2.Differential approaches in the maloclusion with canine impaction.

During diagnostic process of the orthodontic patients, it is not unusual to find canine impaction. Generally, the chief complaint of the patients is crowding or antetior crossbite which is not related with canine impaction, but sometimes they complainted delayed eruption of the canine or deviation of the adjacent teeth caused by canine impaction. Orthodondists have to make the proper treatment plan according to final treatment goals. On the following cases, two patients were diagnosed as a malocclusion with canine impaction, and were treated by different accesses, one by extraction, and the other by non-extraction each.

3.A comparative study of bond strength of recycled brackets.

This study was undertaken to compare the bond strength and the fracture site of new and recycled brackets according to the base design. 252 sound premolars extracted for orthodontic treatment were collected, and Type I, Type II, Type III brackets were divided into four groups by recycling method. Each bracket was then bonded to an extracted premolar. Instron Universal Testing Machine(model 4466) was used to measure the shear bond strength, and the surface of the recycled brackets were viewed in SEM. For the analysis of the results, one way ANOVA and Scheffe's multiple range test was executed using the SPSSWIN program. 1. The shear bond strength showed statistically significant difference according to the bracket base design(P<0.001). Type III bracket(round indentation base, micro-etched) showed the highest bond strength, Type I bracket(foil-mesh base) was second, and Type II bracket(grooved integral base, micro-etched) was last. 2. The effect of recycling on the bond strength was different according to bracket type. The shear bond strength of Type I, Type II brackets showed the smallist reduction when treated for 1 minute in Big Jane(p<0.05), but the shear bond strength of Type III brackets showed no statistically significant difference according to recycling method(p>0.05). 3. In Type I, Type II brackets, frequent fracture site was bracket-resin interface, but in Type III brackets, about half of the resin was retained on the tooth surface frequently. 4. The shear bond strength was highest when about half of the resin was retained on the tooth surface(p<0.05). 5. The resin remnant on the bracket base after recycling had no effect on the shear bond strength.

4.Anesthesia for Living Related Liver Transplantation in Homozygous Protein C Deficiency.

Protein C exerts anticoagulant effects by inactivating factor Va and VIIIa and stimulating fibrinolysis. The homozygous protein C deficiency is extremely rare and often results in life threatening thrombosis and purpura fulminans with necrotic cutaneous lesions. A child with homozygous protein C deficiency was treated at 6 months by a living-related liver transplantaion. After induction of anesthesia, we started an FFP infusion for protein C replacement and a low molecular weight heparin continuous infusion to prevent thrombosis. A complete reconstitution of protein C activity and resolution of the thrombotic condition occured postoperatively. So we report this case with a brief review of the literature.

5.Primary Mucinous Carcinoma of the Skin with Unusual Histopathological Features.

Mucinous carcinoma can occur in non-cutaneous visceral sites and may metastasize to the skin. Thus it is important to exclude the possibility of a non-cutaneous visceral primary tumor before diagnosing primary mucinous carcinoma of the skin (PMCS). We describe a PMCS with unusual histopathological 6ndings. Although the microscopical, immunohistochemical, and ultrastructural characteristics of PMCS have been well defined, only less than sixty cases have been published since the disease was first reported by Lennox. This is an unusual case where combined features of apocrine carcinoma and predominant cribriform patterns reminiscent of adenoid cystic carcinoma exist in part, with highly anaplastic cellular features and frequent mitosis. To our knowledge, there have been no reports of this rare neoplasm in Korea.

6.An Unusual Case of Churg-Strauss Syndrome.

Churg-Strauss syndrome (CSS) is a very rare disease which combines the features of asthma, hypereosinophilia, multisystem necrotizing vasculitis and extravascular granuolma. To our knolwedge, there have been four reports in Korean journals, but none in dermatology journals. We report a 37-year-old male who had allergic rhinitis, pansinusitis, a history of atopic dermatitis, multiple prurigo-nodularis like skin lesions, non-fixed interstitial lung in-filtration, hypereosinophilia and subclinical asthma. Clinical and histopathological features of his skin lesions were nonspecific where perivascular lymphocytic infiltration and mild lymphocytic vasculitis were shown. The presence of a pituitary mass and proximal myopathy as op-posed to peripheral neuropathy have not been reported in previous reports of CSS. However, other features of our patient met the criteria of CSS.

