1.Two Cases of Nickel Dermatitis.

Nickel sensitivity is the most common cause of allergic metal dermatitides,, particularly in women. Women are uaually sensitized. by the objects wom close to the skin ('e.g. earring, necklace fasther, watch, suspenders, brassiere clips, etc.), and men are by occupational exposures (e.g. plating or printing industries, tools, etc.). Nickel dermatitis is frequently developed on the sites where seems to have been directly contact witb nickel substance, but secondary eruptions may develop on the areas where seems to have not been directly exposed to nickel, so that nickel dermatitie is sometimea misdiagnosed as atopic dermatitis, nummular eczema, stasis dermatitis, or other dermatitides. The authora prcsent two cases of nickel dermatitis confirmed by patch test with 2. 5% nickel sulfafte; first case-a 22 years old female who has erythemat,ous maculopapular patches on her ears, around neck, left wriat, elbows, and abdomen; second case tkat was previously misdiagnosed as mummular eczema-a 18 years old female who has large oozing and crusted patches on both lateral aspects of her right thigh and back, The authors reviewed the incidence of nickel sensitivity during past years in korea and the clinical aspect of distribution of nickel dermatitis.

2.The Differential expression of matrix metalloproteinase and their tissue inhibitors in myometrium and leiomyoma.

OBJECTIVES: To determine the level of mRNA expression of various members of the matrix metalloproteinase and tissue inhibitors in uterine leiomyoma compared with unaffected myometrium. Materials & Method: 30 cases of portions of leiomyoma and myometrium were collected immediately followimg hysterectomy. Thirteen cases were from proliferative phase and seventeen were from secretory phase of menstrual cycle. The mean age was 43.7years old. The level of expression of mRNAs of interstitial collagenase, gelatinase, stromelysin, TIMP-1,-2,-3 was determined by reverse transcriptase-polymerase chain reaction(RT-PCR) and normalized to GAPDH(glyceraldehyde-3-phosphate dehydrogenase) mRNA. RESULTS: Myometrium and leiomyoma expressed all the members of above mentioned matrix metalloproteinase family and tissue inhibitors. Leiomyoma expressed a significantly higher level of stromelysin-3 during secretory phase, an extremely lower level of 92kDa gelatinase and a significantly lower level of TIMP-3. The immunohistochemical localization of TIMP-3 was smooth muscle cell and arteriole wall of myometrium and leiomyoma. CONCLUSIONS: The increased expression of stromelysin-3 in uterine leiomyoma compared with myometrium suggests that this MMP may be involved in the formation of a more fibrous extracellular matrix in leiomyoma. The extremely lower expression of 92kDa gelatinase of leiomyoma means that leiomyoma do not invade myometrium and forms a separated mass. Decreased expression of TIMP-3 of leiomyoma suggests that TIMP-3 is required for differentiation and homeostasis of extracellular matrix of normal myometrium and function as a suppressive role of tumor development

3.Clinical Nurses' HPV-related Knowledge and Perception of Cancer Causes: HPV Vaccinated vs. Not Vaccinated.

PURPOSE: The purpose of this study was to compare knowledge level of those clinical nurses who received HPV vaccine and those who did not and their perception of the relatedness of HPV vaccine to causes of cervical cancer. METHODS: A total of 249 clinical nurses were surveyed from June to July, 2009. The questionnaire originally developed by Kim & Ahn (2007) examined HPV-related knowledge originally and the tool for perception of the causes of cervical cancer was originally developed by Kim (1993). The total number of subjects equaled to: vaccination group of 52 (20.9%) and non-vaccination group of 197 (79.1%). RESULTS: Vaccination group showed significantly higher score of both knowledge of HPV vaccination and the perception of the cause of cervical cancer in comparison to the nonvaccination group at (p<.05). Among 4 subscales of the perception of causes of cervical cancer, destiny and constitution subscale scores were significantly different between the two groups at (p<.05). CONCLUSION: Clinical nurses need to constantly update with current knowledge of HPV and be prepared with currently changing cancer prevention strategies, especially in cervical cancer.

5.Non-linear System Dynamics Simulation Modeling of Adolescent Obesity: Using Korea Youth Risk Behavior Web-based Survey.

