In order to evaluate the safe ligation time and sites of dural venous sinuses during neurosurgical operation, systemic arterial pressure(SAP), cerebral perfusion perssure(CPP), intracranial pressure (ICP) and intrasinal pressure(ISP) were measured in cats through neuromonitor before and after obstruction of anterior 1/3, middle 1/3, posterior 1/3 of the superior sagittal sinus and the results were as follows. There were no significant increases of pressures after obstruction of anterior 1/3 of the superior sagittal sinus. In the obstructed middle 1/3 of the superior sagittal sinus group, signifi6int increases were seen un the m-SAP within 2, 4 and 6 minutes, the m-ICP within 2-7 minutes, the r.i-ISP within 14 minutes after obstruction. In the -obstructed posterior 1/3 of the superior sagittal sinus group, there were significant increases of m-SAP within 2, 4 and 6 minutes, the m-ICP within 27 minutes, the m-ISP within 13 minutes. In the obstructed Torcular Herophili,group, there were significant increases of the m-SAP within 1-2 and 4-7 minutes, the m-ICP within 36 minutes and the m-ISP within 1-7 minuted and less significant dercreases of the m-CPP within 5-7 minutes after obstruction. In the obstructed right transverse sinus group, significant increases of the m-ICP and the m-ISP were seen within 1-7 minutes after obstruction. there were no remarkable changes in the obstructed left transverse sinus group.

We report a case of adult granulosa cell tumor arising in the scrotal tunics. The patient was a 34-year-old man who presented with right scrotal swelling, first noticed four months previously. Under the initial clinical impression of epididymo-orchitis, antibiotic treatment was instituted but there was no response. The paratesticular nodules revealed by ultrasound and magnetic resonance imaging mimicked intratesticular lesion, and radical orchiectomy was performed. Although several cases of adult testicular granulosa cell tumor, have been reported, the occurrence of this entity in the paratesticular area has not, as far as we are aware, been previously described.

Following the implementation of the Act on Well-Dying in 2018 as an advance life-sustaining treatment law, the Assisted Justice Act was proposed on June 16, 2022. By further illuminating the meaning of well-dying in the context of hospital and hospice etymology, we understand that the life-sustaining treatment decision law and the legal system of assisted death with dignity are in-line with the principles of the right to self-determination and the right to happiness and dignity at the end of life. By understanding the individual’s right to make decisions, this aims to awaken awareness as to the realization of the values of human dignity so as to create a more desirable end-of-life culture.

Background: Umeclidinium/vilanterol (UMEC/VI; ANORO ELLIPTA, GSK) is a commonly used dual bronchodilator. This study evaluated the safety and effectiveness of UMEC/VI in Korean patients with chronic obstructive pulmonary disease (COPD) over a 6-year period. Methods: This was an open-label, multicentre, observational, post-marketing surveillance study. A total of 3,375 patients were enrolled consecutively in 52 hospitals, by 53 physicians, between July 2014 and July 2020. Patients who were administered UMEC/VI (fixed-dose 62.5 μg/25 μg) at least once and were monitored for safety and effectiveness were included in the analysis. Incidence and severity of adverse events (AEs) reported after administrating at least one dose of UMEC/VI were monitored, including unexpected adverse events (UAEs) and adverse drug reactions (ADRs). Effectiveness of UMEC/VI after 24 weeks of administration was also assessed using physician’s evaluation (effective, ineffectiveo change, worsening, indeterminable) and lung function improvement. Results: Of 3,375 patients, 3,086 were included in the safety assessment group (mean age±standard deviation: 69.76±8.80 years; 85.9% male [n=2,652]; 73.1% aged ≥65 years [n=2,255]). The overall incidence of AEs was 28.8% (n=890), of which 2.2% (n=67) were ADRs. Serious AEs and UAEs were reported in 181 (5.9%) and 665 (21.6%) patients, respectively, and two patients (<0.1%) reported unexpected severe ADR. Of the 903/3,086 patients analysed for effectiveness, most (82.8%, n=748) showed overall disease improvement after UMEC/VI treatment. Conclusion This study confirmed UMEC/VI administered to Korean patients according to the prescribing information was well-tolerated and can be considered an effective option for COPD treatment.

PURPOSE: The purpose of this study is to investigate clinical features and causative organisms in febrile infants younger than three months, to help identification of high risk patients for serious bacterial infection (SBI). METHODS: A total of 313 febrile infants younger than three months, who had visited Seoul National University Children's Hospital from January 2008 to December 2010 were included. Clinical features, laboratory findings, causative organisms, and risk factors of SBI were analyzed by retrospective chart review. Causative bacterial or viral pathogens were identified by gram stain and cultures, rapid antigen tests, or the polymerase chain reaction from clinically reliable sources. RESULTS: Among 313 infants, etiologic organisms were identified in 127 cases (40.6%). Among 39 cases of bacterial infections, Escherichia coli (66.7%) and Streptococcus agalactiae (12.8%) were common. Enterovirus (33.7%), respiratory syncytial virus (19.8%), and rhinovirus (18.8%) were frequently detected in 88 cases of viral infection. Patients with SBI (39 cases) showed significantly higher values of the white blood cell count (14,473+/-6,824/mm3 vs. 11,254+/-5,775/mm3, P=0.002) and the C-reactive protein (6.32+/-8.51 mg/L vs. 1.28+/-2.35 mg/L, P<0.001) than those without SBI (274 cases). The clinical risk factors for SBI were the male (OR 3.7, 95% CI 1.5-8.9), the presence of neurologic symptoms (OR 4.8, 95% CI 1.4-16.8), and the absence of family members with respiratory symptoms (OR 3.6, 95% CI 1.2-11.3). CONCLUSION: This study identified common pathogens and risk factors for SBI in febrile infants younger than three months. These findings may be useful to guide management of febrile young infants.

The clinical and mycological study was made with 38 cases of dermatophytoses in infants and preschoolers in dermatologic clinic of St. Pauls Hospital, Catholic Medical College, from September, 1982 to April, 1984. Following results are obtained: The incidence of dermatophytoses under the age 6 was 2. 2% among 1, 711 out- patients of the sarne age group. 2. The incidence of involving sites was the highest in tinea(T.) corporis(37. 2%) and T. faciale(37, 2%) and then T. capitis(18. 7g). In 0-under 2 groups, the highest incidence in T. corporis(54. 5%) and then T. faciale(3l. 8%.). In 4-under 5 group, the highest incidence in T. capitis(55. 6%). 3 Fungi cultured from their skin lesions were 36 strains belonging to 6 species. These were M. canis(55. 6%), T. rabrum(27. 8%), granular type of T.mentagrophyte(8.3%) and interdigital type of T. mentagrophyte(27. 8%), M gypseum (2.8%). and E. floccosum(2.8%) in order. 4. Arnong 1Q strains of T. rabrum, anthropophilic fungi, 8 strains(80. 0%) were frorn 0-under 2 age groups, E. floccosum, and interdigital type of T. mentagrophyte were also from 0-under 1 age group. Among ZO strains of M, canis, zoophilic fungi, 13 strains(65. 0%) were from the age groups of 2-under 5. Granular type of T. mentagrophytc, 2 among 3 strains were from 4-under 5 age group, 5. The incidence of causative fungi in each sites were as follows: In T. corporis, T. rubrum(50. O%), and M canis(18. 8%) in order. In T. faciale, M. canis(62. 5%), T. rabrum(12. 5%), T. mentagrophyte(12. 5%), and M. gypseum(6. 3%) in order. In T. capitis, all cultured fungi were M. canis
