OBJECTIVE: To test if the intake of H2 receptor antagonists (H2-RAs) increases the risk of gastric cancer in the elderly. METHODS: The source population for this study was drawn from the responders to a questionnaire survey administered to the Korea Elderly Pharmacoepidemiological Cohort (KEPEC), who were beneficiaries of the Korean Medical Insurance Corporation, were at least 65 years old, and residing in Busan in 1993. The information on H2-RAs exposure was obtained from a drug prescription database compiled between Jan. 1993 and Dec. 1994. The cases consisted of 76 gastric cancer patients, as confirmed from the KMIC claims data, the National Cancer Registry and the Busan Cancer Registry. The follow-up period was from Jan. 1993 to Dec. 1998. Cancer free controls were randomly selected by 1:4 individual matching, which took in to consideration the year of birth and gender. Information on confounders was collected by a mail questionnaire survey. The odds ratios, and their 95% confidence intervals, were calculated using a conditional logistic regression model. RESULTS: After adjusting for a history of gastric ulcer symptoms, medication history, and body mass index, the adjusted OR (aOR) was 4.6 (95% CI=1.72-12.49). The odds ratio of long term use (more than 7 days) was 2.3 (95% CI=1.07-4.82). The odds ratio of short term use was 4.6 (95% CI=1.26-16.50). The odds ratio of parenteral use was 4.4 (95% CI=1.16-17.05) and combination use between the oral and parenteral routes (aOR, 16.8; 95% CI=1.21-233.24) had the high risk of gastric cancer. The aOR of cimetidine was 1.7 (95% CI=1.04-2.95). The aOR of ranitidine was 2.0 (95% CI=1.21-3.40). The aOR of famotidine was 1.7 (95% CI=0.98-2.80). CONCLUSION: The intake of H2-RAs might increase the risk of gastric cancer through achlorhydria in the elderly.

Purpose: The purpose of this study was to investigate the effects of emotional labor and communication competence on geriatric nursing in nurses-related stress in nurses caring for older patients. Methods: The sample consisted of 147 general hospital nurses. Data were analyzed through the SPSS/WIN 21.0 program using descriptive statistics, t-test, ANOVA, Pearson’s correlation coefficients, and multiple regression. Results: The factors of emotional labor (β=.38, p<.001)and positive communication competence (β=-.20, p=.021) had the greatest influence on the level of stress related to geriatric nursing in nurses caring for older patients. The total explanatory power was 20.3%. Conclusion It was found that emotional labor and communication competence were influencing factors in the stress related to geriatric nursing. Therefore, to improve nurses’ stress related to geriatric nursing, strategies to manage the emotional labor, and to strengthen and develop positive communication competence need to be developed.

OBJECTIVES: The purpose of this study was to compare psychological test profiles of psychiatric outpatients with high and low depression/suicide ideation and to identify predictor variables for depression/suicide ideation. METHODS: Component scores of the Personality Assessment Inventory (PAI) and Minnesota Multiphasic Personality Inventory-2 (MMPI-2) were compared using t-tests. Discriminant analysis was conducted for predictor variables of depression/suicide ideation. RESULTS: Regarding PAI profiles, somatic complaints (SOM), anxiety (ANX), anxiety-related disorder (ARD), depression (DEP), paranoia (PAR), borderline features (BOR), antisocial features (ANT), mania (MAN) drug problems (DRG) scores were significantly elevated in high depression and high suicide ideation groups. Concerning MMPI-2 profiles, the scores of hypochondriasis (Hs), depression (D), hysteria (Hy), psychopathic deviate (Pd), paranoia (Pa), psychasthenia (Pt), schizophrenia (Sc), social introversion (Si) were significantly elevated in these same groups. The PAI and MMPI-2 profile shapes were remarkably similar between high depression and high suicide ideation groups. Therefore, in terms of psychological profile, depression and suicidal ideation seemed to reflect the same construct. However, in discriminant analysis, significant predictors for depression were found to be Pt and D Sc from MMPI-2, while those for suicide ideation were found to be Pa and Sc, suggest subtle differences. CONCLUSION: The superficial characteristics of depression and suicide ideation groups reflected by the psychological test profiles seemed similar, but the determining factors may differ. Thus, the psychological interventions for these two groups may have to follow different routes considering these subtle differences.