7.Characteristics of Fall Events and Fall Risk Factors among Inpatients in General Hospitals in Korea

PURPOSE: The purpose of this study was to investigate the present status of falls among inpatients in general hospitals and to identify the fall risk factors that reflect the characteristics of domestic hospitals. METHODS: Data were collected between December 15, 2016 and January 15, 2017 from 32 Korean hospitals having 500 or more beds. First, 42 risk factors were extracted based on literature review and expert opinions. Then the importance of each factor was evaluated by 223 nurses from medical and surgical adult ward and intensive care units in 40 hospitals. RESULTS: The incidence rate of falls in 18 hospitals was 3.87 per 1,000 total discharged patients and 0.55 per 1,000 patient-days, and the rate of injury-related falls was 40.5%. Major risk factors for falling were identified as the following: being over 65 years of age, history of falls during admission, physical mobility disorders requiring assistance, physical factors (dizziness or vertigo, unstable gait, general weakness, walking aids, visual problems), cognitive factors (delirium, lack of understanding on limitations), neurological disease, CNS medications CONCLUSION: The findings provide information that can be in the development of a fall risk assessment tool for inpatients in general hospitals in Korea.

8.The Association of Relative Handgrip Strength with Impaired Fasting Glucose among Koreans Aged more than 20 Years Old

Background:
The prevalence of impaired fasting glucose (IFG) in Korea is increasing. Few studies have investigated the relationship between handgrip strength (HGS) and type 2 diabetes among Koreans; however, no study has investigated the relationship between IFG and HGS among Koreans. Therefore, we examined the relationship between relative IFG and HGS to evaluate HGS as a marker of prediabetes.
Methods:
This cross-sectional study analyzed data from participants aged 20 years or older (n=9,190) who did not have diabetes and had had their body mass index, fasting plasma glucose (FPG), and HGS measured in the 2016–2017 Korea National Health and Nutrition Examination Survey. The association between relative HGS and IFG was analyzed using complex sample logistic regression analyses after adjusting for age, education, strengthening exercise, aerobic exercise, smoking, excessive drinking, and chronic diseases.
Results:
The prevalence of IFG was 31.5 (0.9)% and 19.6 (0.7)% for men and women, respectively. According to the increase in quartile of relative HGS, FPG and HbA1c significantly decreased in both men and women (Ptrend<0.001). In multivariate logistic regression, the odds of IFG significantly decreased with the increase in quartile of relative HGS in both men and women (Ptrend=0.001 for men, Ptrend=0.002 for women).
Conclusion
This population-based, nationally representative study suggests that higher relative HGS is associated with a decreased risk of IFG for both men and women.
9.Sudden Death Related to Left Anterior Descending Coronary Artery Fistula Draining to the Left Ventricle: A Case Report with Review of the Literature.

Coronary artery fistula (CAF) is a rare congenital malformation, comprising less than 1% of all congenital heart disease. Most fistulas drain into the right-sided heart chambers or pulmonary artery, and termination in left heart chambers is an uncommon finding. Clinical presentation mainly depends on the severity of the left-to-right shunt, and adult patients usually do not develop any symptom. There are few case reports relating CAF manifested as an exceptional form of sudden unexpected death. We experienced a case of a 62-year-old male with a single CAF arising from left main coronary artery which drains into the left ventricle, and reported with review of the related articles.

10.The Impact of Auricular Acupressure on Depression among Nurses

Purpose:
This study evaluated the impact of auricular acupressure on depression among nurses.
Methods:
This study used a randomized control group pretest-posttest design. Shift-work nurses with the Patient Health Questionnaire-9 (PHQ-9) score over 5 points were randomized to the experimental group(n=20) or the control group(n=20). For the experimental group, auricular acupressure was conducted at the corresponding points of depression (TGI, AH1) weekly for seven days as well as for two weeks. The PHQ-9 was completed before intervention, as well as two weeks after intervention.
Results:
The mean difference in the PHQ-9 score before and after auricular acupressure in the experimental group (-4.11±2.27), was more significant than in the control group (-1.72±3.82) (t=2.28, p=.03).
Conclusion
Result showed that nurses who received auricular acupressure had a decrease in depression, compared to those who did not receive auricular acupressure. Further research is required, to apply the other auricular acupressure points, to evaluate the impact on depressive symptoms.
