractory acidosis, or critical arrhythmias. Mortality was 19% at 30 day and 33.3% at 1 year. CONCLUSION: Although intraoperative CRRT was not used in recipients with severe preoperative renal dysfunction, LT was safely performed. Our experience raises a question about the need for intraoperative CRRT.

10.Papillary Tumors of the Breast: US Findings of the Benign and Malignant Lesions.

PURPOSE: To determine which sonographic findings usefully differentiate between benign and malignant papillary tumors. MATERIALS AND METHODS: We retrospectively rev i ewed the ultrasonographic findings of 42 surgically proven cases of papillary breast lesions [11 malignant lesions (7 inva s i ve papillary carcinomas, 4 intraductal papillary carcinomas) and 31 benign intraductal pa-pillomas]. All 42 cases were classified sonographically as cystic or ductal, or solid type, and the shape, wall change, margin, internal echo-pattern, posterior echo change and other associated findings for the two types were then analysed. RESULTS: Among the 25 cases (5 malignant and 20 benign) of cystic or ductal type, tubular shaped lesions were more frequently benign (60%). In all 20 benign lesions the wall of cystic portion was well-defined, smooth and thin. The solid portion of the cystic type showed an illdefined irregular margin in four malignant lesions (80%) and a smooth margin in 19 which were benign (95%). The internal echo-pattern was heterogeneous mixed-echo in three cases of malignancy, and homogeneously hypoechoic in 19 benign lesions (95%). Posterior enhancement was seen in two malignant lesions (40%), while in 19 benign lesions (95%), there was no posterior echo change. There were 17 solid type lesions (6 malignant cases, 11 benign cases), and most of these, whether benign or malignant, were smooth, oval or lobulated, hypoechoic masses. Posterior enhancement, howeve r, was more frequently observed in malignant lesions (three cases, 50%) than in those which were benign (one case, 9%). CONCLUSION: In cystic or ductal type lesions, an ill-defined irregular thick cystic wall, an illdefined irregular margin, a heterogeneous mixed internal echo-pattern and posterior enhancement of the solid portion suggested malignancy. In solid type lesions, posterior enhancement was more frequently found in malignant than in benign lesions.