OBJECTIVE: The Center for Epidemiological Studies Depression Scale (CES-D) is designed to measure the current level of depressive symptomatology in the general population. However, no review has examined whether the scale is reliable and valid among children and adolescents in Korea. The purpose of this study was to test whether the Korean form of the CES-D is valid in adolescents. METHODS: Data were obtained from 1,884 adolescents attending grades 1–3 in Korean middle schools. Reliability was evaluated by internal consistency (Cronbach’s alpha). Concurrent validity was evaluated by a correlation analysis between the CES-D and other scales. Construct validity was evaluated by exploratory factor and confirmatory factor analyses. RESULTS: The internal consistency coefficient for the entire group was 0.88. The CES-D was positively correlated with scales that measure negative psychological constructs, such as the State Anxiety Inventory for Children, the Korean Social Anxiety Scale for Children and Adolescents, and the Reynold Suicidal Ideation Questionnaire, but it was negatively correlated with scales that measure positive psychological constructs, such as the Korean version of the Rosenberg Self-Esteem Scale and the Connor-Davidson Resilience Scale-2. The CES-D was examined by three-dimensional exploratory factor analysis, and the three-factor structure of the scale explained 53.165% of the total variance. The variance explained by factor I was 24.836%, that explained by factor II was 15.988%, and that explained by factor III was 12.341%. The construct validity of the CES-D was tested by confirmatory factor analysis, and we applied the entire group’s data using a three-factor hierarchical model. The fit index showed a level similar to those of other countries’ adolescent samples. CONCLUSION: The CES-D has high internal consistency and addresses psychological constructs similar to those addressed by other scales. The CES-D showed a three-factor structure in an exploratory factor analysis. The present findings suggest that the CES-D is a useful and reliable tool for measuring depression in Korean adolescents.

Purpose: Preterm infants are known to be at a risk of neurodevelopmental delay; however, limited data are available on the outcomes of moderate-to-late preterm (MLPT) infants (born at 32 to 36 weeks’ gestation). The Korean Developmental Screening Test (K-DST) for infants and children is a recently designed screening test for Korean infants and children. The current study aimed to evaluate the neurodevelopmental outcomes of MLPT infants and investigate the risk factors associated with neurodevelopmental delay. Methods: A total of 119 MLPT infants admitted to a neonatal intensive care unit (NICU) of a tertiary hospital in Korea were enrolled. The infants were assessed during two follow-up periods (first: 16 to 24 months of corrected age; second: 24 to 41 months of corrected age). The perinatal factors in the NICU that were associated with delayed development were analyzed. Results: In all sections of the K-DST, the proportion of infants with developmental delay was higher in the second period (5.6% to 9.3%) than in the first period (0.9% to 5.4%). A total of 10% to 17% of the infants presented with persistent delay throughout the two periods based on five sections of the K-DST. Male sex, oxygen therapy duration, and younger maternal age were the risk factors affecting at least one section during the second period. Conclusion MLPT infants showed greater developmental delay than the general infant population. Considering that early intervention is important for good longterm outcomes, close observation of male MLPT infants and MLPT infants who received oxygen therapy is warranted.

PURPOSE: The aim of this study was to evaluate the difference of clinical features among patients with enteroviral mengitis according to the presence of cerebrospinal fluid (CSF) pleocytosis. METHODS: This was a retrospective analysis of the patients' data diagnosed with enteroviral meningitis by CSF reverse transcriptase polymerase chain reaction (RT-PCR). We reviewed the medical records of children younger than 16 years who visited Korea University Ansan Hospital and Guro Hospital for meningitis or encephalitis between March 2013 and August 2013. Clinical and laboratory variables were compared with regard to the presence of CSF pleocytosis. RESULTS: Among 49 patients with enteroviral meningitis, eight of 49 (16.3%) did not have pleocytosis. The enteroviral meningitis patients without pleocytosis had lower CSF protein level than patients with pleocytosis (18.9+/-4.3 vs 34.9+/-18.3 mg/dL; P=0.002). Age, duration of hospital stay, severity of clinical symptom, peripheral white cell blood counts, absolute neutrophil counts and CSF/serum glucose ratio were not different between two groups. CONCLUSION: Enteroviral meningitis could manifest without pleocytosis. We therefore think that CSF RT-PCR is helpful for the diagnosis, especially in suspected cases, which leads to shorter hospital stay and minimal use of antibiotics.

Vascular neoplasms are the most common non-epithelial tumors in the nasal and paranasal regions. However, heman-giopericytomas usually occur in the retroperitoneum or in the thigh and are uncommon in the nasal cavity and paranasal sinuses. Nasal hemangiopericytoma-like (HPCL) tumors should be differentiated from typical hemangiopericytomas. HPCL tumors, a distinctive variety of vascular neoplasm, often originate in a paranasal sinus and extend into the nasal cavity secondarily. They occur most commonly in adults in the sixth and seventh decades of life and clinically mimic allergic polyps with symptoms of nasal obstruction and epistaxis. Microscopically, HPCL tumors are composed predominantly of spindle cells and lack nuclear or cytoplasmic pleomorphism, mitotic activity, hemorrhage, or necrosis demonstrating a vascular architecture. We report a rare case of HPCL tumor originating from nasal septum in a 45-year-old man with the following immunohistochemical features: vimentin (+), CD34 (+), S-100 protein (-), desmin (-), factor VIII-related antigen (-), and fuchinophilia on Masson trichrome stain.

The replacement of missing teeth, especially in the anterior region, is an essential part of dental practice. Fiber-reinforced composite resin bridges are a conservative alternative to conventional fixed dental prostheses or implants. It is a minimally invasive, reversible technique that can be completed in a single visit. The two cases presented herein exemplify the treatment of root-fractured anterior teeth with a natural pontic immediately after extraction.
