3.MR Findings of Angiomyoma of the Extremities: Two Cases Report.

Angiomyoma is a benign soft tissue tumor that arises from the smooth muscle layer of blood vessels in the deeplayers of the dermis, or in subcutaneous tissue. It occurs as a well-encapsulated solitary mass, most of which areless than 2cm in diameter. Some patients complain of pain with or without tenderness. This tumor has apredilection for the lower extremities of middle-aged females. MRI findings of angiomyoma of the extremities haverarely been reported, and we describe these findings in two cases of angiomyoma of the hand and foot.

5.Importance-performance Analysis of Patients' and Nurses' perspectives on Rehabilitation Nursing Services.

PURPOSE: This study attempted to understand patients' and nurses' perspectives on the priority of rehabilitation nursing services using Importance-Performance Analysis (IPA). METHODS: This study used descriptive research design. Data were collected from 121 patients and 144 nurses using self-reported questionnaires. Statistical analysis included an independent t-test, analysis of variance (ANOVA), and IPA conducted using SPSS/WIN 21.0 version. RESULTS: There were no statistical differences between the patients' and nurses' mean scores on perceptions of the importance (t=-0.83, p=.409) and performance (t=-0.32, p=.751) of rehabilitation nursing services. The IPA matrix showed a difference between patients and nurses in terms of their perceived priority of nursing services. Regarding the perception of patients, "helping a patient to continue to practice bedside physiotherapy and occupational therapy", "providing information on the proper care agency and community resources", and "providing education for the prevention of complications" fell in the "concetrate here" area (2nd quadrant). CONCLUSION: The results showed that the priorities of patients and nurses did not match in terms of some of the rehabilitation nursing services. Thus, rehabilitation nursing services need to be provided based on the patients' needs. Allocation of resources for the service items that fell in the "concentrate here" area of the IPA need be reconsidered for the quality in nursing care.

6.Ultrastructural Localization of Cryptosporidium parvum Antigen Using Human Patients Sera.

The antigen location of Cryptosporidium parvum, which stimulates antibody formation in humans and animals, was investigated using infected human sera. Immuno-electron microscopy revealed that antigenicity-inducing humoral immunity was located at various developmental stages of parasites, including asexual, sexual stages, and oocysts. The amount of antigen-stimulating IgG antibodies was particularly high on the oocyst wall. The sporozoite surface was shown to give stimulation on IgG and IgM antibody formation. Trophozoites implicated the lowest antigenicity to humoral immunity, both IgG and IgM, by showing the least amount of gold labeling. Immunogold labeling also provided clues that antigens were presented to the host-cell cytoplasm via feeder organelles and host-parasite junctions.

7.Delayed Vertebral Artery Dissection after Posterior Cervical Fusion with Traumatic Cervical Instability: A Case Report.

Vascular injury presented immediately after the penetration, but delayed onset of vascular symptom caused by an embolism or vessel dissection after cervical fusion or traumatic event is extremely rare. We present a case of a 56-year-old woman who underwent an operation for cervical fusion for type II Odontoid process fracture. She presented symptoms of seizure with hemiparesis in 6 days after the operation. Multifocal acute infarction due to an embolism from the left VA (V3 segment) dissection was observed without a definite screw breach the transverse foramen. We hereby reported the instructive case report of delayed onset of vertebral artery dissection after posterior cervical fusion with type II odontoid process fracture patient. When a cervical operation performed in the cervical trauma patient, even if no apparent VA injury occurs before and during the operation, the surgeon must take caution not to risk cerebral infarction because of the delayed VA dissection.

It was reported that the etiologies of recurrent spontaneous abortion are immunologic factors, endocrinologic problems, anatomical abnormalities, genetic abnormalities, infection, and unexplained factors. Among those etiologic factors, genetic abnormalities occur in about 5% of the couples who experience recurrent spontaneous abortions, and most common parental chromosomal abnormality contributing to recurrent abortion is balanced translocation. The advent of in vitro fertilization (IVF), the development of skills associated with the handling of human embryo, and an explosion of knowledge in molecular biology have opened the possibility of early diagnosis of genetic disease in preimplantation embryos. Therefore preimplantation genetic diagnosis (PGD) is indicated for couples, infertile or not, at risk of transmitting a genetic disease. A case of successful pregnancy and term delivery by PGD using fluorescence in situ hybridization (FISH) technique in patient with recurrent spontaneous abortion due to balanced translocation is presented with brief review of literatures.

9.Assessment of Intraepidermal Nerve Fiber using Skin Biopsy in Diabetic Polyneuropathy.

BACKGROUND: Immunohistochemical staining of skin for panaxonal marker, protein gene product 9.5 (PGP 9.5), has recently emerged as a useful diagnostic procedure for the assessment of small nerve fibers. The first aim of our study is to quantify the intraepidermal nerve fibers (IEDNF) and to investigate the histological changes of IEDNF in diabetic patents. And the second is to evaluate whether IEDNF has the correlation with the clinical, electrophysiological and biological parameters. METHODS: Skin biopsy specimens were obtained at the distal leg and proximal thigh from twelve type 2 diabetes mellitus patients having the symptoms of peripheral polyneuropathy. Fixed sections were stained using PGP 9.5, and the number of intraepidermal nerve fibers at each site was compared with those of fifteen normal healthy volunteers. We analyzed the correlation of IEDNF density with the clinical history, neuropathic symptom score (NSS), severity of diabetes (serum glucose, HbA1C), and nerve conduction studies. RESULTS: The number of IEDNF in diabetic patients was 3.0 per millimeter at the distal leg and 12.7 per millimeter at the proximal thigh compared with 15.3 per millimeter and 23.3 per millimeter in normal control respectively. Some specimen showed a morphological change of axons such as axonal swelling. The IEDNF density was not correlated with the duration of disease, biological or electrophysiological parameters but correlated with the clinical neuropathic symptom score. CONCLUSIONS: The IEDNF density is reduced in all the diabetic patients, more severely at the distal extremity. We suggest skin biopsy can provide the quantification as well as the objective evidence for the small fiber involvement of diabetic neuropathy.

10.Skin entrance dose for digital and film radiography in Korean dental schools.

PURPOSE: This study was aimed to compare skin entrance dose of digital radiography with that of film radiography and to show the dose reduction achievement with digital systems at 11 dental schools in Korea. MATERIALS AND METHODS: Forty six intraoral radiographic systems in 11 dental schools were included in this study. Digital sensors were used in 33 systems and film was used in 13 systems. Researchers and the volunteer visited 11 dental schools in Korea. Researchers asked the radiologic technician (s) at each school to set the exposure parameters and aiming the x-ray tube for the periapical view of the mandibular molar of the volunteer. The skin entrance doses were measured at the same exposure parameters and distance by the technician for each system with a dosimeter (Multi-O-Meter : Unfors instruments, Billdal, Sweden). RESULTS: The median dose was 491.2micronGy for digital radiography and 1, 205.0 microGy for film radiography. The skin entrance dose in digital radiography was significantly lower than that of film radiography (p< 0.05). CONCLUSIONS: Fifty-nine percent skin entrance dose reduction with digital periapical radiography was achieved over the film radiography in Korean dental schools.