PURPOSE: The purpose of this study was to develop a system dynamics model for adolescent obesity in Korea that could be used for obesity policy analysis. METHODS: On the basis of the casual loop diagram, a model was developed by converting to stock and flow diagram. The Vensim DSS 5.0 program was used in the model development. We simulated method of moments to the calibration of this model with data from The Korea Youth Risk Behavior Web-based Survey 2005 to 2013. We ran the scenario simulation. RESULTS: This model can be used to understand the current adolescent obesity rate, predict the future obesity rate, and be utilized as a tool for controlling the risk factors. The results of the model simulation match well with the data. It was identified that a proper model, able to predict obesity probability, was established. CONCLUSION: These results of stock and flow diagram modeling in adolescent obesity can be helpful in development of obesity by policy planners and other stakeholders to better anticipate the multiple effects of interventions in both the short and the long term. In the future we suggest the development of an expanded model based on this adolescent obesity model.

7.Effect of Several Solvents on Irritant Dermatitis Measured by Non - invasive Techniques.

BACKGROUND: Solvents play an immense role in the industria sector. Irritant dermatitis which is more common than allergic contact dermatitis can be caused by solvents. OBJECTIVE: Our purpose was to compare the skin irritancy of several solvents using human and guinea pig skin models. METHODS: The skin responses to short contact with etharol, acetone, dimethylsulfoxide (DMSO) and xylene were measured by visual scoring of erythm, transepidermal water loss and laser doppler flowmetry. RESULTS: The results are summarized as follows . 1. Guinea pig and human skin responses to normal saline, ethanol, and acetone were nearly negligible. 2. Guinea pig skin responses to 99.9% DMSO under occlusion for 0.5 min were assessed by visual scoring system, TEWL, and LDF. They are measured 3+0.(0,1+21.70, 45+12.70 at 5 min after removal of 99.9% DMSQ, and 0.83+0.41, 10.5+3.83, 36+4.0, to 120 min after removal. 3. Guinea pig skin responses to 97% xylene under occlusion for 5 nin were assessed by visual scoring system, TEWL and LDF. They are measured 3+0.00, 1. 5.82, 77+11.7 at 5 min after removal of 97% xylene, and 1.83+0.75, 5.5+3.21, 39.17+11.53 at l2 min after removal. 4. Human skin responses to 75% DMSO under occlusion for 1 min were assessed by visual scoring system, TEWL and LDF. They are measured 2,5+0.5, 63+25.8, 51+13.7 at 5 min after removal of 75% DMSO, and 0.17+0.41, 14.67+15.87, 21.17-8. 1 at 120 min after removal. 5. Human skin responses to 97% xylene under occlusion for 12 min were assessed by visual scoring system, TEWL and LDF. They are measured 2.7+0.52+4.22, 76+14.30 at 5 min after removal of 97% xylene, and 0+0, 2.5+0.55, 3.17+0.98, 120 emOVBI. CONCLUSION: Short contact vrith DMSO and xylene cause visib erythema and an increase in TEWL and cutaneous blood flow. The reaction patterns in hurr an and guinea pig skin models were similar.

8.Model for Unplanned Self Extubation of ICU Patients Using System Dynamics Approach.

PURPOSE: In this study a system dynamics methodology was used to identify correlation and nonlinear feedback structure among factors affecting unplanned extubation (UE) of ICU patients and to construct and verify a simulation model. METHODS: Factors affecting UE were identified through a theoretical background established by reviewing literature and preceding studies and referencing various statistical data. Related variables were decided through verification of content validity by an expert group. A causal loop diagram (CLD) was made based on the variables. Stock & Flow modeling using Vensim PLE Plus Version 6.0b was performed to establish a model for UE. RESULTS: Based on the literature review and expert verification, 18 variables associated with UE were identified and CLD was prepared. From the prepared CLD, a model was developed by converting to the Stock & Flow Diagram. Results of the simulation showed that patient stress, patient in an agitated state, restraint application, patient movability, and individual intensive nursing were variables giving the greatest effect to UE probability. To verify agreement of the UE model with real situations, simulation with 5 cases was performed. Equation check and sensitivity analysis on TIME STEP were executed to validate model integrity. CONCLUSION: Results show that identification of a proper model enables prediction of UE probability. This prediction allows for adjustment of related factors, and provides basic data do develop nursing interventions to decrease UE.

9.A Longitudinal Study on the Change of Nutrients and Food Consumption with Advance in Age among Middle-aged and the Elderly.

The purpose of this study is to investigate diet quality and changes in nutrient and food intake with increase in age. Subjects were 69 patients(28 males, 41 females) living in Jeon-ju city, over middle-aged, and they were the same patients studied 4-7 years ago in a previous study. Dietary survey with one day 24-hour recall method was used. Energy, calcium, vitamin A and vitamin B2 intakes were lower than the RDA and vitamin C was in excessive status in pre-test and post-test. In food groups examrned, the consumption of potatoes, beans, vegetables, seaweeds, beverages, seasonings, oils, fish & shells and milks was increased amount than 4-7 years before but the consumption of cereals(p<0.01), sugars, seeds, fruits, processed food, meats and eggs was decreased. Diet quality was assessed by %RDA, nutrient adequacy ratio(NAR), mean adequacy ratio(MAR), dietary diversity score(DDS), meal balance and food group pattern. In %RDA, those proportion of appropriate intake in post-test in were higher than those in pre-test. For most nutrient except vitamin A, the levels of NAR in post-test were higher than in pretest. The MAR, an index of overall dietary quality, was 0.77 for pre-test and 0.83 for post-test(p<0.05). When we counted the major food groups consumed(KDDS), 62.3% of subjects had a KDDS of 3 in pre-test and 42.0% of subjects had a KDDS of 4 in post-test. Based on these results, in terms of variety and balance, total diet quality was improved with advancing of age

10.Change on the Food and Nutrient Intake Patterns of Men over Thirty Years Old in Jeon-ju Area.

The objective of this study was to estimate changes on the flood and nutrient intake patterns of men over thirty years old in Jeon-Ju area. The first survey was conducted from December in 1991 to January in 1992, tile second one from January to February in 1994 and the third one from July to August in 1997. The nutrition survey using 24-hour recall method was executed to 303 subjects : 89, 82, 132 in 1991, 1994 and 1997, respectively. Results of the study are as follows : Kimchi, rice, garlic and onions were the most frequently eaten food items. Total daily intakes of foods were 85 : 15, 81 : 19 and 81 : 19 in 1991, 1994 and 1997, respectively. The average numbers of foods per person were 15.7, 20.1 and 21.9 daily in 1991, 1994 and 1997, respectively and tends to increase significantly(p<0.05). The minimum numbers of foods per person were 4, 7 and 9 and the maximum numbers of foods per person were 27, 35 and 39 in 1991, 1994 and 1997, respectively. KDDS(Korean's Dietary Diversity Score) is determined by how many among the five food groups (cereals, vegetables, meats, milks, oils groups)were consumed per day. Most subjects earned the KDDS "3" ; 61, 46 and 42% in 1991, 1994 and 1997, respectively. Average daily energy intakes wee 1,62 kcal(72% of RDA), 2,063 kcal(89% of RDA) and 1,818 kcal (79% of RDA) in 1991, 1994 and 1997, respectively. Energy intake rates of cereals : total energy intake were 65, 59, and 60% in 1991, 1994 and 1997, respectively, which were decreasing. Protein intakes were 58g(72% of RDA), 79g(107% of RDA) and 71g(97% of RDA), respectively and animal protein comprised 46, 53, and 59%, respectively ; which were increased. Fat intakes were 12g, 20g and 20g, respectively and animal protein comprised 38, 46, and 48% ; which were increased. Fat intakes were 12g, 20g and 20g, respectively, of which animal fat comprised 46, 53, and 59%, respectively ; which were increased, too. Malnourished (under 75% of RDA) rates were respectively 64, 34, and 47% in terms of energy ; 64, 31 and 33% in protein ; 67, 51, and 61% in calcium ; 53, 26, and 18% in iron ; 85, 74 and 84% in Vitamin A. Super-nourished(above 125% of RDA) rates were respectively 1, 13, and 3% in energy ; 1, 29, and 21% in protein ; 5, 18, and 7% in calcium ; 16, 31, and 7% in iron ; 16, 31, and 7% in Vitamin A, 42, 76, and 62% in Vitamin C. The percentages of calories from protein : fat : carbohydrate were 14 : 12 : 74, 15 : 16 : 69 and 16 : 17 : 67 in 1991, 1994 and 1997, respectively. KDDS(number of five food groups per day), Meal Balance(number of five food groups per meal), DVS(average numbers of foods per person), amount of foods correlated positively with all the nutrient intakes(p<0.05). KDDS was positively correlated with energy, protein, fat, calcium, thiamin, riboflavin and niacin(p<0.05)
